Physical Training in Parkinson's Disease: A Systematic Review of Neurobiological Mechanisms, Brain-Muscle Adaptations, and Implications for Personalized Rehabilitation.

BACKGROUND: Physical Training (PT), defined a planned, structured, and long-term exercise intervention, is recognized as an important non-pharmacological therapy in Parkinson's disease (PD). Beyond its symptomatic benefits, experimental evidence suggests that PT may also modulate pathological mechanisms involved in dopaminergic neurodegeneration. This systematic review summarizes the neurobiological and neurophysiological adaptations induced by PT in animal models of PD and in patients with PD, and proposes mechanistically driven recommendations for personalized exercise prescription. SUMMARY

Vitamin D receptor FokI polymorphism preferentially modulates cognitive vulnerability in Parkinson's disease: Evidence from an Indian cohort.

INTRODUCTION: Vitamin D deficiency (VDD) and neuroinflammation are emerging as key contributors in the pathophysiology of Parkinson's disease (PD). Characterized by both motor and non-motor symptoms, PD is marked by dopaminergic neuronal loss and α-synuclein accumulation. VD exerts neuroprotective effects by preserving dopaminergic neurons, enhancing neurotransmission, and reducing neuroinflammation. Its biological activity is primarily mediated through the Vitamin D Receptor (VDR), which functions as a transcription factor upon activation. This study investigates ethnic variability of two imp

Neuropsychiatric symptoms and apolipoprotein E genotypes in neurocognitive disorders.

Complex genetic relationships between neurodegenerative disorders and neuropsychiatric symptoms have been shown, suggesting shared pathogenic mechanisms and emphasizing the potential for developing common therapeutic targets. Apolipoprotein E ( APOE ) genotypes and their corresponding protein (ApoE) isoforms may influence the biophysical properties of the cell membrane lipid bilayer. However, the role of APOE in central nervous system pathophysiology extended beyond its lipid transport function. In the present review article, we analyzed the links existing between APOE genotypes and the neurob

Autonomic Dysfunction in Patients With Dementia With Lewy Bodies and Its Relationship With Nigrostriatal Denervation.

BACKGROUND AND OBJECTIVES: Diagnosing dementia with Lewy bodies (DLBs) is challenging because of symptom overlap with other neurodegenerative diseases. Although dysautonomia is a recognized supportive diagnostic criterion, its prevalence and extent remain underexplored. We aimed to evaluate autonomic dysfunction in patients with DLB using a comprehensive battery of autonomic function tests (AFTs) and to investigate whether this dysfunction differs between patients with and without nigrostriatal denervation. METHODS: This prospective cohort study was performed at a Memory Clinic in Rome, Italy

CNV-Finder: Streamlining Copy Number Variation Discovery.

Copy Number Variations (CNVs) play pivotal roles in the etiology of complex diseases and are variable across diverse populations. Understanding the association between CNVs and disease susceptibility is significant in disease genetics research and often requires analysis of large sample sizes. One of the most cost-effective and scalable methods for detecting CNVs is based on normalized signal intensity values, such as Log R Ratio (LRR) and B Allele Frequency (BAF), from Illumina genotyping arrays. In this study, we present CNV-Finder, a novel pipeline integrating deep learning techniques on ar

Alcohol Use Disorder and Dementia: A Review.

PURPOSE: By 2040, 21.6% of Americans will be over age 65, and the population of those older than age 85 is estimated to reach 14.4 million. Although not causative, older age is a risk factor for dementia: every 5 years beyond age 65, the risk doubles; approximately one-third of those older than age 85 are diagnosed with dementia. As current alcohol consumption among older adults is significantly higher compared to previous generations, a pressing question is whether drinking alcohol increases the risk for Alzheimer's disease or other forms of dementia. SEARCH METHODS: Databases explored inclu

The Therapeutic Implications of Tea Polyphenols Against Dopamine (DA) Neuron Degeneration in Parkinson's Disease (PD).

: Accumulative evidence indicated that the pathologically accumulated metal ions (iron species and Mn3+) and abnormally up-regulated monoamine oxidase B (MAOB) activity induced oxidation of endogenous dopamine (DA) can lead to mitochondria impairment, lysosome dysfunction, proteasome inhibition, and selective DA neuron vulnerability, which is implicated in the pathogenesis of Parkinson's disease (PD). The DA oxidation can generate deleterious reactive oxygen species (ROS) and highly reactive DA quinones (DAQ) to induce DA-related toxicity, which can be alleviated by DA oxidation suppressors, R

Atherosclerotic-dose TMAO accentuates redox imbalances and motor dysfunctions in a MPTP mouse model of Parkinson's disease.

Parkinson's Disease (PD) involves the loss of dopaminergic neurons and the formation of Lewy bodies consisting of alpha-synuclein (αSin) aggregates. Trimethylamine N-oxide (TMAO), a gut microbiota metabolite, has emerged as a molecule of interest due to pro-inflammatory, pro-oxidative, and neurodegenerative effects. We aim to assess whether lower doses of TMAO (40 mg/kg bw, gavage, once daily, day 0-42), as those used in atherosclerosis experimental models, can exacerbate oxidative stress, neuroinflammation, motor dysfunction, and neurodegeneration in a semi-acute 1-methyl-4-phenyl-1,2,3,6-tet

Norepinephrine modulates the microglial response and protects against MPTP-induced Parkinson's disease pathology through regulation of the PI3K/AKT/mTOR pathway.

AIMS: Norepinephrine (NE) deficiency is an early and prominent feature of Parkinson's disease (PD), but its specific contributions to pathogenesis have not been fully elucidated. This study aims to determine whether NE exerts neuroprotective effects in PD models through the modulation of autophagy and microglial activation mediated by the PI3K/Akt/mTOR pathway. METHODS: C57BL/6 mice were randomly assigned to four experimental groups: control group, MPTP (1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine)-induced PD model group, MPTP + DOPS (droxidopa) treatment group, and MPTP + DOPS +3-MA (3-meth

Imposter in the brain: aetiological, clinical and neuroimaging characteristics of Capgras syndrome.

Capgras syndrome is a delusional misidentification syndrome characterized by the recurrent belief that someone, usually a family member, has been replaced by an impostor. Although described over a century ago, its aetiology, clinical features and neuroimaging characteristics remain poorly understood due to its rarity. This study aimed to clarify these aspects through the analysis of a large cohort and to explore its clinical implications and underlying mechanisms. We conducted a retrospective cohort study by reviewing medical records of patients diagnosed with Capgras syndrome at the Mayo Clin

Oxidative stress and inflammation in neurodegenerative disorders.

The brain's consumption of approximately 20% of the body's oxygen contributes to oxidative stress, a significant pathological factor in neurodegenerative diseases such as Alzheimer's disease, Parkinson's disease, Huntington's disease, and amyotrophic lateral sclerosis. This oxidative stress, linked to low levels of antioxidant enzymes, drives neuronal death by facilitating membrane peroxidation of fatty acids, proteins, and DNA. Alzheimer's disease is characterized by amyloid-beta (Aβ) plaque accumulation and hyperphosphorylated tau aggregates, both of which interact with mitochondria to gener

CHCHD2 and CHCHD10 promoted autophagic clearance of protein aggregates via GABARAPs.

Mutations in mitochondrial protein CHCHD2 and its paralog CHCHD10 were identified in patients with Parkinson disease (PD), amyotrophic lateral sclerosis (ALS), frontotemporal dementia (FTD) or Alzheimer disease (AD). CHCHD2 and CHCHD10 mutations caused neurodegeneration in model animals as seen in patients, but their pathophysiological roles remain elusive. Here we reported a direct role of CHCHD2 and CHCHD10 in autophagy. We identified a protein complex composing of CHCHD2-CHCHD10-C1QBP/p32-Atg8-family proteins (ATG8s), in which each molecule interacted with another. CHCHD2, CHCHD10 and C1QBP

More fit KL-VS heterozygotes have more favorable AD-relevant biomarker profiles.

INTRODUCTION: Although hallmarked by β-amyloid plaques (Aβ) and neurofibrillary tangles (tau), Alzheimer's disease (AD) is a multifactorial disorder that involves neuroinflammation, neurodegeneration, and synaptic dysfunction. AD-associated biomolecular changes seem to be attenuated in carriers of the functionally advantageous variant of the KLOTHO gene (KL-VSHET). Independently, better cardiorespiratory fitness (CRF) is associated with better health outcomes related to AD pathology. Here we investigate whether the relationships between CRF (peak oxygen consumption (VO2peak)) and cerebrospinal

Hypothermia Shifts Neurodegeneration Phenotype in Neonatal Human Hypoxic-Ischemic Encephalopathy but Not in Related Piglet Models: Possible Relationship to Toxic Conformer and Intrinsically Disordered Prion-like Protein Accumulation.

Hypothermia (HT) is used clinically for neonatal hypoxic-ischemic encephalopathy (HIE); however, the brain protection is incomplete and selective regional vulnerability and lifelong consequences remain. Refractory damage and impairment with HT cooling/rewarming could result from unchecked or altered persisting cell death and proteinopathy. We tested two hypotheses: (1) HT modifies neurodegeneration type, and (2) intrinsically disordered proteins (IDPs) and encephalopathy cause toxic conformer protein (TCP) proteinopathy neonatally. We studied postmortem human neonatal HIE cases with or without

The Yin and Yang of Microglia-Derived Extracellular Vesicles in CNS Injury and Diseases.

Microglia, the resident immune cells of the central nervous system (CNS), play a crucial role in maintaining neural homeostasis but can also contribute to disease and injury when this state is disrupted or conversely play a pivotal role in neurorepair. One way that microglia exert their effects is through the secretion of small vesicles, microglia-derived exosomes (MGEVs). Exosomes facilitate intercellular communication through transported cargoes of proteins, lipids, RNA, and other bioactive molecules that can alter the behavior of the cells that internalize them. Under normal physiological c
