HYPOTHESIS: It is unknown whether certain methods of distal biceps tendon repair lead to an increased propensity of impingement of the repaired tendon. The purpose of this study was to evaluate various repair techniques in a cadaveric model to determine the radioulnar space available for the repaired biceps tendons.
METHODS: Nine matched pairs of quartered, fresh-frozen cadaveric arms were transected at the level of the humeral mid shaft and the distal radiocarpal joint. Distance measurements and the angular relation of the bicipital tuberosity were measured at 5 forearm pronation-supination

BACKGROUND: The subscapularis is an important mover and stabilizer of the glenohumeral joint. Since the advent of shoulder arthroscopy, partial tears are found in 43% of rotator cuff patients. While partial tears to the upper band occur more commonly, little is known about the structure and mechanical behavior of the individual bands. Therefore, the objective of this study was to measure tensile mechanical properties, corresponding collagen fiber alignment, and histology in the upper and lower bands of the rat subscapularis tendon.
MATERIALS AND METHODS: Thirty adult Sprague-Dawley rats were

The Achilles tendon is the most frequently ruptured tendon in the lower limb and comprises approximately 20% of all large tendon ruptures.Spontaneous rupture of the Achilles tendon is usually observed in healthy, active individuals in the third to fifth decades of life and is twice as common in males. It usually occur following high-performance activities. The authors report a case of unusual presentation of bilateral simultaneous Achilles tendon ruptures that occurred following a low-effort-level sport and in the absence of risk factors. In the case presented, nonoperative treatment led to a s

Elastic stable intramedullary nail fixation has become established as an acceptable method of treatment for diaphyseal fractures of both forearm bones in the paediatric population. It is considered safe, minimally invasive and does not compromise physeal growth. We report a case of delayed rupture of extensor pollicis longus due to attrition over the sharp edges of a protruding nail end after elastic stable intramedullary nailing of a paediatric radial diaphyseal fracture.

Ankle instability in the athlete is a common problem that is routinely treated non-operatively, with a 90% success rate. With proprioceptive training, preventive equipment (bracing/taping), and closed kinetic chain strengthening, surgery for ankle instability is uncommon. Nonetheless, some athletes present with recurrent ankle instability that, despite work-up and conservative treatment, requires surgical correction. The use of a primary ligament repair (Brostrom procedure) versus augmented (anatomic) reconstructions is discussed in detail in this article.

PURPOSE: Tissue-engineered tendon grafts will meet an important clinical need. To engineer tendons, we used acellularized allogeneic tendon as scaffold material. To determine the ideal cell type to seed the scaffolds, we studied in vitro characteristics of epitenon tenocytes, tendon sheath fibroblasts, bone marrow-derived mesenchymal stem cells (BMSCs), and adipoderived mesenchymal stem cells (ASCs). Subsequently, we implanted reseeded acellularized tendons in vivo as flexor tendon grafts.
METHODS: Tenocytes, sheath fibroblasts, BMSCs, and ASCs were obtained from adult rabbits. For all cell l

BACKGROUND: While a great deal is known regarding the performance of muscle with intact tendon, little is known about muscle performance when tendon is surgically lengthened or shortened. This knowledge may allow surgeons to more accurately predict functional outcome following tendon repair when correcting a simple tendon laceration or performing a more complex vascularized neuromuscular transfer.
MATERIALS AND METHODS: We studied muscle performance 12 wk following extensor tendon repairs producing altered tendon lengths. Forty male Fischer 344 rats underwent division of the proximal and dist

The Anterior Cruciate Ligament (ACL) is regarded as critical to the normal functioning of the knee, its disruption causing functional impairment. In recent years central third of the patellar tendon (PT) and combined Semitendinosis and gracilis tendons (HT) have become the most frequently used graft types for anterior cruciate knee ligament reconstruction. For the past two decades, the gold standard in ACL reconstructions has been the PT, but increasingly the HT graft has been used. This shift in popularity has occurred for several reasons, including concerns about damaging the knee extensor a

BACKGROUND: Controversy regarding the optimal treatment of the fresh total Achilles tendon rupture remains.
PURPOSE: To compare the results of percutaneous and open Achilles tendon repair.
STUDY DESIGN: Cohort study; Level of evidence, 2.
METHODS: The results of 132 consecutive patients with acute complete Achilles tendon rupture who were operated on exclusively with modified percutaneous repair under local anesthesia from 1991 to 1997 and followed up for at least 2 years were compared to the results of 105 consecutive patients who were operated on exclusively with open repair under general

We studied 101 patients who were operated on at Oulu University Hospital for ruptured Achilles tendon from 1987 to 1992 (inclusive), and 87 healthy control patients from among Army conscripts. The mean inequality in length of legs (ILL) was 5 +/- 4 mm. Among the 48 patients with ILL > or = 5 mm, the side affected with ruptured tendon was longer in 48% of cases and shorter in 52%. An underpronating alignment of the ankle and foot (based on beta45 measurement of the angle < or = 4 degrees) was found in 21% of patients and 5% of controls (P < 0.001). A combination of high longitudinal arch and un

We analysed 32 patients with fresh Achilles tendon rupture. 30 operations with a special primary suture and plasty of the fascia triceps muscle. Most of the patients were content with the operative treatment and the functional result. We saw one wound infection and one wound rand necrosis. The objective controls of OSG foot function and the muscle activity is good.

Introduction/PurposeDelayed treatment of acute Achilles tendon rupture is associated with worse outcomes and higher complication rates. Prior studies suggest reduced access to care among Medicaid and uninsured patients. This study examines the effect of insurance status on the timing of diagnosis and surgical treatment among patients undergoing surgical repair of acute Achilles tendon rupture.MethodsA retrospective chart review was conducted of patients aged ≥18 years with acute traumatic Achilles tendon ruptures identified by Current Procedural Terminology codes 27650 and 27654 at 2 hospitals

Athletic injuries in or around the hip in the adolescent athlete encompass possible causes such as a single, traumatic event to those of repetitive microtrauma. The injuries may involve the bone or the soft tissues, with former involving the epiphysis, apophysis, metaphysis, or diaphysis, whereas the latter includes muscles and tendons. With the improvements in surgical technique and instrumentation for hip arthroscopy and the development of magnetic resonance arthrography, clinicians have been able to diagnose and treat labral tears, hip instability, snapping hip, loose bodies, chondral injur

BACKGROUND: Animal models of muscle injury have primarily relied on methods which do not mimic the chronic scarring that typically occurs adjacent to the myotendinous junction (MTJ). The goal of this study was three-fold: (i) to create a strain-induced in vivo model of rectus femoris MTJ injury in rats; (ii) to document clinical manifestations of injury using longitudinal tracking of individual animals via voluntary and compulsory (treadmill) mobility analyses and (iii) to validate and assess the model for persistent scarring through serial histologic assessment and development of a semi-quant

Complex injuries of the distal forearm and the hand by industrial machines often require extensive reconstructive procedures. Crush injuries with soft tissue damage extending over the limits of visible injury require a special approach. Large soft tissue defects often have to be covered by extensive flap procedures. The optimum point of time for plastic reconstruction is an essential question and represents a controversial issue in the literature. To be able to compare different patient cohorts in the future, we classify crush injuries into five clinically relevant categories. In the course of

Fourteen patients with ring avulsion injuries were treated at the University of Ioannina from 1982 to 1991. One patient was classified as Urbaniak class II, and microsurgical repair produced good functional results. The remaining 13 patients had class III injuries. Six of these patients had class IIIa injuries, defined as skin avulsions at the level of the proximal phalanx, amputation at the distal interphalangeal joint with an intact flexor digitorum superficialis. Replantation was successful in four patients. Seven patients had class IIIb injuries, defined as skin avulsion and complete amput

BACKGROUND: Surgical repair of full thickness biceps tears has demonstrated adequate outcomes in short and mid-term studies. However, data on the long-term outcomes of full thickness distal biceps injuries and their treatment are currently lacking.
PURPOSE/HYPOTHESIS: The purpose of this study was to report on patient demographics, injury characteristics, and long-term outcomes for patients with full-thickness distal biceps tears. It was hypothesized that complete distal biceps tears managed operatively would demonstrate robust clinical success at long-term follow-up.
