Advanced age has traditionally been a contraindication to cardiac transplantation. We have, however, offered cardiac transplantation to patients older than 60 years with end-stage heart failure if they were otherwise acceptable candidates. Zygomycosis is a rare but highly invasive fungal infection that occurs in transplant recipients. We report a case of invasive gastrointestinal zygomycosis that occurred in a heavily immunosuppressed liver transplant recipient 5 days after retransplantation and that presented as gastric perforation. The International Bone Marrow Transplant Registry (IBMTR) receives and analyses detailed information contributed by transplant teams at more than 175 institutions worldwide. This collaborative research effort has grown rapidly; there are now more than 8000 cases in the database. This is a summary of the current status of bone marrow transplantation in leukemia and severe aplastic anemia, a brief summary of key findings reported by the IBMTR during the past year, as well as studies planned for the coming year.

In most centers allogeneic bone marrow transplantation is restricted to patients with HLA-identical siblings as donors. We have transplanted 16 patients with marrow of donors other than HLA-identical siblings. Seven patients were grafted in the years 1978 until 1984. Six died of transplant complications and one of recurrent leukemia. Natural killer activity against herpes simplex virus type 1 infected fibroblasts NK(HSV-1) was studied prospectively in patients undergoing allogenic bone-marrow or fetal-tissue stem-cell transplantation.
