Mechanisms Matter: Intentional Injury and Its Impact on Pediatric Facial Fractures.

INTRODUCTION: Pediatric facial fractures resulting from violent mechanisms (eg, assault, gunshot wounds, and stabbings) represent a distinct subset of trauma with unique epidemiologic and clinical implications. This study evaluates how violent mechanisms influence pediatric facial fracture patterns, management, and demographic disparities. METHODS: The authors retrospectively reviewed patients younger than 18 years with facial fractures, comparing violent mechanisms (VM) and nonviolent mechanisms (NVM). VM were defined as intentional injury inflicted by others or oneself. Patients were treate

Outcomes in the Older Appalachian Trauma Population After Falls Caused by Dogs: Who Let the Dogs Out?

Introduction Falls continue to be among the leading causes of nonfatal injuries in the United States. Falls are common among the older population, and the Appalachian region is considered one with an aging population. Pet ownership is common and has even increased since 2020. Few studies exist on the topic of falls caused by dogs. Methods Data were retrospectively pulled from the trauma registry at a rural Appalachian Level 1 trauma center between January 1, 2017, and June 15, 2022. Patients ≥18 who presented to the facility due to falling or tripping caused by dogs were included. Injuries due

Thrombin-Antithrombin Complex and Fibrinogen Predict Microangiopathic Acute Kidney Injury in Injured Patients.

Acute kidney injury (AKI) develops in 20-40% of critically injured patients and contributes to poor outcomes. In sepsis and critical illness, dysregulated coagulation and fibrinolysis are linked to microangiopathic kidney dysfunction, but similar relationships in trauma are not well characterized. We performed a secondary analysis of a single-center randomized controlled trial of severely injured adult trauma patients. Plasma thrombin-antithrombin (TAT), plasmin-2-antiplasmin (PAP), and fibrinogen were measured within 2 hours of injury and 6 hours later. The primary outcome was AKI, defined b

The cortisol stress response following surgery for proximal femur fractures in geriatric patients - a prospective pilot study.

INTRODUCTION: Aging impairs the suprachiasmatic nucleus function, compromising the adrenocortical circadian rhythmicity and, potentially, affecting the cortisol stress response. Relative cortisol insufficiency may contribute to peri- and postoperative complications and increase mortality. While proximal femoral fractures in geriatric patients are among the most common musculoskeletal injuries, little is known about the adrenal stress response in geriatric patients undergoing surgery for this injury. METHODS: The present prospective pilot study investigated the postoperative cortisol levels in

In Vivo Characterization of Fatty Acid Amide Hydrolase in Posttraumatic Stress Disorder: A [11C]CURB Positron Emission Tomography Study.

BACKGROUND: Posttraumatic stress disorder (PTSD) is associated with impaired fear extinction and dysfunction in amygdala-centered circuits, for which treatments remain insufficient. The endocannabinoid system, particularly anandamide signaling regulated by fatty acid amide hydrolase (FAAH), has been implicated in stress and fear regulation. Preclinical studies suggest increased FAAH is linked to heightened stress responsivity, but FAAH status in PTSD remains unclear. We used [11C]CURB positron emission tomography (PET) to characterize FAAH levels in the amygdala and in corticolimbic regions in

Tension Viscerothorax with Concomitant Pneumothorax and Associated Intrathoracic Gastric Perforation after Thoracoabdominal Stab Wound in a Resource-Limited Setting: A Case Report.

INTRODUCTION: Traumatic diaphragmatic injury after penetrating thoracoabdominal trauma is rare and can be missed when CT is not available. Tension viscerothorax can mimic tension pneumothorax and may persist despite pleural decompression. CASE PRESENTATION: A 28-year-old man suffered a knife stab to his left posterior thoracoabdomen. A 28-French left chest tube inserted at a local health center for a suspected tension pneumothorax (without imaging) did not improve the patient's condition, and later drained the gastrointestinal (GI) content. Upon arrival, chest radiography revealed a left pneu

The long shadow of 9/11: Mental health outcomes in adult children of World Trade Center Responders with PTSD.

Intergenerational transmission of trauma (ITT) has been documented among offspring of parents exposed to war, genocide, and interpersonal violence, yet little is known about ITT among families of World Trade Center responders (WTC-R). The aftermath of 9/11 terrorist attacks provides a unique naturalistic context for examining long-term psychological outcomes among now-adult offspring raised during periods of parental trauma exposure and recovery. The WTC-R cohort comprised traditional first responders (i.e., law enforcement officers) and nontraditional recovery workers, among whom those with P

A single-center experience in the management of combat-related extremity vascular injuries.

OBJECTIVE: Combat-related vascular injuries account for up to 12% of trauma cases, with peripheral vessel injuries being the most prevalent among casualties. The ongoing military conflict in the Middle East has led to the widespread use of explosive devices and missiles, resulting in a high incidence of peripheral vascular trauma. METHODS: This study presents a retrospective single-center analysis of all patients with peripheral vascular trauma admitted to our facility between October 2023 and November 2024. Data on patient demographics, injury patterns, surgical interventions, and outcomes w

Incorrect blood typing and mis-transfusion due to low-titer group O whole blood resuscitation.

BACKGROUND: Resuscitation strategies for massive hemorrhage increasingly involve the use of low-titer group O whole blood (LTOWB) due to evidence suggesting improved patient outcomes. However, the potential risk of incorrect ABO typing following LTOWB administration, possibly leading to mis-transfusion, remains insufficiently explored. This case series aims to highlight the potential risk of ABO mistyping associated with LTOWB transfusions in trauma settings. STUDY DESIGN AND METHODS: We retrospectively reviewed three cases involving trauma patients who received LTOWB transfusions at a high-v

Outcome of femoral fractures treated with cerclages and intramedullary nailing.

INTRODUCTION: Femoral shaft fractures are a common entity in trauma surgery. The gold standard is closed reduction and intramedullary nailing. However, in some fracture patterns, the surgeon may choose to open the fracture for reduction. The purpose of this retrospective study was to analyze the outcome of femoral shaft fractures treated with open reduction, cerclage wiring and intramedullary nailing. METHODS: We included adult patients with femoral shaft fractures treated with open reduction, cerclage wiring and intramedullary nailing at a level 1 trauma center in Switzerland during an 11-ye

Injuries from blank cartridge shots in suicide attempts within the South Korean military: a case series of five patients.

PURPOSE: Blank cartridges are designed to produce the sound and gas flare of a gunshot without firing a bullet. However, blank cartridge shots (BCS) can still cause injuries ranging from minor to life-threatening. Within the South Korean military, most BCS-related injuries have occurred during suicide attempts. This study documents a case series of patients who sustained injuries from BCS during such attempts. METHODS: We examined the medical records of Korean soldiers at a military trauma center between April 2022 and April 2024 who had sustained injuries from BCS during suicide attempts. Th

Neural processes of emotional conflict detection and prediction of posttraumatic stress disorder symptom clusters in traumatic injury survivors.

OBJECTIVE: Given the prevalence and significant burden of posttraumatic stress disorder (PTSD), identifying early predictors of symptom development following trauma is critical. PTSD is a heterogeneous disorder comprised of distinct symptom clusters-reexperiencing, avoidance, negative mood, and hyperarousal-that contribute to the broad range of possible symptom profiles. Affective and attentional regulation processes, such as emotional conflict detection, are impaired in individuals with PTSD; however, the neural mechanisms underlying these alterations and their predictive utility for the deve
