[Is hearing preservation feasible with the retrosigmoid approach?].

The main advantages of the retrosigmoid approach are fast and good visualization of the brainstem structures and the possibility to remove tumors of all sizes. Hearing preservation, although possible, is not always achieved. The aim of this study is to present our experience with the retrosigmoid approach for acoustic neuroma resection, emphasizing our hearing results. The notes of 56 cases of acoustic neuroma removed through a the retrosigmoid approach were reviewed. Tumor size ranged from 8 to 50 mm (mean 24 mm). Considering patients with preoperative serviceable hearing, postoperative servi

Vocal fold atrophy: quantitative glottic measurement and vocal function.

Videostroboscopic glottic measurements and vocal function were evaluated in 41 vocal fold atrophy patients with bowed vocal folds. The amount of bowing in the resting position and the glottal gap area and vibratory amplitude during phonation were measured from digitized videostroboscopic images. Vibratory amplitude was not decreased on atrophic vocal folds. With the same amount of total bowing, the glottal gap area for bilateral atrophy was smaller than for unilateral atrophy. These results suggest that vocal fold atrophy is not disadvantageous to thyroplasty type I, and that bilateral procedu

Auditory deficits and hearing loss associated with focal brainstem haemorrhage.

Four cases of central pontine haemorrhage are described in which auditory dysfunction was documented. Two cases had a hearing loss, in one of which there was recovery of the low frequencies. This case provides support for the tonotopic organization of the auditory pathways in the caudal pontine area, with the lowest frequencies being encoded medially. In all cases, there were abnormalities of the auditory brainstem responses, wave V being consistently involved, while wave III was abnormal in only one patient. In three cases, the masking level differences and crossed acoustic reflex thresholds

[Harmonic/noise ratio and spectrographic analysis in vocal abuse pathology].

Vocal abuse lesions disturb voice production by increasing the presence of noise in the voice. The harmonic-to-noise ratio (H/N) and spectrography are two different ways to evaluate noise components in the voice. A study was made of 52 patients (32 polyps, 11 Reinke's edema and 9 nodules) who underwent microlaryngeal phonosurgery. The grade of glottic closure was determined by stroboscopy; H/N and the long-term average spectrum (LTAS) at low-frequency (0-2000 Hz) were measured with a CSL-4300 instrument. Stroboscopy and acoustic analysis were made before surgery and two week and one month afte

Measures of vocal function during changes in vocal effort level.

The purpose of this article is to present the results of a controlled study of the day-to-day variabilities of three acoustic parameters (jitter, shimmer, and normalized noise energy), and two electroglottographic parameters (contact quotient and contact quotient perturbation) for vowels produced at three vocal efforts (low, normal, high). Data were obtained with use of a sophisticated bilinear interpolation pitch detection method. A repeated measures design required subjects to produce the vowels /ae/ and /a/ five times a day over 3 days at each vocal effort level. The jitter, shimmer, and no

Gait analysis in patients with vertigo.

Gait analysis utilizing foot switches and electromyography of antigravity muscles was performed on 31 patients with vertigo. Twenty-one cases had peripheral vestibular lesions (such as vestibular neuronitis, Menière's disease, traumatic labyrinthitis, small acoustic neuroma), and 10 cases had central lesions (such as spinocerebellar degeneration, pontine glioma, cerebellar tumor and giant acoustic neuroma). Parameters employed for analysis included time from heel strike to forefoot strike (HA-I), time from heel off to forefoot off (HA-II) as well as durations of stance, swing and (initial) dou

Changes in voice characteristics after uvulopalatopharyngoplasty.

Previous reports have warned that tonsillectomy or uvulopalatopharyngoplasty (UPPP) may alter patients' speech by increasing the amount of nasal resonance as well as by changing voice timbre due to enlargement of the vocal tract. However, very few objective investigations, excluding nasality problems, have been carried out. We studied eight patients who underwent surgery for UPPP and recorded pre- and postoperative speech. The speech samples were then rated by seven experienced listeners, and acoustic spectra of two long vowels,/a/ and /e/, were analyzed using a computer program (MacSpeech Lab

Ventricular dysphonia: a profile of 40 cases.

Ventricular dysphonia is a poorly understood disorder involving ventricular fold participation during phonation. A population of ventricular dysphonia patients was evaluated using phonatory function studies such as laryngovideostroboscopy, advanced acoustic analysis, and electroglottography to identify shared epidemiologic characteristics and to discuss possible neuromuscular mechanisms and causes. Forty patients with ventricular dysphonia were studied and epidemiologic, acoustic, and histologic data were analyzed. In almost all cases, the authors found abnormalities affecting the glottis caus

[Evoked auditory potentials in neonatal hyperbilirubinemia].

Neonatal hyperbilirubinemia (greater than 20 mg%) is an audiological risk factor. In order to check early detection of bilirubin-induced neurological damage in the brainstem, 19 newborns were enrolled in the present study. The criteria was a bilirubin level ranging from 12 to 20 mg%. A likely mechanism for bilirubin intoxication may be the slowing and desynchronization of acoustic stimuli in the brainstem. Auditory brainstem responses were performed using non filtered clicks at 100 dB SPL (peak equivalent). Absolute and interpeak latencies of waves I and V were measured and correlated to bilir

The role of pars flaccida in human middle ear sound transmission.

The role of the pars flaccida in middle ear sound transmission was studied with the use of twelve otoscopically normal, fresh, human temporal bones. Peak-to-peak umbo displacement in response to a constant sound pressure level at the tympanic membrane was measured with a noncontacting video measuring system capable of repeatable measurements down to 0.2 micron. Measurements were made before and after pars flaccida modifications at 18 frequencies between 100 and 4000 Hz. Four pars flaccida modifications were studied: (1) acoustic insulation of the pars flaccida to the ear canal with a silicone

Serial changes in sonographic appearance after transcatheter hepatic arterial embolization.

Serial changes in sonographic appearance after transcatheter hepatic arterial embolization (TAE) were studied in 22 patients who had hepatoma and one patient who had a hepatoblastoma. These changes were classified into three types. In cases of type 1, the internal echo of the entire tumor became remarkably echogenic with or without an acoustic shadow. In cases of type 2, echogenic areas or scattered echogenic spots with or without acoustic shadows were observed. In Type 3 cases, no changes in sonographic appearance were observed. Tumor diameters in all of the type 1 cases were less than 4.9 cm

[Using brainstem auditory evoked potentials in cerebellopontine angle tumors (author's transl)].

The brainstem electric responses on acoustic stimulation are recorded on 37 patients with verified cerebellopontine angle tumors. These responses are compared with those of 101 normal hearing young adults and those of 47 patients with cochlear hearing loss. The Interval between the latency of the first wawe (acoustic nerve) and the fifth (midbrain) or I-V delay is related to the lesion location. The I-V delay is increased in retro-cochlear hearing loss and not different of the normal in cochlear hearing loss. The delay variation is best seen by comparing the I-V delay for the two ears. The int

Physiological aspects of the effects of sound on man and animals.

A review of some short-term and long-term physiological effects of sound on non-auditory body functions is given. It is pointed out that the short-term effects depend closely on the acoustic properties of the sound. Habituation is rapid for steady signals but slow for interrupted ones. Irrelevant, meaningless sound, presented to rats over their life-time in such a way as to simulate an industrial acoustic environment, did not affect blood pressure, life-span or morbidity incidence. As far as the injurious effect of sound on the inner ear is concerned, it was shown that spontaneously hypertensi

Sound localization: value in localizing lesions of the auditory pathway.

Reports in the literature suggest that patients with otosclerosis, neural lesions such as acoustic neuroma, pontine lesions, and temporal lobe lesions may show abnormal sound localization ability. In the present experiment, 15 normal subjects and 22 patients with various auditory pathway lesions had sound localization testing using special apparatus. The results confirmed that patients with otosclerosis were unable to localize low frequency sounds. Those with temporal lobe lesions made judgments not significantly different from normal subjects. Patients with unilateral sensorineural deafness m

Transtympanic electrocochleography in the diagnosis of retrocochlear tumours.

Transtympanic electrocochleography has been used as a diagnostic procedure in over 250 adult patients with sensorineural deafness. No complications have been encountered. Among these patients vestibulocochlear Schwannoma (acoustic neuroma) has been confirmed in 56 ears while 11 other ears have been found to be affected by other space-occupying lesions. As a result of our observations in these patients, we would like to propose that there are at least three separate criteria to be considered in reaching or strongly suspecting a diagnosis of such pathology. These are broadening of the action pot

A Voice Ergonomic Study in Icelandic Preschools: What Causes the Activity Noise and How to Attenuate It.

INTRODUCTION: Preschool sound environments are predominantly shaped by activity noise generated by voices, object impacts, furniture movement, and daily transitions. Although Icelandic preschools typically comply with reverberation standards, staff frequently report excessive noise, high listening effort, and vocal strain. METHODS: This mixed-method pre-post study examined the sources and reducibility of activity noise in three Icelandic preschools using a voice-ergonomic framework. Data included objective noise measurements at child ear height, structured observations, and teacher questionna

Utility of Unified Spasmodic Dysphonia Rating Scale for Adductor Type Laryngeal Dystonia (AdLD).

OBJECTIVE: The Unified Spasmodic Dysphonia Rating Scale (USDRS) was developed in 1997 to standardize evaluation of adductor-type laryngeal dystonia (AdLD) but has been sparsely utilized since then. The goal of this study was to explore the descriptive value and contemporary relevance of the USDRS in characterizing AdLD by examining its relationships with established patient-reported, clinician-perceptual, and acoustic measures. METHODS: Patients with stable-dose, botulinum toxin-A (BtxA)-responsive AdLD presented to a tertiary laryngology center for follow-up BtxA injections. USDRS, PROMs (Vo

Frequency-Dependent Developmental Toxicity of Anthropogenic Noise: Effects of 40 Hz, 174 Hz and 528 Hz on Zebrafish Embryos.

Characterizing anthropogenic noise pollution in aquatic environments is inherently complex due to the multifaceted physical properties of acoustic waves, including variations in sound intensity, frequency, wavelength and temporal pattern. Among these parameters, contribution of specific frequencies, which are an integral component of broader noise spectrum remains poorly understood. To isolate the influence of this specific variable from overall complexity of underwater sound exposure, the present study employed Danio rerio as a model for aquatic developmental health and systematically investi
