Treatment of comminuted distal radius fractures: an approach based on pathomechanics.

Following dorsally displaced fractures of the distal radius, the classic position of immobilization is with the wrist flexed and in ulnar deviation. This is not the position of function and entails morbidity in the form of finger stiffness, which may require prolonged rehabilitation. We treated 20 consecutive, comminuted, intraarticular distal radial fractures using a new external fixation system with the wrist in a neutral to extended position, thereby promoting metacarpophalangeal joint flexion by relatively relaxing the finger extensor tendons. Supplemental pin fixation was used in eight ca

Pulmonary damage after intramedullary femoral nailing in traumatized sheep--is there an effect from different nailing methods?

Stabilization of femoral shaft fractures is a controversial issue in the management of patients with multiple trauma. Intramedullary nailing usually is preferred primarily; in recent years, however, pulmonary complications (e.g., ARDS) have been reported that were attributed to the reaming procedure. To study the effects of different nailing methods in a model of severe trauma, hemorrhagic shock and lung contusion were created at day 1 in sheep prepared by the method described by Staub. After recuperation (day 3) the animals in the study group (group 1) underwent intramedullary nailing of a cl

Transarticular fixation for severely displaced supracondylar fractures in children.

We have reviewed 34 children who had been treated by open reduction through a medial incision and transarticular pinning for a severely displaced supracondylar fracture of the humerus. Follow-up ranging from nine months to 20 years showed that 27 of 34 elbows (79%) had excellent or good results, with satisfactory resolution of neurovascular problems and no complications due to the method of treatment.

Pugh nail system as a form of treatment for fractures of the proximal femur.

A retrospective study of the results of treatment of 177 patients who had sustained a fracture of the proximal femur was undertaken. The social background, type of injury, operation performed, duration of stay after operation, mortality and morbidity were recorded and compared with the results published by other workers. It is concluded that the Pugh sliding nail system adequately fixes most fractures of the proximal femur. It allows early postoperative mobilization and discharge to the home environment for most patients. The overall mortality and morbidity compares favourably with results pub

Intramedullary nailing of open fractures of the femoral shaft.

The cases of eighty-six patients in whom eighty-nine open fractures of the femoral shaft had been treated by intramedullary nailing with reaming were retrospectively reviewed. Twenty-seven fractures were classified as grade-I open fractures; sixteen, as grade-II open fractures; and forty-six, as grade-III open fractures. Immediate intramedullary nailing was done for fifty-six fractures, and delayed stabilization (five to seven days after delayed closure of the wound) was done for thirty-three fractures. A prerequisite for immediate intramedullary nailing was that irrigation and debridement of

Cast brace management of the femoral shaft fracture in children and young adults.

Seventy-two children and adolescents, 5-19 years of age, sustaining femoral fractures were treated with immediate cast bracing at Oklahoma Children's Memorial Hospital. Ambulation was started an average of 3 days after fracture when there were no associated injuries. The average length of hospitalization was 18.7 days; the average length of immobilization in plaster was 67.4 days. Results were generally comparable to those of more standard methods of treating a fractured femur in this age group. Proximal as well as middle and distal fractures were treated, but varus and anterior angulation was

Ender nailing of intertrochanteric and subtrochanteric fractures of the femur.

Condylocephalic intramedullary Ender nailing of fractures of the proximal end of the femur offers four advantages. The operation is short and is minimally traumatic, with little blood loss. The patient returns to functional ambulatory status within a few days. Infection of the fracture site and at the nail portals is a negligible risk, and the risk of delayed union and non-union is greatly reduced. The method has also introduced a group of new complications such as irritation at the knee, decreased range of knee motion, and distal and proximal migration and penetration of the nails, yet these

Nail plate construct in complex distal femur fractures allow safe, early unrestricted weight bearing with high rates of union and a low rate of failure: five-year experience at a UK major trauma centre.

PURPOSE: Distal femur fractures are challenging to manage and fixation with intramedullary nails or plates alone can fail. Linked nail plate constructs (NPC) are increasingly used for complex cases. This five-year review from a UK major trauma centre reports indications and patient outcomes. METHODS: Patients treated for a distal femoral fracture with a NPC between 1st January 2020 and 1st January 2025 were identified. Patient demographics, mechanism of injury, fracture classification, procedural characteristics, postoperative weight-bearing status, union rates, complications and mortality we

Management of Humeral Shaft Fractures With Intramedullary Interlocking Nail Versus Locking Compression Plate.

Surgical fixation of humeral shaft fractures generally involves plating or nailing. It is unclear whether one method is more effective than the other. The aim of this study was to compare the results of the intramedullary nail and locking compression plate for the treatment of humeral shaft fractures. A total of 60 patients with humeral shaft fractures were randomized to undergo surgery with an intramedullary interlocking nail (n=30) or locking compression plate (n=30). The outcome was assessed in terms of intraoperative blood loss, operative time, hospital stay, union time, union rate, functi

When does hip fracture surgery fail?

INTRODUCTION: Hip fractures remain the most common orthopaedic injury requiring hospital admission. Failed surgery for any cause carries a higher morbidity, mortality, and healthcare-related cost. The aims of this study were to determine risk factors for surgical complications of hip fracture surgery, when they occurred and their effect on mortality. PATIENTS AND METHODS: From a prospectively collected consecutive database of 795 hip fractures admitted between July 2007 and June 2008, all surgical and non-surgical complications were identified as well as re-operation for any cause and mortali

Rushpin fixation for midshaft clavicular nonunions: good results in 13/14 cases.

We treated 14 patients having a symptomatic mid-shaft clavicular nonunion with intramedullary Rushpin fixation and autogenous bone grafting. 13 nonunions healed within 4 months. There were no complications related to the operation. After a mean follow-up of 40 (4-156) months, shoulder function (Constant-Murley score) was 90 (44-100)%, compared to the contralateral shoulder. Rushpin fixation with autogenous bone grafting is a simple and reliable method for treating nonunions of the midshaft of the clavicle.

The association between weight-bearing status and early complications in hip fractures.

BACKGROUND: Early mobilization and weight-bearing have been proposed to improve hip fracture outcomes. This study aimed to compare early postoperative complications and outcomes of patients who underwent weight-bearing as tolerated (WBAT) on postoperative day one (POD1) with those that did not on: (1) 30-day mortality; (2) 30-day postoperative major and minor complications; (3) length of stay (LOS); and (4) discharge disposition after hip fracture management. METHODS: The NSQIP database was used to identify 7947 hip fracture patients managed with a hemiarthroplasty and internal fixation, slid

Ankle fractures: Getting it right first time.

INTRODUCTION: Ankle fractures are common injuries presenting to trauma departments and ankle open reduction and internal fixation (ORIF2) is one of the first procedures targeted in early orthopaedic training. Failure to address the fracture pattern with the appropriate surgical technique and hardware may lead to early failure, resulting in revision procedures or premature degenerative change. Patients undergoing revision ORIF are known to be at much greater risk of complications and many of these secondary procedures may be preventable. METHOD: A retrospective analysis of all patients attendi
