Non-motor features of Parkinson's disease are receiving greater recognition. Constipation affects up to 50% of patients with Parkinson's disease and sigmoid volvulus remains an under recognised complication with mortality rates up to 50%. The incidence of sigmoid volvulus in the general population is 1.7/100,000/year. BACKGROUND: A 19-year-old mentally retarded man with failed exstrophy repair and ureterosigmoidostomy urinary diversion presented with high fever, vomiting and right-flank pain of 2 days' duration. Past medical history was notable for a left nephrectomy to treat an infected staghorn calculus in a poorly functioning kidney. Physical examination revealed pyrexia and right-flank tenderness.
INVESTIGATIONS: Physical examination, renal function tests, electrolyte and metabolic assessment, urine and blood cultures, abdominal CT, ANTEGRADE PYELOURETEROGRAPHY, sigmoidoscopy and histopathology.

Acute sigmoid diverticulitis may present in different forms. Firstly, symptomatic diverticulosis and acute diverticulitis have to be differentiated to allow for the proper selection of treatment. We prospectively compared biopsy specimens obtained with reusable and disposable stainless steel biopsy forceps of varying sizes and configurations. Many surgical procedures have been described for the management of full-thickness rectal prolapse. Currently, the three procedures most frequently used are anterior resection with or without suture rectopexy, transabdominal mesh fixation without resection, and perineal proctosigmoidectomy. Only the latter procedure avoids a laparotomy, and the mesh fixation technique has a high incidence of severe constipation postoperatively. Recently, there have been two reports of laparoscopic mesh fixation for rectal prolapse which were successful. However, the long-term concerns are probably very similar.

Electrical and mechanical activity on longitudinal and circular layers of the human sigmoid colon were simultaneously studied. Recordings were obtained from two electrode sites spaced 3 cm apart in a piece of colon which had been resected surgically and perfused in an organ bath. Spontaneous electrical activity of the colon showed slow waves and spikes. Ten cases of sigmoid colocystoplasty carried out over a 19-year period have been reviewed. The indications were bladder gangrene (1), interstitial cystitis (2), bladder fibrosis (1), failed vesico-vaginal fistula (1), tuberculous contracture (2), atonic bladder (1) and invasive bladder carcinoma (2). All patients void with control every 2 to 4 h throughout the day and twice at night. There has been no long-term deterioration of the upper tract and no delayed complications.

Tuberculous aneurysm is a very rare disease and the prognosis is not good. The authors carried out an emergency abdominal section on a 48 year old patient who had a serious hematochezia and vasography was performed on the patient in order to diagnose the bleeding site. However, the patient had a second serious hematochezia during vasography and fell into the state of shock.
