Predictive Factors for Peritoneal Metastases in Colorectal Cancer: A Retrospective Study Conducted at a Tertiary Care Center in Riyadh, Saudi Arabia.

Background The incidence of colorectal cancer (CRC) is increasing nationally, emphasizing the importance of early detection. Late-stage diagnosis, particularly with peritoneal metastasis (PM), is associated with poor outcomes despite advancements in treatment. PM poses a challenge owing to the lack of standardized treatment, although cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) shows promise in improving survival rates. This study aimed to document the incidence and risk factors for patients presenting with PM following CRC. Materials and methods This retr

Quadratus Lumborum Block: The New Gold Standard in Abdominal Analgesia?

The quadratus lumborum block (QLB) is a common regional anesthesia technique for abdominal and pelvic surgeries. Unlike traditional blocks (e.g., transversus abdominis plane (TAP) block) that target only somatic pain, QLB provides both somatic and visceral analgesia by facilitating the spread of local anesthetic through the thoracolumbar fascia. Its use in enhanced recovery after surgery (ERAS) protocols has increased due to its effectiveness in reducing postoperative opioid consumption, enabling early mobilization, and minimizing systemic side effects. Compared to epidural and TAP blocks, QLB

Impact of PD1 blockade added to neoadjuvant chemoradiotherapy on rectal cancer surgery: post-hoc analysis of the randomized POLARSTAR trial.

BACKGROUND: The addition of PD1 blockade to neoadjuvant chemoradiotherapy (CRT) has been shown to significantly increase pCR rates in locally advanced rectal cancer (LARC). Yet, its impact on total mesorectal excision (TME) remains unknown. METHODS: A post-hoc analysis of the randomized POLARSTAR trial, which enrolled patients with LARC at eight major colorectal cancer centres in Beijing to compare neoadjuvant CRT plus PD1 blockade with CRT alone, was undertaken. Patients received one of three combinations of neoadjuvant treatments before TME surgery: CRT plus concurrent PD1 blockade (concurr

Determination of biomarkers associated with neoadjuvant treatment response focusing on colibactin-producing Escherichia coli in patients with mid or low rectal cancer: a prospective clinical study protocol (MICARE).

INTRODUCTION: The management of mid and low rectal cancer is based on neoadjuvant chemoradiotherapy (CRT) followed by standardised surgery. There is no biomarker in rectal cancer to aid clinicians in foreseeing treatment response. The determination of factors associated with treatment response might allow the identification of patients who require tailored strategies (eg, therapeutic de-escalation or intensification). Colibactin-producing Escherichia coli (CoPEC) has been associated with aggressive colorectal cancer and could be a poor prognostic factor. Currently, no study has evaluated the p

Laparoscopic middle colic artery-preserved right hemicolectomy with true D3 lymph node dissection for right-sided colon cancer: modified complete mesocolic excision.

BACKGROUND: Complete mesocolic excision (CME) has been demonstrated to be a useful surgical procedure for advanced colon cancer. We previously reported on laparoscopic (Lap) CME with true central vascular ligation (CVL) for advanced right-sided colon cancer. Lap CME with true CVL is highly plausible from the perspective of surgical oncology. However, true CVL of the middle colic artery (MCA) may require extensive resection of the transverse colon. The Japanese Classification of Colorectal Cancer defines D3 as main lymph node dissection around the superior mesenteric artery (SMA), and true CVL

Factors predicting occurrence and therapeutic choice in malignant colorectal polyps: a study of 13 years of colonoscopic polypectomy.

Colorectal carcinoma represents a major cause of mortality and 0.2-12% of resected colonic polyps have malignant cells inside. We performed a retrospective study of patients with resected polyps during a period of 13 years. A total of 905 patients had 2033 polyps removed; 122 polyps (109 patients) had malignant cells. Prevalence of malignant polyps with submucosal invasion was 1.23% and for all polyps with malignant cells was 6%; malignant polyps had a larger size (23.44 mm mean diameter) vs benign polyps (9.63 mm); the risk of malignancy was increased in polyps larger than 10 mm, in lateral s

Pressurized intraperitoneal aerosol chemotherapy (PIPAC) in combination with standard of care chemotherapy in primarily untreated chemo naïve upper gi-adenocarcinomas with peritoneal seeding - a phase II/III trial of the AIO/CAOGI/ACO.

BACKGROUND: Peritoneal metastasis is a common and dismal evolution of several gastrointestinal (GI) tumors, including gastric, colorectal, hepatobiliary, pancreatic, and other cancers. The therapy of peritoneal metastasis is largely palliative; with the aim of prolonging life and preserving its quality. In the meantime, a significant pharmacological advantage of intraperitoneal chemotherapy was documented in the preclinical model, and numerous clinical studies have delivered promising clinical results. METHODS: This is a prospective, open, randomized multicenter phase III clinical study with

Reoperation in an adult female with "right-sided" Hirschsprung's disease complicated by refractory hypertension and cough.

Hirschsprung's disease (HD) is an intestinal malformation caused by the innate absence of ganglion cells in the neural plexus of the colorectal wall, and is most common in male infants. It is rare in adult, and is usually left-sided. Herein we reported based on the CARE guidelines a case of a 47-year-old adult female suffering from "right-sided" HD complicated by refractory hypertension and cough. The patient with a history of cesarean section and with digestive unfitness (abdominal pain, distention, and constipation) only since 20 years old had recurrence of HD after initial surgery due to th

Laparoscopic colostomy reversal after a Hartmann procedure: a prospective series, literature review and an argument against laparotomy as the primary approach.

BACKGROUND: Open restoration of bowel continuity after a Hartmann procedure has been associated with significant morbidity, including anastomotic leak, incisional hernia, wound infections and inability to re-establish intestinal continuity. Few studies have examined the role of laparoscopy in performing a Hartmann reversal. The aim of this study was to review our laparoscopic Hartmann reversal (LHR) experience with an emphasis on intra- and postoperative adverse events. METHODS: A prospectively collected laparoscopic colorectal database involving 3 surgeons in 4 academic centres between 1991

Ulcerative colitis and coexisting colorectal cancer: recurrence rate after restorative proctocolectomy.

BACKGROUND: The association between mucosal ulcerative colitis (MUC) and adenocarcinoma is well established. METHODS: Records of patients who had undergone restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) from 1983 through 1992 were examined. Of these, 604 had MUC and 27 (4.3%) had MUC with coexisting cancer. Patients were surveyed annually for recurrent disease. Pouch function and quality of life were evaluated with a questionnaire and physical examination. RESULTS: The duration of disease was longer (p = 0.001) in patients with cancer (16.1 +/- 8.0 years) than in those

Modified posterior pelvic exenteration with pelvic side-wall resection requiring both intestinal and urinary reconstruction during surgery for ovarian cancer.

OBJECTIVE: Because of the anatomic proximity of the rectosigmoid to the female pelvic organs and its frequent involvement in ovarian cancer, an en bloc resection of ovarian tumors together with the uterus and rectosigmoid, also known as a modified posterior pelvic exenteration (MPPE), is frequently performed to achieve optimal cytoreduction [1]. Additionally, if the tumor has infiltrated the pelvic side-wall, a MPPE combined with pelvic side-wall resection can be selected [2]. We report the details of a technique for this surgery requiring intestinal and urinary reconstruction. METHODS: A 55-

Association of the Primary Care Frailty Index with Postoperative Outcomes in Older Patients Undergoing Major Gastrointestinal Oncologic Surgery: A Retrospective Cohort Study.

Background/Objectives: Frailty is increasingly recognized as a major determinant of postoperative outcomes in older patients undergoing oncologic surgery. The Primary Care Frailty Index (PC-FI), a deficit accumulation-based instrument derived from routinely available clinical information, has recently been proposed as a practical frailty assessment tool. However, evidence supporting its application in gastrointestinal surgical oncology remains limited. Methods: We conducted a retrospective cohort study including patients aged ≥65 years who underwent elective major colorectal or gastric cancer

Artificial intelligence classification of rectal neoplasia by endoscopic fluorescence perfusion analysis.

Disordered vascularity is a hallmark of carcinogenesis. Fluorescence microperfusion heterogeneity may discriminate malignant transformation within significant (> 20 mm) rectal polyps enabling in-situ endoscopic classification via machine learning (ML) methods to inform clinical care. Patients referred for transanal management of rectal neoplasia were recruited. Indocyanine green was administered intravenously and near-infrared (NIR) endoscopic video recorded. Videos were processed with bespoke fluorescence quantification software producing intensity-timeseries plots for neoplastic and normal r

Single-incision robotic complete mesocolic excision for right-sided colon neoplasms: technical and oncologic feasibility.

BACKGROUND: Single-incision robotic surgery (SIRS) using the da Vinci SP system (dVSP) offers advantages such as improved cosmesis, reduced invasiveness, and technical precision in colorectal surgery. However, real-world data regarding the technical feasibility and oncologic outcomes of dVSP-based complete mesocolic excision (CME) with central vascular ligation (CVL) for right-sided colon neoplasms remain limited. METHODS: We retrospectively reviewed the medical records of patients who underwent single-incision robotic right colectomy for colon cancer or precancerous lesions with dVSP between…
