Surgical treatment of pituitary apoplexy in association with hemispheric infarction.

We report a patient with pituitary apoplexy in whom cerebral infarction developed, possibly secondary to vasospasm. Pituitary apoplexy is a clinical syndrome caused by acute hemorrhage or infarction of the pituitary gland. Our patient's clinical symptoms and radiographic findings greatly improved after surgical resection of the apoplectic pituitary gland. An extensive literature review was performed, including all previously reported cases of pituitary apoplexy leading to cerebral infarction. The clinical features, pathophysiological mechanisms, management and outcome of cerebral infarction fo

Micro-endoscopic-discectomy (MED) for far lateral disc herniation in the lumbar spine. Technical note.

This study describes a new experience of the authors in the treatment of extraforaminal disc herniation via the micro-endoscopic far lateral approach to establish a less traumatic approach to extraforaminal disc herniation with less stay in hospital and less cost. Seventeen patients who underwent surgery for extraforaminal disc herniation were analysed and long-term follow up was done revisiting all of them in hospital. The results of surgical decompression via the micro-endoscopic far lateral approach were good in all patients with minimal discomfort. There was complete resolution of leg pain

[Tarsal tunnel syndrome in athletes].

Pain in the hindfoot due to overuse has many causes. One uncommon cause is the entrapment of the posterior tibial nerve: the tarsal tunnel syndrome. Pain when palpating posterior the medial malleolus and pain in the abductor hallucis muscle are typical symptoms. Application of a local anaesthetic leads to immediate pain relief. The distal motoric latency is prolonged, but not in all cases. First of all, treatment consists of local and oral antiphlogistics, orthopaedic arch-support and physiotherapy. In case of therapy-resistant pain over more than three months period, surgical decompression of

Redundant nerve roots of the cauda equina. A report of five cases.

Five personally examined cases of redundant nerve roots of the cauda equina are added to the 13 hitherto on record. Except for segmental neurological deficits, intermittent claudication-like symptoms of the cauda equina dominated the histories of four of our five cases. The diagnosis was confirmed at surgical exploration, which included laminal and dural decompression. At follow-up (two months--three years) the neurological deficit, or the intermittent claudication, or both, had improved in all our patients. The pathogenesis of root redundancy is obscure. It seems that the symptoms arise throu

Excision of a Giant Bilobed Second Branchial Cleft Cyst: A Case Report.

Second branchial cleft cysts (BCCs) are congenital cystic anomalies of the lateral neck that rarely attain giant proportions exceeding 10 cm. When they do, their size and proximity to critical neurovascular structures present distinctive diagnostic and surgical challenges. We present the case of a 24-year-old woman with a one-year history of a painless, enlarging left neck mass. Imaging revealed a large, bilobed cystic lesion deep to the sternocleidomastoid muscle extending from the skull base to the subclavicular region. Fine-needle aspiration cytology excluded malignancy, and the cyst recurr

Conservative treatment of carpal tunnel syndrome: an outcome study of adjunct exercises.

We conducted a prospective study to evaluate the effect of a new non-surgical protocol for treating carpal tunnel syndrome (CTS). It incorporates the Carpal Tunnel Decompression Exercise Program (Exercise Program) into conventional non-surgical treatment. Twenty-eight patients (33 hands) met the inclusion criteria and were followed for 18 months. The outcome measurement tools used were the Symptom Severity Scale (SSS) and the Functional Status Scale (FSS). The outcome tools were administered at admission and repeated at 1, 2, and 18 months after admission to the treatment program. Using the ad

Ogilvie's syndrome in the postcesarean section patient.

Two cases of colonic pseudo-obstruction (the so-called Ogilvie's syndrome) are reported. Both patients were in the immediate postcesarean section puerperium. The importance of early diagnosis in these cases is stressed, because this complication has a high mortality rate, frequently in relation to delayed diagnosis and treatment. One patient was successfully treated with conservative measures because diagnosis was made early. A plain X-ray abdominal film which shows cecum dilatation, with or without ascending and transverse colon dilatation, and no distal air, makes the diagnosis. A cecum diam

Single-Stage Posterior-Only In Situ Fixation and Fusion for Neglected Congenital Vertebral Aplasia With Kyphoscoliosis in a Resource-Limited Setting: A Case Report.

Neglected congenital kyphoscoliosis with vertebral aplasia, absent pedicles, and rib anomalies presents a formidable challenge, especially in low-resource settings. We report the case of a 28-year-old man with progressive thoracolumbar deformity, cosmetic concerns, and fatigue. Imaging revealed congenital vertebral aplasia with pedicle agenesis from D10 to D12, a missing left 12th rib, and a dysplastic right L1 pedicle. The patient underwent a single-staged posterior-only surgical correction using basic fluoroscopic guidance. Surgical management included multilevel pedicle screw fixation, wide

Challenges in Postoperative Compliance and Follow-Up Among Trauma Patients: A Case Report of a Trans-scaphoid Perilunate Dislocation.

The wrist joint is a complex articulation that involves the distal radius, ulna, eight carpal bones (scaphoid, lunate, triquetrum, pisiform, trapezium, trapezoid, capitate, and hamate), and the bases of the metacarpals. Trans-scaphoid perilunate fracture-dislocations are complex wrist injuries, most commonly resulting from high-energy mechanisms involving hyperextension and ulnar deviation. Prompt diagnosis and surgical management are essential in restoring wrist function and stability. This report focuses on the case of a 28-year-old male who sustained a trans-scaphoid perilunate dislocation

Management of neonatal upper limb trauma complicated by arterial injury: A case report.

This case report presents the clinical course and management of a 29-day-old female infant who presented with upper limb trauma secondary to entrapment beneath a sewing machine. Following admission, the patient exhibited clinical signs consistent with arterial injury and compartment syndrome, necessitating prompt diagnostic evaluation and surgical intervention. Despite challenges in arterial visualization via Doppler ultrasound, radiographic imaging confirmed displaced distal humerus fracture of the left upper limb, prompting further evaluation and surgical planning. Surgical decompression and

Expanding the spectrum on brain-heart interactions in cerebellopontine angle surgeries.

Lesions at the cerebellopontine angle (CP angle) are associated with various brain-heart interactions, which can include those from stimulation of the fifth cranial nerve along the scalp incision in a retrosigmoid suboccipital surgical approach. A 27-year-old male patient with recently diagnosed hypertension (on calcium channel blocker) underwent left CP angle lesion decompression. Transient episodes of bradycardia, hypotension, and bradypnea were observed from the skin incision onward, exacerbated during tumor manipulation. Most episodes subsided with cessation of the surgical stimulus while

Ossification of the Posterior Longitudinal Ligament: Pathophysiology, Diagnosis, and Management.

Ossification of the posterior longitudinal ligament (OPLL) occurs as heterotopic bone forms in the posterior longitudinal ligament, resulting in neural compression, myelopathy, and radiculopathy. OPLL is most commonly observed in East Asian populations, with prevalence rates of 1.9% to 4.3% reported in Japan. OPLL rates are lower in North American and European patients, with reported prevalence of 0.1% to 1.7%. Patients typically develop symptoms due to OPLL in their cervical spines. The etiology of OPLL is multifactorial, including genetic, metabolic, and anatomic factors. Asymptomatic or sym

Gastric necrosis following a hiatal hernia: A case report.

INTRODUCTION: Gastric volvulus are rare. Complications can be life threatening, including necrosis and perforation. Assessment of mucosal viability is essential, and urgent surgical intervention is mandatory in case of vascular compromise. PRESENTATION OF CASE: An 72-year-old female known for a paraesophageal hiatal hernia was admitted at our emergency department with acute abdominal pain. Blood count demonstrated leukocytosis and increased C-reactive protein. Abdominal computed tomography showed a mesenteroaxial gastric volvulus. Urgent upper endoscopy revealed mucosal ischemia, which prompt

Reduction of high-grade isthmic and dysplastic spondylolisthesis in 5 adolescents.

Treatment of high-grade isthmic and dysplastic spondylolisthesis in children and adolescents remains a challenge. Surgical treatment of spondylolisthesis has been recommended in adolescents with pain refractory to nonoperative modalities, slippage progression, or > 50% slippage on presentation. Controversy exists as to the optimal surgical approach for high-grade spondylolisthesis. In this report, we describe 5 cases of high-grade isthmic and dysplastic spondylolisthesis in adolescents and review the literature on surgical treatment for this entity. Operative records, charts, x-rays, and Scoli

Combined endovascular and surgical repair of a large traumatic pseudoaneurysm of the thigh-a case report.

Thigh pseudoaneurysms are rare compared to pseudoaneurysms of the groin, and usually result from direct injury to an arterial branch. Direct open repair can be associated with a large volume blood loss. The authors describe a combined endovascular and surgical approach to a large, traumatic, pseudoaneurysm of the thigh. The patient was a 49-year-old man with a history of left femur fracture treated by open reduction and internal fixation, who presented with a painfully swollen left thigh. Duplex ultrasound and computed tomography (CT) scan suggested a large (7.7 x 5.0 x 6.3 cm) pseudoaneurysm
