Development of the first adult respiratory extracorporeal membrane oxygenation center in Croatia under emergency circumstances.

INTRODUCTION: We report on the challenges of establishing a successful adult respiratory extracorporeal membrane oxygenation (ECMO) center in a developing country like Croatia under emergency conditions. We further introduce measures that would improve the outcome of patients treated with ECMO for respiratory failure at the national level. METHODS: 100 consecutive adult patients treated with venovenous (VV) ECMO for acute respiratory failure were enrolled to the database prospectively from October 2009 until June 2016. A review of methodology in establishing an adult respiratory ECMO center i

Depression, anxiety, and post-traumatic stress disorder among youth in low and middle income countries: A review of prevalence and treatment interventions.

BACKGROUND: Low and middle income countries (LMICs) not only have the majority of the world's population but also the largest proportion of youth. Poverty, civil conflict and environmental stressors tend to be endemic in these countries and contribute to significant psychiatric morbidity, including depression, anxiety and post-traumatic stress disorder (PTSD). However, mental health data from LMICs is scarce, particularly data on youth. Evaluation of such information is crucial for planning services and reducing the burden of disability. This paper reviews the published data on the prevalence

Multivitamin use and adverse birth outcomes in high-income countries: a systematic review and meta-analysis.

BACKGROUND: In high-income countries, a healthy diet is widely accessible. However, a change toward a poor-quality diet with a low nutritional value in high-income countries has led to an inadequate vitamin intake during pregnancy. OBJECTIVE: We conducted a systematic review and meta-analysis to evaluate the association between multivitamin use among women in high-income countries and the risk of adverse birth outcomes (preterm birth [primary outcome], low birthweight, small for gestational age, stillbirth, neonatal death, perinatal mortality, and congenital anomalies without further specific

A forecast of typhoid conjugate vaccine introduction and demand in typhoid endemic low- and middle-income countries to support vaccine introduction policy and decisions.

A Typhoid Conjugate Vaccine (TCV) is expected to acquire WHO prequalification soon, which will pave the way for its use in many low- and middle-income countries where typhoid fever is endemic. Thus it is critical to forecast future vaccine demand to ensure supply meets demand, and to facilitate vaccine policy and introduction planning. We forecasted introduction dates for countries based on specific criteria and estimated vaccine demand by year for defined vaccination strategies in 2 scenarios: rapid vaccine introduction and slow vaccine introduction. In the rapid introduction scenario, we for

Bacillus Calmette-Guérin immunisation at birth and morbidity among Danish children: A prospective, randomised, clinical trial.

BACKGROUND: Studies from low-income countries report positive non-specific effects of early Bacillus Calmette-Guérin (BCG) immunisation on childhood health and survival. Neonatal immunisation with BCG may prime the immune system and offer partial protection against other infectious and possibly allergic diseases. The potential clinical value of these non-specific effects has not yet been examined in a large randomised trial in high-income countries. METHODS: The Danish Calmette Study is a multicentre randomised clinical trial conducted between October 2012 and November 2015. Within the first

Surgical treatment of femoral diaphyseal fractures in children using elastic stable intramedullary nailing by open reduction at Yopougon Teaching Hospital.

INTRODUCTION: Elastic stable intramedullary nailing (ESIN) has transformed children's femoral shaft fracture treatment, but this technique requires an image intensifier. Without it, open reduction is used to check fracture reduction and pin passage. The aim of this study was to describe our techniques and to evaluate our results at the middle term. HYPOTHESIS: The open reduction and ESIN technique provides satisfactory results with few major complications. PATIENTS AND METHODS: This was a retrospective study that focused on femoral diaphyseal fractures treated in the pediatric surgery unit a

Genital chlamydia prevalence in Europe and non-European high income countries: systematic review and meta-analysis.

BACKGROUND: Accurate information about the prevalence of Chlamydia trachomatis is needed to assess national prevention and control measures. METHODS: We systematically reviewed population-based cross-sectional studies that estimated chlamydia prevalence in European Union/European Economic Area (EU/EEA) Member States and non-European high income countries from January 1990 to August 2012. We examined results in forest plots, explored heterogeneity using the I² statistic, and conducted random effects meta-analysis if appropriate. Meta-regression was used to examine the relationship between stud

Commercial motorcycle drivers' perceptions of risk and road safety in urban Nigeria: an explorative study.

Road traffic injury is a great public health challenge with an emerging trend of increasing rates and high mortality involving commercial motorcycles in Nigeria. A qualitative approach was used with 10 in-depth interviews conducted to explore the risk perceptions of commercial motorcyclists in Ibadan, Nigeria. The data analysis using manifest and latent content analysis resulted in an overarching theme: inadequate structures and internalised norms prevent change. The three themes leading to the overarching theme are: risk-taking as generally acceptable; risk-taking as an intrinsic part of occu

Rabies control and elimination: a test case for One Health.

One Health approaches have already been shown to be successful in controlling rabies in different parts of the world. In this article, the latest in Veterinary Record's series promoting One Health, Sarah Cleaveland and her colleagues at the University of Glasgow discuss why integrated strategies are needed to enhance the cost effectiveness of measures to control and eliminate rabies, particularly in low-income countries.

Intimate partner violence and its contribution to mental disorders in men and women in the post genocide Rwanda: findings from a population based study.

BACKGROUND: In low income countries, mental disorders are a neglected health problem. Mental disorders are influenced by a number of factors in people's everyday life of which intimate partner violence (IPV) commonly form an important part. The aim of this study was to investigate the prevalence of mental disorders in young men and women in Rwanda and their risk factors with main emphasis on IPV and its contribution to mental disorders, taking into account the genocide context. METHODS: This population-based study included a representative sample of 917 men and women aged 20-35 years. The pre

Towards people-centred health systems: a multi-level framework for analysing primary health care governance in low- and middle-income countries.

Although there is evidence that non-government health system actors can individually or collectively develop practical strategies to address primary health care (PHC) challenges in the community, existing frameworks for analysing health system governance largely focus on the role of governments, and do not sufficiently account for the broad range of contribution to PHC governance. This is important because of the tendency for weak governments in low- and middle-income countries (LMICs). We present a multi-level governance framework for use as a thinking guide in analysing PHC governance in LMI

Causes of maternal mortality in Ethiopia: a significant decline in abortion related death.

BACKGROUND: Although the common direct obstetric causes of maternal mortality are known from the literature, the contribution of each cause and the change in trend over decades is unknown in Ethiopia. The objective of this review was to assess the trend of proportion of maternal mortality due to the common direct causes. METHODS: This systematic review was done on eighteen health facility based maternal mortality studies conducted between 1980 and 2012 in Ethiopia. Emphasis was given to the proportion of maternal mortality due to direct causes and their case fatality rates. RESULTS: The summ

Attitudes of primary health care physician managers toward research: a pre-experimental study.

Research in primary health care (PHC) is underdeveloped and scarce, especially in developing countries. It is important to understand the attitudes and aspirations of PHC physicians for the promotion of research. The aim of this study was to determine the attitudes of PHC physician managers toward research in Qassim province and to identify barriers that impede performing research in the PHC system. The study was based on social cognitive theory framework, and was pre-experimental with a 'one-group pre-test-post-test' design. The study participants were physician managers in PHC administration…

Developments in CD4 and viral load monitoring in resource-limited settings.

CD4 counts and human immunodeficiency virus (HIV) load testing are essential components of HIV care, and making these tests available in resource-limited settings is critical to the roll-out of HIV treatment globally. Until recently, the evidence supporting the importance of laboratory monitoring in resource-limited settings was lacking, but there is now a consensus emerging that testing should become routine to ensure the longevity of treatment programs. Low-cost, point-of-care testing offers the potential to fill this role as it potentially improves all aspects of HIV care, ranging from the
