A comparative study of the fallopian canal at the meatal foramen and labyrinthine segment in young children and adults.

If entrapment is a critical factor in pathogenesis, the lower incidence of Bell's palsy in children compared with adults may have an anatomical basis. Histologic sections of 20 temporal bones from children younger than 2 years were examined to determine the diameter of the facial nerve and fallopian canal at the meatal foramen and in the labyrinthine segment. No statistically significant difference in the nerve/canal ratios in these areas was found. Comparisons were made with similar data from 10 adult temporal bones. The nerve/canal ratios in the labyrinthine portion were similar in both age

A critical comparison of results of endolymphatic subarachnoid shunt and endolymphatic sac incision operations.

Detailed information was obtained by questionnaires, office visits, chart reviews, and telephone calls from 171 of 179 consecutive patients who underwent endolymphatic sac surgery between 1976 and 1986. A comparison of surgical results was made. For patients followed more than 24 months, complete control of vertigo was obtained in 40 of 68 patients (59%) by endolymphatic subarachnoid shunt and in 27 of 60 patients (45%) who underwent a variety of sac incision operations. This difference achieved borderline statistical significance (p less than or equal to 0.057). Profound or total hearing loss

Electrocochleographic study of experimentally induced endolymphatic hydrops.

An endolymphatic hydrops was induced in the left ear of each of 32 guinea pigs by obliteration of the endolymphatic sac. Both the hydropic ear and the control ear were examined by electrocochleography 1, 2, 4 or 8 months after obliteration of the sac using electrodes on the apex and near the round window. One month after obliteration, the threshold of the compound action potential (AP) and the increase of the AP amplitude with sound pressure level (SPL) were the same as the results recorded from the control ears despite the presence of a histologically confirmed hydrops. In the 2-month group,

Effect of drug therapy on compensation from vestibular injury.

The influence of selected drugs on compensation from unilateral labyrinthectomy was studied in the cat. The drugs investigated included: a. amphetamine, b. diazepam, c. dimenhydrinate, d. scopolamine, and e. trimethobenzamide. The most beneficial drugs for improving recovery were a stimulant (amphetamine) and a general anti-emetic (trimethobenzamide). It is postulated they had this effect by improving the overall activity level with general exercise being a known positive influence. Conversely the other drugs either had little effect on recovery or hindered it, by presumably suppressing the se

Hearing preservation in vestibular neurectomy.

The goal of vestibular neurectomy is to control disabling vertigo while preserving hearing in patients with nonhydropic intractable peripheral vertigo or in patients with Meniere's disease in which an endolymphatic sac procedure has failed. Labyrinthectomy continues to be used to treat patients with intractable vertigo and serviceable hearing. We feel that a labyrinthectomy is contraindicated when any useful hearing remains. Vestibular neurectomy affords the surgeon a means to eliminate the abnormal vestibular input without sacrificing hearing. Two approaches have been used to section the vest

Endolymphatic fistulization.

Three previous papers have detailed my experience with various methods of surgical treatment of Meniere's disease: 1) surgery for all patients; 2) surgery for only those patients who demonstrated patency of the vestibular aqueduct as seen on tomographic examination; and 3) prognostic tests to determine which patients were most suitable for endolymphatic sac surgery. This paper details the results of endolymphatic shunt surgery performed on 43 patients over a 3-year period, restudying my prior conclusions. All patients were operated using a new method of capillary endolymph dispersement regardl

[Ocular and auditory organs of Ochotona rufescens].

Ochotona rufescens is an animal species native to Afghanistan, and is among many new animal species currently being raised for use in experimental research in Japan. In the present paper, we report morphological findings on ocular and auditory character of Ochotona rufescens examined first under normal conditions and then under the influence of various agents, and the results were compared with those on other experimental animals used in this study. On histological examination, the cochlear portion of the labyrinth was found to consist of a coiled canal making two and one half turns from its b

Klippel-Feil syndrome and associated ear deformities.

The Klippel-Feil syndrome is characterized by a short, thick neck with restricted head mobility and a low occipital hairline. The head appears to rest directly on the thorax and the trapezius muscles flare out on either side of the neck, producing a pterygium-like effect. Cervical roentgenograms demonstrate fusion of some or even all of the vertebral bodies into bony blocks. Ear deformities occur in approximately one-third of the Klippel-Feil patient population. We report five cases of Klippel-Feil syndrome and describe their otologic and roentgenographic features. Although no single character

Longterm results of ultrasonic irradiation in Meniere's disease.

Thirty-seven patients with Ménière's disease were treated with ultrasonic irradiation to the lateral and superior semicircular canals. In a longterm follow-up study, lasting alleviation of the vertiginous attacks was found in 24 out of 33 patients (72%). In most of the cases, hearing remained unchanged. It is suggested that the therapeutic effect is associated with a change in the endolymphatic drainage dur to histological alterations in the labyrinth produced by ultrasound.

Middle fossa vestibular nerve section in the management of Ménière's disease.

The triad of fluctuating hearing loss, tinnitus, and episodic attacks of vertigo known as Ménière's disease can be a frustrating therapeutic problem. While most patients respond to a medical regime consisting of a low salt diet, diuretics, diazepam, and propantheline bromide, there is a significant group who remain refractory to conservative management. These individuals either continue to experience their distressing symptoms or undergo some type of surgical procedure in an attempt to seek relief. There are a multitude of such techniques available to the otologic surgeon and he must make the

Endolymphatic sac surgery for hearing conservation in Meniere disease.

Thirty-five patients with Meniere disease underwent endolymphatic sac decompression and drainage with a Silastic sac-mastoid shunt. On the basis of patients' responses to a questionnaire and on analysis of preoperative and postoperative audiometric studies, we found the method to be an effective means for conservation or improvement of hearing and for relief of vertigo in patients with positive glycerol tests. Of the 35 patients, eight had negative glycerol tests and did not benefit from surgery (P less than .001). Of the 27 patients with positive glycerol tests, 70.4% (19) had no further vert

Matrix metalloproteinases 2 and 9 in the cochlea: expression and activity after aminoglycoside exposition.

The matrix metalloproteinases (MMPs) are a family of proteins involved in the remodelling and homeostasis of the extracellular matrix. These proteases have been well studied in the retina and the brain, marking their importance in neuronal cell survival and death [Chintala (2006) Exp Eye Res 82:5-12; Candelario-Jalil et al. (2009) Neuroscience 158:983-994]. The neuroepithelia of the eye and the inner ear share common characteristics. Therefore, we hypothesized that MMPs could play a similar role in the cochlea as described in the retina. We focused on the localization and function of MMP-2 and

CCDC181, a kinocilium-localization protein, is dispensable for hair bundle development and auditory perception.

Inner ear cochlear hair cells exhibit a highly specialized ciliary architecture that consists of a bundle of stereocilia responsible for mechanotransduction and, during development, a single kinocilium that governs hair bundle polarity. While the morphogenesis of stereocilia is being extensively characterized and many key molecules have been identified, the molecular mechanisms governing the formation and subsequent degeneration of the kinocilium remain largely unknown. Through immunohistochemical staining, we found that CCDC181, a microtubule-binding protein, is abundantly expressed in the po

Systemic metabolic alterations in Ménière's disease: Insights from urinary 1H NMR-based metabolomics.

Ménière's disease (MD) is a chronic inner ear syndrome characterized by vertigo, tinnitus, and fluctuating sensorineural hearing loss. Despite its well-defined clinical presentation, the underlying pathophysiology remains elusive, often resulting in empirical and partially effective treatments. This multicenter pilot study employed 1H NMR-based urinary metabolomics to identify metabolic pathways and potential diagnostic biomarkers related to the disease. The study analyzed 50 urine samples (30 MD patients, 20 controls), identifying 109 metabolites from both endogenous and exogenous sources. Wh

Successful Natalizumab Treatment of Two Female Individuals With Susac Syndrome.

BACKGROUND: Susac syndrome is a rare autoimmune endotheliopathy that affects the central nervous system, retina, and inner ear, characterized by encephalopathy, branch retinal artery occlusions, and sensorineural hearing loss. Due to the heterogeneity of its presentation, early diagnosis, and treatment remain challenging. OBJECTIVE/METHODS: To evaluate the clinical outcomes and radiological responses in two patients with Susac syndrome treated with natalizumab in an off-label therapeutic approach, clinical assessments and serial magnetic resonance imaging (MRI) were performed over a follow-up
