Of 214 croquet players who responded to a questionnaire, 76 reported at least one injury to hand, wrist or forearm caused by striking the ball. There was no obvious relation to which of the three main grips the player applied to the mallet. Injuries were somewhat more frequent when the mallet shaft consisted of fibreglass than when it was wood, metal or carbon fibre, but a causal relation has not been established. Back injuries seem less troublesome in croquet than in golf.

Activity related pain on the lateral side of the elbow or proximal on the forearm may be caused by compression of the posterior interosseous nerve in the radial tunnel. A number of different specialties can be involved in this patient. Often there is no effect of conservative treatment. We report a patient who developed erythematous indurated plaques with alopecia on the face and multiple well-demarcated infiltrated scaling lesions on the trunk and extremities. Skin biopsy showed a mucinous degeneration of the follicular outer root sheaths and sebaceous glands consistent with the diagnosis of follicular mucinosis. A histologic examination of a specimen from the left forearm showed microscopic changes of mycosis fungoides. The present case confirms the need of multiple skin biopsies in the evaluation of patients with follicular mucinosis.

Wound botulism is an uncommon disorder that continues to be rarely reported in the United States. A 34-year-old intravenous heroin user was admitted to the Loma Linda, Calif., Veterans Administration hospital with multiple abscesses on his forearms. His clinical course was compatible with botulism, and his culture and serum were positive for Clostridium botulinum toxin type A. Early culture and/or serum identification can lead to prompt diagnosis, treatment, and improvement in the morbidity and mortality rates of this disease.

Strength and stability of the shoulder girdle affect function of the entire upper extremity. Isolated muscle weakness may be compensated for by adjacent muscles; however, profound weakness of the deltoid muscle, for example, affects not only placement of the extremity, but also strength and endurance of the forearm and hand. Orthotic substitutes for weakened muscles are not successful. If the muscle weakness is nonprogressive and stable, then surgical treatment will improve glenohumeral stability, increase endurance, and provide more uniform strength for many activities of daily living.

Currently, myoelectric prostheses lack dexterity and ease of control, in part because of inadequate schemes to extract relevant muscle features that can approximate muscle activation patterns that enable individuated dexterous finger motion. This project seeks to apply a novel algorithm pipeline that extracts muscle activation patterns from one limb, as well as from forearm muscles of the opposite limb, to predict muscle activation data of opposite limb intrinsic hand muscles, with the long-range goal of informing dexterous prosthetic control.

Cervical carcinoma is the second most common female malignancy worldwide. It usually spreads by direct local extension or the lymphatic vessels. Hematogenous dissemination with distant skeletal muscle metastases is a rare phenomenon. We report here 2 patients whose recurrent squamous cell carcinomas of the cervix presented with symptomatic skeletal muscle metastases affecting the muscles of the thoracic wall and forearm, respectively. We also discuss the differential diagnosis and comprehensively review the previously published literature on this rare presentation of cervical carcinoma.

Many options exist for reconstruction of the posterior elbow/olecranon area following wound formation. Careful early wound management is crucial to ensure successful outcomes following reconstruction. Local and regional options are preferred methods for soft tissue coverage in this region. Common flap options include the reversed lateral arm flap, the radial forearm flap, posterior interosseous artery flap, brachioradialis muscle flap, flexor carpi ulnaris flap, and the latissimus flap. The advantages and disadvantages of these flap options are discussed in this review.

Acquired radial club hand caused by infection has been reported occasionally. We report a rare case of acquired radial club hand with an associated septic dislocation of the elbow, primarily caused by compartment syndrome following venous cannulation. The deformity and instability were treated with distraction using an external fixator followed by a centralisation procedure. At the 2-year follow-up the appearance and functions of the left forearm had improved considerably and the wrist was stable. There was no deterioration in ulnar growth.

OBJECTIVE: To explore the effect of skin flaps and muscular flaps transplanted in the use of tissue defect repairing.
METHOD: Thirty cases of tissue defect were repaired or reconstructed by forearm flaps, pectoralis major muculocutaneous flaps, frontal flaps, nasolabial fold flaps or paranavel rectus abdominis muscle flaps.
RESULT: Twenty-nine cases were successful but one case was unsuccessful.
CONCLUSION: Skin flaps and muscular flaps reconstruction are safe and reliable. But the indication should be selected and the complication should be prevented.

A 41-year-old man noticed weakness and atrophy in his right hand and forearm resembling the non-progressive juvenile muscular atrophy of unilateral upper extremity (Hirayama's disease). MRI showed an abnormal cavity in the posterior epidural space which appeared on neck flexion communicating with the subarachnoid space in addition to the flattening of the lower cervical spinal cord on neck flexion. When evaluating atypical cases of Hirayama's disease, the pathomechanism demonstrated in the present case should be taken into consideration.

Several congenital upper extremity anomalies may be treated using micro-surgical techniques. Long-term studies have shown the usefulness of microvascular toe transfer in the treatment of adactyly with the incorporation of the transferred digit into grasp-and-pinch function. The use of free fibular transfer for long bone deformities of the forearm has been shown to provide bony union as well as growth. Factors that must be considered include patient age, vessel availability, and lack of other possible reconstructive options.

Acute compartment syndrome has multiple causes: fractures, crush injury, vascular trauma and burns. Exertional compartment syndrome may be acute (progressive) or chronic (usually reversible). The acute form usually occurs after intensive exercise. Closed muscle rupture is an uncommon cause with few reports. We report two cases, in the peroneal compartment of the leg and the flexor compartment of the forearm, to show that a high index of suspicion, allowing prompt diagnosis and fasciotomy, will enable a full recovery without complications.

This article presents a previously unreported double type III epiphyseal separation of the ulnar head associated with ulnar nerve palsy. The ulnar epiphysis burst into volar and dorsal segments, the latter giving rise to the ulnar nerve palsy. The patient was treated by open reduction of the translocated epiphysis and neurolysis of the ulnar nerve. Although good clinical results were obtained after surgery, forearm rotation remained limited, confirming that open reduction immediately after injury is mandatory in treating type III epiphyseal separations.

A student in a veterinary program noted an inflammatory tinea corporis on the right forearm after working with cows. Culture grew Trichophyton verrucosum. A nine-day course of treatment with oral fluconazole, 100 mg daily, resulted in prompt resolution. Awareness of the clinical presentations of barn itch, and familiarity with wet mount and histopathologic features, and special growth requirements of T. verrucosum will facilitate prompt diagnosis and treatment. Therapy with newer antifungal agents such as fluconazole may offer advantages over grisefulvin.

A thorough knowledge of anatomy, injury patterns, repair techniques, and evolving rehabilitation methods is necessary to best treat extensor tendon injuries. These injuries are conceptualized as occurring in one of eight zones, which are numbered distally to proximally in the hand and forearm. Even though surgical technique and rehabilitation are specific in each zone, injuries over and proximal to the proximal interphalangeal joint tend to yield less satisfactory results. Dynamic postoperative extension splinting is one factor that is improving long-term results.

Chromoblastomycosis is a rare, deep mycosis seen mostly in tropical and subtropical regions. We have encountered a kidney transplant recipient with this subcutaneous mycosis developing as an asymptomatic mass in the right forearm. The lesion recurred after an initial local excision, but it is now apparently cured 10 months after wide surgical excision. The patient had previously been a shepherd. A 29-year-old kidney transplant recipient developed a rare deep chromoblastomycosis infection and was cured by aggressive diagnosis and surgical treatment.

We report five cases of factitial panniculitis produced by blunt trauma. Localized forms include Secrétan's syndrome and l'oedeme bleu, which are usually unilateral and involve the hand and forearm, respectively. When the lesions are more generally distributed, the self-induced nature may be less apparent. These patients may have been diagnosed as having Weber-Christian disease, pancreatic fat necrosis, or superficial thrombophlebitis. The microscopic picture of organizing hematoma may be supplemented by the appearance of amorphous polysaccharide masses commingled with iron pigment.

The authors report 2 cases of Kaposi's sarcoma in whose immunosuppressive therapy is implicated. In a 47-year-old Italian man, cutaneous lesions were rapidly aggravated after a renal transplant and treatment with corticosteroids and azathioprine; new lesions appeared close to an arteriovenous fistula of the forearm. In a 83-year-old Algerian man, Kaposi's sarcoma appeared during steroid therapy for bullous pemphigoid. The role of acquired immunosuppression, local predisposition and genetical factors are reviewed and discussed for the pathogenesis of Kaposi's sarcoma.
