[Modified technique for transurethral resection of the prostate with suprapubic drainage and local anesthesia].

PURPOSE: To establish a technique for transurethral resection of the prostate (TURP), combining the use of local anaesthesia with an Amplatz suprapubic tube, in patients at high surgical risk. MATERIALS AND METHOD: A study was carried out in 32 patients who underwent TURP with a 30 F Amplatz suprapubic tube following local anaesthesia. This technique was indicated where surgery presented a general risk, in elderly patients, and for patients refusing to undergo spinal intradural or general anaesthesia. The mean age of the patients was 70 years (61-82 years). The risk of surgery was assessed ac

Termination of a fe/male transsexual patient's analysis: an example of general validity.

The author describes the termination of an analysis, which, while relating to the particular case of a male-to-female transsexual patient, may be relevant to all analysts, particularly those whose patients need to integrate disavowed and split-off parts of themselves. The patient had undergone sex-change surgery at the age of 20. Having lived as a woman thereafter, she had asked for analysis some twenty years later. The author, who discussed the first three years of that analysis in an earlier paper, as well as her hesitation about undertaking it, considers that its termination after seven yea

The surgical management of colorectal complications from irradiation for carcinoma of the cervix.

Bowel injury from radiation given for carcinoma of the cervix is a complex management problem. Prospectively collected computerized data from April 1989 to June 1997 (8 years) were analysed. There were 84 women with a mean age of 60.6 (standard error 1.2) years. Bleeding from radiation proctitis presented much earlier [mean 19.9 (1.9) months after radiotherapy] than either strictures [mean 81.9 (18.4) months, P = 0.008] or rectovaginal fistula [mean 95.5 (39.0) months]. Topical formalin application successfully controlled bleeding in 49 of 55 patients (89.1%) with radiation proctocolitis. The

Orthopaedic surgery on the HIV-positive patient: complications and outcome.

Orthopaedic surgeons practicing in areas with high prevalence of HIV-infected individuals may anticipate that up to 10% of their emergent cases and a highly variable percentage of elective cases will be HIV-positive. Basic science studies have demonstrated impairment of defenses to routine orthopaedic pathogens as well as opportunistic organisms. Clinical studies show that this impairment has not resulted in an increased incidence of postoperative infections or failure of wound healing in the asymptomatic HIV-positive patient. Current medical management seems adequate to prevent increased risk

The Quiet Surgeon: A Qualitative Analysis of the Introverted Experience Throughout a Career in Academic Surgery.

OBJECTIVE: This study seeks to describe the lived experiences of self-identified introverts throughout a career in academic surgery. SUMMARY BACKGROUND DATA: Surgeons and leaders are often perceived as extroverted, with the belief that introverts must project extroversion to succeed as surgeons or leaders. However, many surgical trainees entering academic surgery identify as introverted. METHODS: Semi-structured interviews were conducted with self-identified introverted general surgery residents and attending surgeons at a tertiary academic medical center in the Northeast. Participants were …

Non-infectious skeletal complications in the lower extremity after treatment with a transfemoral bone-anchored prosthesis: a retrospective observational study.

BACKGROUND: The use of bone-anchored prostheses (BAPs) has greatly increased quality of life for lower limb amputees. However, the long-term frequency of skeletal fractures and the need for arthroplasty surgery in the lower extremities following BAP use is scarce. OBJECTIVES: The current study aimed to investigate the frequency of fractures and arthroplasties in the lower limb after BAP surgery with the Osseointegrated Prosthesis for the Rehabilitation of Amputees (OPRA) system. DESIGN: Retrospective cohort study using the OPRA database and medical record review for data collection. SETTING

Thyroid Obstacle During Anterior Cervical Discectomy and Fusion: When to Call Your ENT Surgeon Fellow? A Scoping Review of Individual Patient Data.

Anterior cervical discectomy and fusion for single-level degenerative disease is a common spine surgery procedure, and thyroid lobectomy is a frequent ENT surgery procedure for symptomatic goiter as well. Exceptionally, a pathological thyroid obstacle may hamper a safe surgical approach to the anterior cervical spine. In such a case, staged surgery or a combined four-hand procedure can both be proposed. This review aims to summarize current knowledge regarding single-staged combined surgery for thyroid disease and cervical spine surgery by the anterior approach. Scoping review on the Medline d

The effect of major abdominal surgery on the sublingual microcirculation: an observational study.

PURPOSE: It remains unknown whether the sublingual microcirculation is impaired during noncardiac surgery. We, therefore, aimed to investigate the sublingual microcirculation in patients undergoing major abdominal surgery. Specifically, we sought to test the primary hypothesis that the sublingual microcirculation is persistently impaired during major abdominal surgery. METHODS: In this prospective observational study, we assessed the sublingual microcirculation using vital microscopy before induction of general anesthesia, at the time of surgical incision, every 20 min during surgery, and on

Incidence of adhesive small bowel obstruction after surgery for colorectal cancer in Sweden 2007-2016.

AIM: Population-based data on incidence and risk factors of adhesive small bowel obstruction (SBO) are limited. The aims of this study were to assess the risk of SBO and SBO surgery after bowel resection for colorectal cancer (CRC) and to assess whether this risk is modified by minimally invasive surgery (MIS) and radiotherapy in a retrospective national study. METHODS: CRCBaSe, a nationwide register linkage originating from the Swedish Colorectal Cancer Register, was used to identify Stage I-III CRC patients who underwent resection in 2007-2016, with follow-up throughout 2017. Matched CRC-fr

Dose-dependent relationship between SGLT2 inhibitor hold time and risk for postoperative anion gap acidosis: a single-centre retrospective analysis.

BACKGROUND: Use of sodium-glucose transporter-2 (SGLT2) inhibitors has dramatically increased over the past decade. This medication class predisposes patients to euglycaemic diabetic ketoacidosis, particularly during times of physiologic stress, including fasting and surgery. Beyond case reports and series, a systematic description of perioperative metabolic effects of SGLT2 inhibitors is lacking. METHODS: We examined the degree of anion gap acidosis, controlling for non-ketone anions, in patients undergoing surgery at Massachusetts General Hospital in 2016-22. We constructed a multivariable

Craniomaxillofacial Surgery Techniques of 15th Century Turkish Surgeon Serefeddin Sabuncuoglu.

INTRODUCTION: Şerefeddin Sabuncuoğlu, in his surgical medicine book entitled "Imperial Surgery" (Cerrâhiyetü'l-Hâniyye) that was published in the 15th century, described many diseases using colored miniatures, and explained the repair of these conditions using surgical methods and the required patient care after surgery. In his handwritten treatise, Sabuncuoğlu raised the curtain on his surgical methods with his illustrations. As a first in medical history, Sabuncuoğlu's book should be considered one of the most important original works from that era. MATERIALS AND METHOD: The handwritten "Im

Early Genital Surgery in Disorders/Differences of Sex Development: Patients' Perspectives.

Controversy continues over a proposed moratorium on elective genital surgery in childhood for disorders/differences of sex development (DSD). Empirical evidence on patient preference is needed to inform decision-making. We conducted a multicentre survey by cross-sectional questionnaire in 14 specialized clinics in six European countries. The sample comprised 459 individuals (≥ 16 years) with a DSD diagnosis, including individuals with congenital adrenal hyperplasia (CAH) (n = 192), XY DSD with prenatal androgen effect (A) (n = 150), and without (nA) (n = 117). Main outcome measures were level

Long-term cognitive decline in older subjects was not attributable to noncardiac surgery or major illness.

BACKGROUND: Persistent postoperative cognitive decline is thought to be a public health problem, but its severity may have been overestimated because of limitations in statistical methodology. This study assessed whether long-term cognitive decline occurred after surgery or illness by using an innovative approach and including participants with early Alzheimer disease to overcome some limitations. METHODS: In this retrospective cohort study, three groups were identified from participants tested annually at the Washington University Alzheimer's Disease Research Center in St. Louis, Missouri: t

Use of antimicrobial prophylaxis for major surgery: baseline results from the National Surgical Infection Prevention Project.

HYPOTHESIS: Surgical site infections (SSIs) are a major contributor to patient injury, mortality, and health care costs. Despite evidence of effectiveness of antimicrobials to prevent SSIs, previous studies have demonstrated inappropriate timing, selection, and excess duration of administration of antimicrobial prophylaxis. We herein describe the use of antimicrobial prophylaxis for Medicare patients undergoing major surgery. DESIGN: National retrospective cohort study with medical record review. SETTING: Two thousand nine hundred sixty-five acute-care US hospitals. PATIENTS: A systematic r
