Mitral regurgitation is the most prevalent valve disease worldwide. Recurrent severe primary mitral regurgitation from annuloplasty ring dehiscence is very rare and is associated with adverse outcomes. We present a case where transcatheter edge-to-edge mitral repair with MitraClip was used in high surgical risk patient using a peri-ring approach due to unfavorable anatomy for a conventional intra-ring approach.

Recent advances in device design have resulted in a wide variety of transcatheter treatment options for patients with symptomatic mitral valve disease. Surgery remains the gold standard for patients with symptomatic, primary mitral regurgitation, while transcatheter devices can be considered in higher-risk patients. A case of quadricuspid aortic valve associated with aortic steno-insufficiency was described. A 60-year-old man who had been shown to be suffering from heart murmur was admitted to our hospital. Both aortogram and echocardiogram showed a significant aortic insufficiency with mild stenosis. Between 1970 and 1979 50 patients over the age of seventy years underwent aortic valve replacement. The overall hospital mortality was 14% but has been reduced to 5% following the introduction of cold cardioplegia for myocardial protection. Actuarial survival curves predict a 60% five year survival.
