Outcomes of patients with relapsed/refractory lymphoplasmacytic lymphoma/waldenström macroglobulinemia treated with venetoclax: a multicenter retrospective analysis.

Venetoclax showed promising activity in a small phase II trial in relapsed/refractory Waldenström macroglobulinemia (WM). To report the clinical activity of venetoclax and prognostic factors associated with outcomes in a larger cohort, we retrospectively identified 76 patients with relapsed/refractory lymphoplasmacytic lymphoma (LPL)/WM treated with venetoclax monotherapy at nine US medical centers. The median age at venetoclax treatment initiation was 66 years. MYD88, CXCR4, and TP53 mutations were detected in 65 (94%), 23 (40%), and 10 (22%) patients, respectively. The median number of prior

Validation of a LC-MS/MS Assay for Rapid and Simultaneous Quantification of Cobicistat and Venetoclax in Human Plasma and Serum.

Measuring cobicistat and venetoclax concentrations in human plasma and serum facilitates therapeutic drug monitoring (TDM) and pharmacokinetic (PK) boosting studies. Therefore, the objective of this study was to develop and validate a rapid LC-MS/MS analytical method for the simultaneous determination of cobicistat and venetoclax concentrations in plasma and serum. The method was validated according to EMA and FDA guidelines. Chromatographic separation was performed using a liquid chromatography (LC) system with a C18 column. The elution gradient involved two mobile phases: mobile phase A (amm

Effect of dolutegravir-based antiretroviral therapy on glycemic control in female mice.

Dolutegravir (DTG), is recommended for all people with HIV, including pregnant women. Weight gain and hyperglycemia have been reported with DTG use, as well as a signal for neural tube defects (NTDs) that has waned over time. Obesity and hyperglycemia are risk factors for NTDs. We explored the impact of DTG-based antiretroviral therapy (ART) on weight gain and glucose homeostasis in female mice. C57BL/6 mice were treated daily for 9 weeks with water (control), 1xDTG (2.5 mg/kg DTG + 33.3/50 mg/kg emtricitabine/tenofovir-yielding therapeutic levels), or 5xDTG (12.5 mg/kg + 33.3/50 mg/kg emtrici

Magnetic Field-Driven Electrogenic Scaffold Enhances the Nerve Regeneration.

BACKGROUND: Neural conduits (NGCs) are widely used for peripheral nerve regeneration but have shown limited therapeutic effects. Electrical stimulation combined with NGCs is becoming a new treatment paradigm. Conventional electrical stimulation approaches for nerve regeneration are typically hindered by inconvenience, infection, and may cause secondary damage. METHODS: This study developed a non-contact electrical stimulation platform using a magnetic field-coupled polycaprolactone/conductive polymer poly(3,4-ethylenedioxythiophene)/black phosphorus nerve conduit. This composite nerve conduit

Upadacitinib as rescue therapy for corticosteroid failure acute severe ulcerative colitis: an Asian experience from Taiwan.

PURPOSE: Acute severe ulcerative colitis (ASUC) is a medical emergent condition, and approximately one-third of patients with ASUC do not respond to corticosteroid. Whether small molecule drugs are efficient and safe for salvage therapy of ASUC is not well-understood. METHODS: Consecutive patients with ASUC who failed responding to first-line corticosteroid were retrospectively enrolled. Clinical, laboratory, endoscopic, and pathological data were analyzed. RESULTS: Five Asian male patients (median age of 38.9 years old) with ASUC who received salvage therapy with upadacitinib were enrolled.

Non-muscle myosin II inhibition at the site of axon injury increases axon regeneration.

Motor axon regeneration following peripheral nerve injury is critical for motor recovery but therapeutic interventions enhancing this are not available. We conduct a phenotypic screen on human motor neurons and identified blebbistatin, a non-muscle myosin II inhibitor, as the most effective neurite outgrowth promotor. Despite its efficacy in vitro, its poor bioavailability limits in vivo application. We, therefore, utilize a blebbistatin analog, NMIIi2, to explore its therapeutic potential for promoting axon regeneration. Local NMIIi2 application directly to injured axons enhances regeneration

Comparative Effectiveness and Safety of Adalimumab, Secukinumab, and Upadacitinib in Psoriatic Arthritis: A Prospective Cohort Study Based on PARWCH Cohort.

Psoriatic arthritis (PsA) is a chronic inflammatory disease, with prevalence among psoriasis patients ranging from 6% to 42% across populations. Although targeted therapies such as adalimumab (ADA), secukinumab (SEC), and upadacitinib (UPA) have demonstrated efficacy in randomized controlled trials, real-world head-to-head comparisons remain limited. This study aimed to compare the real-world effectiveness and safety of ADA, SEC, and UPA in PsA patients. We conducted a prospective cohort study using data from the PARWCH database. PsA patients treated with ADA, SEC, or UPA were included and fol

Targeting the CXCL12/CXCR4 pathway by an optimized derivative or EPI-X4 preserves chondrocyte function and offers a novel therapeutic approach in rheumatic diseases.

BACKGROUND: The C-X-C motif chemokine 12 (CXCL12) and its receptor CXCR4 are pivotal in tissue regeneration and inflammation, yet their role in osteoarthritis (OA) remains ambiguous. However, it is assumed that the CXCL12/CXCR4 axis likely contributes to OA progression through subchondral bone-cartilage crosstalk. This study compares the efficacy of the CXCR4 inhibitors AMD31000 and novel endogenous peptide inhibitors in human cartilage and isolated chondrocytes (hAC). METHODS: Human cartilage and hAC were obtained from OA patients undergoing arthroplasty. Expression of both CXCL12 receptors

Antiretroviral-Induced Toxicity in Umbilical Cord Blood-Derived Haematopoietic Stem/Progenitor Cells.

Improvements in administration and efficacy of antiretroviral therapy (ART) have reduced rates of mother-to-child transmission (MTCT) of human immunodeficiency virus (HIV) to < 2%. However, in utero exposure to antiretrovirals (ARVs) leads to abnormalities in HIV-exposed uninfected (HEU) infants. We determined the effect of five ARVs on human umbilical cord blood (UCB)-derived haematopoietic stem/progenitor cells (HSPC) with the aim of exploring a potential causal relationship with haematological abnormalities. Efavirenz (EFV) was cytotoxic to HSPCs alone and in combination with tenofovir diso

Effect of weight gain on blood pressure in Ugandan persons with HIV on dolutegravir/lamivudine/ tenofovir disoproxil fumarate over 48 weeks.

BACKGROUND: Most people living with HIV in low and middle-income countries are taking fixed dose combination tenofovir disoproxil fumarate/lamivudine/dolutegravir (TLD). Dolutegravir use has been associated with weight gain, a known risk factor for hypertension. We aimed to determine if weight gain in Ugandan anti-retroviral therapy (ART) naïve patients on TLD correlated with increase in blood pressure. METHODS: We analyzed data from the 'Glucose metabolism changes in Ugandan persons with HIV (PLHIV) on Dolutegravir (GLUMED)' study which was a prospective cohort study with ART naïve persons w
