Barriers to breastfeeding and targeted breastfeeding social transfer programs in São Paulo, Brazil: a qualitative study.

BACKGROUND: Exclusive breastfeeding (EBF) is recommended for the first six months of life, yet only 48% of infants under six months are exclusively breastfed worldwide. Breastfeeding rates vary significantly across regions, countries, and even within cities. Brazil reports national rates of EBF of approximately 50%; however, there are notable in-country socioeconomic disparities, where EBF prevalence is lower in low-income communities compared to the wealthier communities. This study aims to: 1) investigate barriers to breastfeeding by exploring perceptions and experiences of breastfeeding in

Long-term follow-up of patients supported with the HeartWare left ventricular assist system.

The aim of the present study is to report our long-term experience with the HeartWare Ventricular Assist System (HVAD). Between July 2009 and February 2018, a total of 103 patients (mean age 50.0 ± 14.4, range 28-74 years; 22 females) received HVAD implantation in a single center institution. A total of 26 (25.4%) patients were in cardiogenic shock preoperatively and received extracorporeal life support (ECLS) prior to HVAD implantation. The aim of left ventricular assist device (LVAD) implantation was bridge to transplantation (BT) in 59 (57.3%), destination therapy (DT) in 28 (27.2%), and br

A trial assessing N-3 as treatment for injury-induced cachexia (ATLANTIC trial): does a moderate dose fish oil intervention improve outcomes in older adults recovering from hip fracture?

BACKGROUND: Proximal femoral fractures are associated with increased morbidity and mortality. Pre-existing malnutrition and weight loss amongst this patient group is of primary concern, with conventional nutrition support being largely ineffective. The inflammatory response post proximal femoral fracture surgery and the subsequent risk of cachexia may explain the inability of conventional high energy high protein management to produce an anabolic response amongst these patients. Omega-3 fatty acids derived from fish oils have been extensively studied for their anti-inflammatory benefits. Due t

Convergent evidence for 2',3'-cyclic nucleotide 3'-phosphodiesterase as a possible susceptibility gene for schizophrenia.

CONTEXT: Convergent data make 2',3'-cyclic nucleotide 3'-phosphodiesterase (CNP) a candidate gene for schizophrenia. Reduced expression has been reported in the schizophrenic brain. The CNP gene maps to a region to which we have reported linkage to schizophrenia. Mice in which the CNP gene has been knocked out display central nervous system pathological characteristics reminiscent of some features observed in schizophrenia. 2',3'-Cyclic nucleotide 3'-phosphodiesterase is used as a marker of myelin-forming cells and is detectable in cells of oligodendrocyte lineage throughout life. Because CNP

Reliability, failure mode, and stress distribution in conical connections with different types of retention.

STATEMENT OF PROBLEM: The tapered interference fit (TIF) implant system, a screwless system with purely frictional retention, was developed to minimize prosthetic complications associated with screw loosening. However, evidence regarding its biomechanical behavior is scarce. PURPOSE: The purpose of this in vitro study was to compare the reliability, failure mode, and stress distribution of frictional TIF implant systems with straight and custom in-office angled abutments and conventional taper integrated screw-in (TIS) systems. MATERIAL AND METHODS: One hundred and five implants with single

RadioMe: an automated home-based radio, music playlist, and diary reminder system: Report on recruitment, music compilation, and listening, and preliminary testing of heart rate activated music.

BACKGROUND: One of the leading reasons for early admission to a care home in dementia is the escalation of neuropsychiatric symptoms (NPS), and music listening can help regulate these symptoms. AIMS: RadioMe was a project designed for people living at home with dementia to build a system to help them maintain the highest quality of life there for as long as possible, with three functional components: 1. Streaming radio; 2. Providing pre-recorded spoken diary reminders; 3. interrupting the radio with pre-compiled playlists when a wrist-worn heart rate (HR) monitor detects stress. This article

Improving adherence to pulmonary hypertension screening in patients with systemic sclerosis: Overcoming the provider-level barriers.

BACKGROUND: Pulmonary arterial hypertension is a life-limiting complication in patients with systemic sclerosis. Current recommendations suggest a minimum screening of an annual transthoracic echocardiogram and pulmonary function test. We hypothesize that modifiable provider-level hurdles contribute to inadequate screening and can be alleviated. METHODS AND RESULTS: We performed a longitudinal study over 23 months of all systemic sclerosis patients seen in pulmonary and/or rheumatology clinics. Pulmonary arterial hypertension screening was measured monthly as percentage of patients with up-to

Long-term impact of frailty on mortality and quality of life in mechanically ventilated acute myocardial infarction patients undergoing percutaneous coronary intervention.

BACKGROUND: Frailty is a recognized predictor of adverse outcomes after percutaneous coronary intervention (PCI), but its long-term association with mortality and health-related quality of life (HRQoL) remains uncertain. This study evaluated the three-year association between discharge frailty and subsequent frailty, mortality, and HRQoL in post-PCI patients. METHODS: This retrospective cohort study included patients with acute myocardial infarction (AMI) who underwent PCI, required ICU/CICU admission with mechanical ventilation and/or advanced life support, and remained alive at routine 6-mo

The EORTC updated breast cancer quality of life questionnaire EORTC QLQ-BR42: A psychometric study with Spanish patients.

BACKGROUND: The EORTC Quality of Life Group recently developed the new version of its breast-cancer-specific quality of life questionnaire, the EORTC QLQ-BR42. This questionnaire updates EORTC QLQ-BR23, which was published in 1996. Since then, significant changes have occurred in breast cancer treatment. The aims of this study were to evaluate the psychometric properties of the QLQ-BR42 in Spanish breast cancer patients and compare the results with those of the EORTC international validation study. METHODS: A retrospective analysis of QLQ-BR42 data from three Spanish breast cancer patient coho

Nebulized Nitroglycerin Improves Carotid Blood Flow During Cardiopulmonary Resuscitation in a Swine Model of Cardiac Arrest.

OBJECTIVES: Pulmonary vasodilators have shown promise in reducing pulmonary vascular resistance during CPR. Most are not currently available outside of an intensive care unit (ICU) setting. Nitroglycerin is widely used by emergency medical services to treat chest pain. If beneficial, the inhalation route of nitroglycerin administration could be implemented by basic life support personnel at the scene of a cardiac arrest. The aim of this investigation was to assess the hemodynamic effects of nebulized nitroglycerin (NIN) in a swine model of ventricular fibrillation (VF)-induced cardiac arrest.

Phytotherapy as an adjunct to the treatment of rheumatoid arthritis - a systematic review of clinical trials.

BACKGROUND: Rheumatoid arthritis (RA) is a chronic systemic inflammatory autoimmune disease whose incidence is on an upward trend. The condition leads to progressive joint destruction, manifested by symptoms such as morning stiffness, pain, and swelling, all of which significantly reduce patients' quality of life. Standard therapeutic approaches, despite significant progress, are associated with side effects and do not always achieve sustained remission of the disease. For this reason, there is growing interest in complementary methods, including phytotherapy. Numerous plant raw materials exhi

Early mobilization for children in intensive therapy: A protocol for systematic review and meta-analysis.

INTRODUCTION: Intensive care units focus primarily on life support and treatment of critically ill patients, but there are many survivors with complications, such as generalized muscle disorders, functional disability and reduced quality of life after hospital discharge, resulting from prolonged stays in these units. The current evidence suggests that early mobilization-based rehabilitation (exercise initiated immediately after the patient's significant physiological changes have stabilized) in critically ill adults can alleviate these complications from immobility and critical illness. Substance abuse and cancer.

Substance abuse is a health problem with serious psychological and psychiatric dimensions and multiple social and economic consequences. Cancer is a disease that threatens not only life and physical integrity but mental health as well. Oncology patients suffer from mental disorders in high rates, especially from depression and anxiety. The role of substance abuse in the pathogenesis of cancer is studied systematically, since there are research data supporting the mutagenic effects of certain substances. It has been supported that a possible dysregulation of the immune system is linked to the o
