Unmasking a rare variant: 17β-HSD3 deficiency in a male infant with disorders of sex development.

17-beta hydroxysteroid dehydrogenase-3 enzyme is an enzyme expressed almost exclusively in the testes; it plays a crucial role in gonadal differentiation by catalysing the conversion of androstenedione (Δ4) to testosterone (T). The enzyme is encoded by the 17-B-hydroxysteroid dehydrogenase-3 (HSD17B3) gene. Mutations in this gene result in undervirilisation of the external male genitalia. We present a case of a child born with ambiguous genitalia. The gonads were palpable in the inguinal canal bilaterally and their presence was confirmed by ultrasound. This raised the suspicion of 46XY disorde

Novel Compound Heterozygous Mutation of the ABCA3 Gene in a Patient with Neonatal-Onset Interstitial Lung Disease.

Background: Children's interstitial and diffuse lung diseases, commonly referred to as "chILDs", include around 200 rare conditions that disrupt normal lung function. They are classified, based on etiopathogenesis, into several subgroups, having a varied and multifaceted clinical presentation depending on the type of genetic mutation present. Methods and Results: We describe the case of a late preterm newborn presenting soon after birth with respiratory distress syndrome poorly responsive to surfactant administration, in whom a targeted gene panel analysis for pulmonary congenital diseases, pe

Challenging diagnosis: unusual presentation of Unicentric Castleman disease-a case report.

Castleman's Disease is an idiopathic rare lymphoproliferative disorder that is clinically Classified to multicentric to unicentric types. Only few cases were reported in children, with majority of them are unicentric and usually located in the mediastinum. We report a unique case of a 13-year-old boy who presented with a palpable enlarged mass in the left inguinal region without any constitutional symptoms. Surgical removal of this mass was essential to exclude worrying causes. Pathologic examination revealed proliferative changes consistent with Castleman's disease plasma cell type which is o

Relational trauma.

The aim of this paper is to briefly present the current knowledge on the definition of relational trauma, place in classifications, prevalence, consequences, and applied therapeutic methods. Reports from many countries indicate that in the relationship with adults, usually parents, on whom the child is dependent for many years, various forms of abuse may occur, related not only to violence, but also to emotional, physical, and intellectual neglect. Behind them, the unregistered area of aversive experiences of children who enter adulthood with the experience of relational trauma is hidden. Thes

Navigating parental decision-making: Intersex surgeries in Italy.

Decision-making dynamics in Italian paediatric care for Intersex and Variations of Sex Characteristics (VSC) involve a complex interplay between medical practices, parental perspectives, and socio-cultural factors. This article explores how medical professionals and parents make decisions amid cultural debates on gender, the body, and autonomy. It addresses aspects of why surgical intervention, with limited child involvement, is often seen as the 'only option' in the 'conservative' culture of Italy. The article continues to highlight the rise of parent-led human rights-based activism in Italy,

Identification of a pathogenic variant and pre-implantation genetic testing for a Chinese family affected with split-hand/foot malformation.

Split-hand/foot malformation is a serious congenital limb malformation characterized by syndactyly and underdevelopment of the phalanges and metatarsals. In this study, we reported a case of a fetus with hand-foot cleft deformity. Whole exome and Sanger sequencing were used to filter out candidate gene mutation sites and provide pre-implantation genetic testing(PGT) for family members. Genetic testing results showed that there was a homozygous mutation c.786G>A (p.Trp262*) in the fetal WNT10B, and both parents were carriers of heterozygous mutations. PGT results showed that out of the two blas

Use of transcranial motor evoked potentials (TcMEPs) in spine deformity surgery in a case of Charcot-Marie-tooth disease-what we should know? A case report.

INTRODUCTION: Charcot Marie tooth disease (CMTD) is also known as Hereditary sensory motor neuropathy. It poses difficulties in attaining intra-operative neuromonitoring signals for deformity correction surgery. In this case report, we intent to mention key points for obtaining good neuromonitoring signals in these cases which increases the safety in scoliosis surgery. CASE PRESENTATION: We present a 14-year-old boy, known case of CMTD, presented with progressive deformity of the back. The child was wheelchair-bound and could walk only a few steps with support. He was unable to maintain a sit

Like Father, Like Daughter - Ectodermal Dysplasia-Syndactyly Syndrome: A Case Report.

Ectodermal dysplasia-syndactyly syndrome 1 (EDSS1) is an exceedingly rare condition associated with mutations in the PVL4 gene. It is characterised by sparse, brittle hair, eyebrows and eyelashes, abnormal dentition and nails, along with bilateral cutaneous syndactyly involving the fingers and toes. We report a 2-year-old girl who presented to us with bilateral complete simple syndactyly of the third and fourth web spaces of the hands, along with bilateral syndactyly of both feet involving the second to fourth toes. Upon examination, sparse hair and eyebrows, along with abnormal dentition, wer

Successful Management of Traumatic Cervical Spondyloptosis with Incomplete Neurological Deficit in a Child: A Case Report.

BACKGROUND: Spondyloptosis is a condition in which one vertebral body is completely displaced over the corpus of the adjacent one. It is more common in the lumbar region of the spine, but it very rarely occurs in the cervical spine. In this study, we report a case of post-traumatic cervical spine spondyloptosis in a child. CASE REPORT: We describe a case of a 14-year-old male, who was admitted for cervicalgia with incomplete neurological deficit as a result of falling from a tree. Computed tomography (CT) of the neck revealed a posterior C5-C6 spondyloptosis with anterior spondylolisthesis of

Segmental Absence of Intestinal Musculature: An Enticing Predicament.

Segmental absence of intestinal musculature (SAIM) was first identified as a cause of intestinal perforation and remains difficult to diagnose clinically. Diagnosing SAIM preoperatively can be difficult; however, detection during the surgical procedure may be feasible if gross observations reveal bowel alterations such as a thinned-out wall, signs of gangrenous changes, or adjacent bowel dilatation. A final diagnosis can be made with the help of histopathology, making it a gold standard. Even though this condition is more prevalent among newborns, only a handful of cases have been published in

Sodium-dependent multivitamin transporter defects: a rare cause of recurrent vomiting and faltering growth.

We describe the case of a previously fit child with recurrent vomiting, faltering growth, persistent diarrhoea and rashes who was diagnosed with a sodium-dependent multivitamin transporter (SMVT) defect. Whole exome sequencing revealed he was homozygous for a SLC5A6 missense variant. The SLC5A6 gene produces SMVTs, which are expressed in various tissues including the intestine, brain, liver, lung, kidney, cornea, retina and heart. It plays a major role in the uptake of biotin, pantothenate and lipoate in the digestive system and transporting B-group vitamins across the blood-brain barrier. Thi

Family Functioning Style as a Predictor of the Quality of Cognitive Functioning of Primary School Students With ADHD.

OBJECTIVE: This study aimed to specify whether family communication and satisfaction are predictors of a child's executive functions and whether attention deficit hyperactivity disorder (ADHD) severity lies in the pathway between these variables. METHOD: Two hundred Polish children with ADHD, aged 10 to 13, were tested using Conners 3, the PU1 Battery of Cognitive Tests and Stanford-Binet Intelligence Scale, Fifth Edition (SB5). Parents filled out the FACES IV-SOR questionnaire. Structural equation modeling (SEM) was used to test the hypotheses. RESULTS: The quality of family communication a

Frontline support services for boys who have experienced child sexual exploitation: A thematic review of survey data from seven countries.

Child sexual exploitation and abuse (CSEA) affects all children, but research on the needs and experiences of boys is lacking, support services are limited, and workers lack specialized training to meet their specific needs. This paper explores the perspectives and experiences of 404 Frontline Support Workers providing services to children with CSEA experiences in seven countries, considering trends and implications for boys. A mixed-methods online survey of 121 questions explores characteristics of cases, perceived vulnerabilities, and gender-based challenges in CSEA response. A descriptive a

Congenital giant fibroepithelial polyp of the scrotum in an infant: the first case report from China.

Giant fibroepithelial polyp (FP) of the scrotum in infants is a rare disease. We reported the first case of FP in China. The child was only 9 months and 12 days old and was admitted to the hospital due to rapid growth and rupture of the scrotal mass. The patient underwent scrotal exploration under general anesthesia, and the mass was cystic-solid with clear boundaries. The tumor did not invade the sarcolemma of the scrotum and testicular tissue. The intraoperative pathological frozen section tended to be benign, and the scrotum's tumor and subcutaneous pedicle tissue were removed entirely afte

Limited dorsal myeloschisis involving one hemicord of a split cord malformation - a "hemi-LDM".

This is a case report of an exceedingly rare case of a limited dorsal myeloschisis (LDM) with its stalk inserted on the midline dorsal surface of one of a pair of hemicords in a type II split cord malformation. This entity, literally a "hemi-LDM," has been seen only once by the senior author in his catalogue of over 200 cases of LDM (Pang et al., 2020), nor has it been reported elsewhere before. We postulate that here the mechanism of focal nondisjunction of the hemi-neural plate during primary neurulation, which produces LDMs, occurs at the cusp of the consecutive developmental stages of gast

Rare case of giant myoepithelioma in minor salivary glands of palate in a 9-year-old child.

Myoepitheliomas (MEs) are extremely rare benign neoplasms composed of ectodermally derived contractile smooth muscle cells (myoepithelial cells). Various tissues such as the salivary glands, breast, larynx, and sweat glands show the presence of these myoepithelial cells. They occur, principally, in the parotid gland and infrequently in minor salivary glands. The term "Myoepitheliomas" was first coined by Sheldon in 1943. It is an uncommon salivary gland tumor which accounts for < 1% of all major and minor salivary gland tumors. Batasakis considers the ME to be "one-sided" variant at the opposi

Anophthalmia, Global Developmental Delay, and Severe Dysphagia in a Young Girl With 14q22q23 Microdeletion Syndrome.

14q22q23 microdeletion syndrome, also called Frias syndrome, is an extremely rare partial deletion of the long arm of chromosome 14 characterized by the anomalies of the pituitary gland, eyes, and hand/foot. Intellectual disability and facial dysmorphism are other common manifestations. Haploinsufficiency of the genes bone morphogenetic protein 4 (BMP4) and orthodenticle homeobox 2 (OTX2) accounts for most of the phenotypic abnormalities seen in these patients. There are only a few cases reported with Frias syndrome in the literature, and there are multiple variations present, which are not we

Bile pulmonary embolism-related sudden death following an abdominal firearm injury: An autopsy case report and review of literature.

Bile pulmonary embolism (BPE) is a rare type of non-thrombotic pulmonary embolism (NTPE). For bile emboli to pass through the bloodstream to the lungs, there must be a connection created between the biliary and the venous vessels. This article reports a fatal BPE in a 12-year-old child who died 14 days following an abdominal firearm injury. The right lobe of the liver had a yellowish-green stained lacerated track along with bile collection. Both lungs were congested with peripheral dark areas in all lobes. Microscopically, many interstitial pulmonary arterioles were occluded by bile emboli alo
