Complete excision of the metastatic tumor is associated with long-term survival and low perioperative mortality. Surgical repair of a postinfarct ventricular septal defect (VSD) remains a difficult surgical challenge associated with a significant operative mortality. Between 1972 and 1995, 179 patients with a postinfarct VSD have undergone operation in this institution. There were 118 males and 61 females, with a mean age of 66 years (range 43 to 80). We report on a case of an 11-year-old asymptomatic child with aortico-left ventricular tunnel arising from the left aortic sinus. A young Coloured man with a history of acute transmural anteroseptal and anterolateral myocardial infarction presented with angina pectoris. Skeletal muscle biopsy showed unequivocal evidence of polyarteritis nodosa. Complete cardiac catheterization, left ventricular cine angiography, and selective coronary arteriography demonstrated a large aneurysm involving the apex and the left ventricular free wall. BACKGROUND: The ideal substitute for in situ reconstruction of aortic graft or native aortic infection has yet to be defined.
