A prospective study using image analysis to assess the efficacy of a topical treatment kit for mild onychomycosis.

BACKGROUND: Onychomycosis is a fungal infection of the nails and is one of the most common dermatological infections worldwide. Infection occurs predominantly in the nails of the feet and if left untreated patients, particularly in at-risk populations, can develop more serious complications, including pain, fissuring and secondary infections. Fungal infections are also contagious and may spread to other nails or to family members. Topical treatments can be effective for mild cases of the disease and typically have fewer contraindications. However, generation of robust efficacy data for topical

Long-term alteration of the hypothalamic-pituitary-adrenal axis in children undergoing cardiac surgery in the first 6 months of life.

Children with congenital heart disease (CHD) have poorer neurodevelopmental and psychological outcomes. The mechanisms underlying this remain unclear. One mechanism could be that the stressful experience of cardiac surgery early in life influences long-term hypothalamic-pituitary-adrenal (HPA) axis regulation. Dysregulation of the HPA axis has been linked to poorer neurocognitive and psychological outcomes in other study populations. This case-control study aims to compare HPA-axis regulation (circadian rhythm and reactivity) using salivary cortisol in 3- to 5-year-olds with CHD who did and di

Non-ischaemic cardiomyopathy, sudden death and implantable defibrillators: a review and meta-analysis.

OBJECTIVE: The recent Danish Study to Assess the Efficacy of ICDs in Patients with Non-ischemic Systolic Heart Failure on Mortality (DANISH) trial suggested that implantable cardioverter defibrillators (ICDs) do not reduce overall mortality in patients with non-ischaemic cardiomyopathy (NICM), despite reducing sudden cardiac death. We performed an updated meta-analysis to examine the impact of ICD therapy on mortality in NICM patients. METHODS: A systematic search for studies that examined the effect of ICDs on outcomes in NICM was performed. Our analysis compared patients randomised to an IC

[The direct and computed radiographic assessment of vertebral locations in children with acute lymphatic leukemia].

The children affected with acute lymphoblastic leukemia (ALL) often exhibit secondary lesions of the spine. The diagnosis of spinal involvement is currently made by means of conventional radiography in postero-anterior and lateral views. The osteolytic lesions of the vertebral body present with collapse of the vertebral plates in a wide range of severity. Body evaluation is usually made by comparison with the adjacent vertebrae. Since leukemia is a systemic disease, several vertebrae are possibly involved in each case. In the attempt to develop a more sensitive method, which is less dependent

Cumulative incidence of advanced breast cancer in women aged 40-49 years in the Japan Strategic Anti-cancer Randomised Trial (J-START) of adjunctive ultrasonography: a prespecified secondary analysis.

BACKGROUND: The J-START randomised controlled trial found that adjunctive ultrasonography was associated with significantly higher rates of breast cancer detection than mammography alone. This report aims to evaluate the long-term effect of adjunctive ultrasonography screening on the cumulative incidence of advanced breast cancer as a prespecified secondary outcome of J-START. METHODS: We enrolled asymptomatic women in 42 study sites in 23 of 47 prefectures in Japan. Eligible women were aged 40-49 years without a history of breast cancer, including in-situ cancer, or other cancers in the prev

Primary prevention of ischaemic cardiovascular disorders with antiplatelet agents.

In those who have already survived myocardial infarction (MI) or stroke, or have had a transient ischaemic episode (TIA), daily low dose aspirin (ASA) reduces the risk of recurrences by an amount that greatly exceeds the risk of serious bleeding (secondary prevention). ASA is therefore recommended for these people. However, in primary prevention-reducing risk in those so far free of clinically manifest episodes-the benefit is of the same order as the bleeding hazard, (which is much the same in both primary and secondary prevention contexts). The use of other effective agents such as statins fu

Spinal manipulation for primary and secondary dysmenorrhoea.

BACKGROUND: Dysmenorrhoea refers to the occurrence of painful menstrual cramps of uterine origin and is a common gynaecological condition. One possible treatment is spinal manipulation therapy. One hypothesis is that mechanical dysfunction in certain vertebrae causes decreases spinal mobility. This could affect the sympathetic nerve supply to the blood vessels supplying the pelvic viscera, leading to dysmenorrhoea as a result of vasoconstriction. Manipulation of these vertebrae increases spinal mobility and may improve pelvic blood supply. Another hypothesis is that dysmenorrhoea is referred p

Dose Comparison (1 mL versus 2 mL) of Superparamagnetic Tracer for Sentinel Node Biopsy in Early Breast Cancer: Real-World Analysis from 456 Patients.

BACKGROUND: Superparamagnetic iron oxide (SPIO) tracers offer a radiation-free technique for sentinel lymph node biopsy (SNB) in early breast cancer (eBC). However, the data on optimal administration in daily practice, such as the optimal tracer volume and injection method, are still lacking. METHODS: In this real-world data analysis, patients with clinically node-negative eBC who underwent SNB with SPIO (Magtrace®) between January 2020 and December 2022 were included. Primary endpoint was the impact of tracer volume on the detection rate. Secondary endpoints evaluated number of removed senti

Trends and predictors of caesarean section in Thailand before and during the COVID-19 pandemic: a retrospective analysis of national hospitalisation data under the Universal Coverage Scheme.

OBJECTIVES: Since 1985, the international healthcare community has recommended the ideal rate of caesarean section (CS) to be 10%-15% at the national level. The literature has reported that overused CS without necessary medical indications can be harmful to both maternal and child health. To generate evidence to support policy on CS, this study evaluated the trend over time of CS in Thailand during January 2016 to October 2021 (which included the COVID-19 pandemic period) and explored predictors of CS use. DESIGN AND SETTING: This study was a retrospective secondary data analysis of de-identi…

Inter-facility transfers for emergency obstetrical and neonatal care in rural Madagascar: a cost-effectiveness analysis.

CONTEXT: There is a substantial lack of inter-facility referral systems for emergency obstetrical and neonatal care in rural areas of sub-Saharan Africa. Data on the costs and cost-effectiveness of such systems that reduce preventable maternal and neonatal deaths are scarce. SETTING: We aimed to determine the cost-effectiveness of a non-governmental organisation (NGO)-run inter-facility referral system for emergency obstetrical and neonatal care in rural Southern Madagascar by analysing the characteristics of cases referred through the intervention as well as its costs. DESIGN: We used secon

Catheter Ablation of Ventricular Arrhythmia in Patients With an Implantable Cardioverter-Defibrillator: A Systematic Review and Meta-analysis.

BACKGROUND: Implantable cardioverter-defibrillator (ICD) shocks are associated with higher rates of mortality and reduced quality of life. In this study we aimed to investigate the effectiveness of catheter ablation (CA) of ventricular tachycardia in patients with an ICD. METHODS: An electronic literature search was conducted to identify randomized controlled trials that compared CA vs control. The primary outcomes were recurrence of ventricular arrhythmia (ventricular tachycardia or ventricular fibrillation) and mortality. Kaplan-Meier curves for these outcomes were digitized to obtain indiv

Effect of intra-arrest trans-nasal evaporative cooling in out-of-hospital cardiac arrest: a pooled individual participant data analysis.

BACKGROUND: Randomized trials have shown that trans-nasal evaporative cooling initiated during CPR (i.e. intra-arrest) effectively lower core body temperature in out-of-hospital cardiac arrest patients. However, these trials may have been underpowered to detect significant differences in neurologic outcome, especially in patients with initial shockable rhythm. METHODS: We conducted a post hoc pooled analysis of individual data from two randomized trials including 851 patients who eventually received the allocated intervention and with available outcome ("as-treated" analysis). Primary outcome

Evaluating the utility of computed tomography of the chest for gastric cancer staging.

BACKGROUND: International guidelines recommend routine computed tomography (CT) of the chest for gastric cancer staging. In Asian countries, where the incidence of pulmonary metastases is less than 1%, some guidelines recommend chest CT only for gastroesophageal junction cancers. If the incidence of pulmonary metastases is also low in Canada, routine chest CT may not be beneficial. METHODS: We performed a retrospective review of patients in northern Alberta with newly diagnosed gastric cancer from January 2010 to July 2016. The primary aim of the study was to determine the incidence of pulmon
