Outcomes of Displaced Greater Tuberosity Fractures Treated with Screw Fixation and Double Row Suture Bridge Technique.

INTRODUCTION: Closed treatment of comminuted, displaced greater tuberosity fractures (GTFs) can result in nonunion or malunion, causing rotator cuff dysfunction and/or subacromial impingement. This study evaluates the outcomes and complications of combined suture anchor and screw fixation technique for the treatment of GTFs. MATERIALS AND METHODS: This retrospective case series included 13 patients with displaced GTFs (> 5 mm) who underwent screw and double-row suture bridge fixation from 2019 to 2024 at a single institution. Patient demographics, injury mechanisms, fracture characteristics,

Prespinal Versus Conventional Hemicontralateral C7 Nerve Transfer in the Treatment of Total Brachial Plexus Roots Avulsion Injuries: A Retrospective Study With a Minimum Follow-Up Period of 4 Years.

PURPOSE: Contralateral C7 (CC7)-to-median nerve transfer has been commonly used to restore hand function in brachial plexus injury. To shorten the nerve graft, the prespinal route was described and achieved direct coaptation when combined with humeral shortening osteotomy. The limb was positioned at 0° shoulder abduction and neutral head position. Given our concern about donor-site morbidity when harvesting the whole CC7 nerve and tension across the neurorrhaphy site after mobilization, we aimed to describe our modified prespinal route and compare its outcomes and complications with the conven

Functional passive range of motion of individuals with chronic cervical spinal cord injury.

OBJECTIVE: Functional passive range of motion (PROM) requirements for individuals with cervical spinal cord injury (SCI) are clinically accepted despite limited evidence defining the specific PROM needed to perform functional tasks. The objective of this investigation was to better define the minimum PROM needed for individuals with cervical SCI to achieve optimal functional ability, and as a secondary outcome gather self-reported standardized functional data via the Spinal Cord Independence Measure-III (SCIM-III), and the Spinal Cord Injury Functional Index (SCI-FI). DESIGN: Observational co

The multiscale structural and mechanical effects of mouse supraspinatus muscle unloading on the mature enthesis.

The musculoskeletal system is sensitive to its loading environment; this is of particular concern under conditions such as disuse, paralysis, and extended-duration space flight. Although structural and mechanical changes to tendon and bone following paralysis and disuse are well understood, there is a pressing need to understand how this unloading affects the bone-tendon interface (enthesis); the location most prone to tears and injury. We therefore elucidated these effects of unloading in the entheses of adult mice shoulders that were paralyzed for 21 days by treatment with botulinum toxin A.

The active compression test: a new and effective test for diagnosing labral tears and acromioclavicular joint abnormality.

Labral tears and acromioclavicular joint abnormalities were differentiated on physical examination using a new diagnostic test. The standing patient forward flexed the arm to 90 degrees with the elbow in full extension and then adducted the arm 10 degrees to 15 degrees medial to the sagittal plane of the body and internally rotated it so that the thumb pointed downward. The examiner, standing behind the patient, applied a uniform downward force to the arm. With the arm in the same position, the palm was then fully supinated and the maneuver was repeated. The test was considered positive if pai

Orthopedic problems in athletes.

Physicians and patients must recognize the demands of the sport involved, as well as the individual's ability to meet those demands. Careful analysis of the sport and examination of the patient to assess recovery and chances of reinjury are mandatory, if he is to enjoy sports after injury. Physicians should become familiar with padding, shoes, warm-up programs, preparticipation examinations, and protective equipment and devices, all of which are helpful in preventing sports injuries. We have touched only briefly on the epidemic problem of quality of life trauma that is part of our society. Bec

A review of current literature on physiological tests and soft tissue biomarkers applicable to work-related upper limb disorders.

Work-related upper limb disorders (WRULD) are among the most commonly reported occupational illnesses. Epidemiological evidence of work-relatedness has been reported for a number of conditions, including carpal tunnel syndrome, hand/wrist tendinitis, shoulder tendinitis and hand-arm vibration syndrome. A range of electrodiagnostic techniques and psychophysical tests has been used to assess neurological dysfunction associated with WRULD, whereas only very few studies have examined biochemical markers of soft tissue injury. This report reviews key literature on physiological tests and biochemica

Clinical Efficacy Comparison of Bilaterally Threaded Elastic Locking Intramedullary Nailing Versus Minimally Invasive Percutaneous Locking Plate Fixation in Treating Midshaft Clavicle Fractures.

AIM: Midshaft clavicle fractures are a common form of upper extremity injury, with various treatment modalities available to stabilize displaced midshaft clavicular fractures. However, the optimal surgical method remains a matter of controversy. This study aimed to compare the clinical efficacy of bilaterally threaded elastic locking intramedullary nailing (BTELIN) versus minimally invasive percutaneous locking plate (MIPLP) fixation in treating midshaft clavicle fractures. METHODS: A retrospective cohort analysis was conducted on 110 patients with midshaft clavicle fractures treated between

Effect of closed reduction and external fixation in elderly patients with anterior shoulder dislocation combined with humeral surgical neck fracture: a retrospective cohort study based on fracture-dislocation classification.

BACKGROUND: Anterior shoulder dislocation combined with humeral surgical neck fracture is a severe injury that often requires emergency surgery. However, for some patients, such as those of advanced age or with surgical contraindications, closed reduction can be considered. This retrospective cohort study aimed to confirm the feasibility and clinical outcomes of the closed reduction technique with effective external fixation for the treatment of anterior shoulder dislocation combined with humeral surgical neck fracture in elderly patients. METHOD: From January 2010 to March 2024, a total of 5

The Bristow and Latarjet procedures: why these techniques should not be considered synonymous.

BACKGROUND: Recurrent shoulder instability is commonly associated with glenoid bone defects. Coracoid transfer procedures, such as the Bristow and Latarjet procedures, are frequently used to address these bone deficiencies. Despite the frequent synonymous labeling of these transfers as the "Bristow-Latarjet" procedure, their true equivalence has not been demonstrated. Therefore, our purpose was to compare the biomechanical effects of these two procedures. METHODS: Eight cadaveric specimens were tested on a custom shoulder simulator capable of loading nine muscle groups and of accurately orien

High-velocity gunshot-related humeral fractures managed with circular external fixation in a resource-limited setting, Samburu County, Kenya: Surgical technique highlights from a case series.

INTRODUCTION: High-velocity gunshot-related humeral fractures are complex injuries often associated with a high rate of nonunion, infection, and poor functional outcome. In low- and middle-income countries these injuries are further complicated by delayed presentation and implant unavailability. Staged management with spanning external fixation followed by internal fixation with intramedullary nail or plate has been well described in the literature; however, the use of a circular frame as a definitive treatment has been very seldom mentioned. MATERIALS AND METHODS: Six high-velocity gunshot-r

EVIDENCE-BASED REVIEWS & RECOMMENDATIONS FROM THE BRAZILIAN SOCIETY OF SHOULDER AND ELBOW SURGERY: EARLY VERSUS LATE REPAIR FOR TRAUMATIC ROTATOR CUFF TEARS.

INTRODUCTION: Traumatic rotator cuff tears are acute structural injuries to one or more rotator cuff tendons caused by an identifiable trauma, generally associated with the abrupt onset of pain, weakness, and shoulder dysfunction. They must be distinguished from chronic degenerative tears and from acute injuries superimposed on previously degenerated tendons. The optimal timing of surgical repair remains debated. METHODS: A systematic review was conducted in PubMed, Embase, Cochrane Library, and the Virtual Health Library (VHL) up to March 2026 to evaluate whether early repair is more effecti
