Cavernous sinus meningiomas are rare benign tumors arising from the dural wall of the cavernous sinus. These complex lesions are difficult to access surgically but can lead to cranial nerve palsies if left untreated. The case described in this video involved a 14-yr-old girl who presented initially with right ocular pain at 9 yr old. Strabismus of the right eye developed when she was 10 yr old; it was treated surgically without improvement. She developed ptosis and worsening strabismus at age 13 yr, at which time magnetic resonance imaging revealed a lesion in the anterior cavernous sinus and

INTRODUCTION: The growth and development of the craniofacial complex are under the influence of genetic and environmental factors, which determine its morphological and functional characteristics. Twin studies provide significant insight into how many genetic and environmental factors determine dental and craniofacial characteristics.
AIM: The aim of the study was to determine the genetic influence on craniofacial complex using a twin study model.
METHODS: The study sample comprised 52 pairs of twins who were referred to the Orthodontic Department, School of Dental medicine, University of Sa

Crouzon syndrome patients develop normal intracranial volume and potential restricted posterior cranial fossa volume with growth. This study aims to trace the segmental anterior, middle and posterior cranial fossae volume, and structural morphology in these patients, in order to help discern more focused and individualized surgical treatment plan. Ninety-two preoperative CT scans (Crouzon, n = 36; control, n = 56) were included, and divided into 5 age related subgroups. CT scans were measured using Mimics and 3-matics software. Overall, Crouzon syndrome patients grew to a 27% (p = 0.011) incre

OBJECTIVE: To correlate traditional Steiner's skeletal and dental measurements to similar measurements that use the eyes and natural head position as references instead of the cranial base.
SETTING AND SAMPLE: One hundred and fifty-two lateral cephalometric radiographs (66 female and 86 male) from the Harvard Forsyth twin sample were included in the study based on record availability.
MATERIAL AND METHODS: This was an observational study in which all cephalometric radiographs were traced and analysed using Steiner's cephalometric analysis. Each cephalogram was then altered to perform a simil

CONCLUSIONS: Epithelioid osteoblastoma of the skull base is exceedingly rare but should be included in the differential diagnoses of all extra axial tumors. Preoperative radiographic clues are limited, and final diagnosis relies solely on accurate pathologic examination. A diagnosis of epithelioid osteobl …

The course of the internal carotid artery (ICA) and its segment classifications were reviewed by means of a new and freely available 3D interactive model of the artery and the skull base, based on human neuroimages, that can be freely downloaded at the Public Repository of the University of Barcelona (http://diposit.ub.edu/dspace/handle/2445/112442) and runs under Acrobat Reader in Mac and Windows computers and Windows 10 tablets. The 3D-PDF allows zoom, rotation, selective visualization of structures, and a predefined sequence view. Illustrative images of the different classifications were ob…

BACKGROUND: Animal models of optic nerve injury are often used to study central nervous system (CNS) degeneration and regeneration, and targeting the optic nerve is a powerful approach for axon-protective or remyelination therapy. However, the experimental delivery of drugs or cells to the optic nerve is rarely performed because injections into this structure are difficult in small animals, especially in mice.
NEW METHOD: We investigated and developed methods to deliver drugs or cells to the mouse optic nerve through 3 different routes: a) intraorbital, b) through the optic foramen and c) tra

Background Surgical management of headache due to anomalies in the cervical spine is uncommon, as most cases improve with drugs and/or physical therapy. Case We report two instances of a very uncommon congenital upper cervical spine anomaly due to the presence of a pseudoarthrosis between a unilateral paracondylar process in the base of the skull and an epitransverse process arising from the transverse apophysis (PCP/ETA). The first one corresponds to a male on whom an endoscopic guided puncture was performed, and the second to an adult male from the Neolithic period who showed two cranial tre

BACKGROUND: The medial opticocarotid recess (MOCR) is located in the posterior wall of the sphenoid sinus, medial to the junction of the optic canal (OC) and the carotid prominence (CP). There is controversy in the literature in relation to the presence of the MOCR and its constancy, which is relevant when approaching the skull base through an endoscopic route.
METHODS: The morphometric relations of the MOCR with the surrounding structures were studied in 18 cadaveric specimens after endoscopic endonasal approach (EEA).
RESULTS: The distance between both MOCR was 11.06 ± 1.14 mm; the distanc…

AIM: Rotating burs (RB), routinely used in skull base and cerebellopontine angle (CPA) surgery for craniotomy and opening of the internal auditory canal (IAC) carry some risks for neurovascular tissue due to their rotating power. This paper describes the use of piezosurgery (PS), which selectively cuts bone with preservation of soft tissue, in lateral suboccipital craniectomy and opening of the IAC in the rat.
METHODS: A lateral suboccipital craniectomy and opening of the bony IAC were performed with the Mectron® piezosurgical device under microsurgical conditions in the anesthezised rat. The…

The facial nerve is the third most frequent location of intracranial schwannomas, with facial paresis the most common sign.(1,2) A 77-year-old woman presented with amnesia; the Mini-Mental State Examination score was 26, with normal cranial nerve function. A giant multicystic VII nerve schwannoma was identified in the left middle fossa, with components of the tumor in the temporal bone facial canal, geniculate ganglion, and internal auditory canal (figure). Compression of the hippocampus may have accounted for the clinical presentation. The middle fossa component of the tumor was resected for

Dysfunction of the hypoglossal nerve (nervus hypoglossus) occurs usually as part of a larger symptom complex, only rarely being the sole symptom of a neurologic disorder. Peripheral etiology must also be kept in mind, especially in patients with malignant primary disease. We describe a patient who developed an isolated right hypoglossal nerve palsy, which later became bilateral. In computerized tomography the bones of the cranial base of the patient with prostatic cancer were damaged by metastases especially in the vicinity of hypoglossal canals.

Esthesioneuroblastoma is an aggressive neuroectodermal tumor that originates from the olfactory mucosa and often recurs locally. Distant metastasis of esthesioneuroblastoma has been described, but there are few reports of drop metastasis to the spinal cord. Here, we report a case of multiple drop metastases to the cervical, thoracic, and lumbar regions of the spinal cord that occurred 18 years after resection and radiotherapy of the original anterior cranial fossa lesion. There was no evidence of local recurrence. The symptomatic lesion was treated with resection and adjuvant chemotherapy. The

OBJECTIVE: The objective of this study is to recognize the available endoscopic routes during approaches to the suprasellar region and the surgical endoscopic anatomy of the related neurovascular structures.
MATERIAL AND METHODS: Extended endoscopic endonasal approach to the suprasellar region (EEASR) through the planum sphenoidale was performed in five fresh adult cadavers. The anatomic characteristics of the suprasellar parachiasmatic cisterns were studied and documented following the resection of the planum sphenoidale and opening the dura to expose the anterior incisural space.
RESULTS:

OBJECTIVE: We used the diving technique to go beyond mere visualization of the surgical field and used it as an important step in removing the lesion itself, improving the optical field, and optimizing visualization with a dynamic fluid film lens. Likewise, having extended endoscopic endonasal surgery to the entire base of the skull and in particular to the sinus cavity, "diving surgery" has proven to be effective in visualizing and dissecting more extended tumors.
METHODS: We performed diving surgery in more than 350 surgical procedures to remove lesions in the sellar, sinus cavity, and cliv
