Sleep EEG oscillations in neurodevelopmental disorders without intellectual disabilities.

Neurodevelopmental disorders (NDDs) are often characterised by sleep problems, and recent evidence indicates alterations of the sleep electroencephalographic (EEG) oscillations in these patients. Sleep microstructure plays a crucial role in cognitive functioning and brain maturation. In this view, modifications in sleep EEG oscillations in NDDs could further impair the cognitive maturation process in these patients. We provide an overview of sleep microstructure alterations observed in three NDDs without intellectual disabilities (attention-deficit/hyperactivity disorder, high-functioning auti…

First-night effect on sleep time in dairy cows.

In human sleep studies, the probability of discomfort from the electrodes and the change in environment usually results in first-night recordings being discarded. Sleep recordings from the first night in human subjects often differ in amount of REM (rapid eye movement) sleep and the overall sleep architecture. This study investigated whether recordings of sleep states in dairy cows also show a first-night effect. Non-invasive electrophysiological recordings were carried out on nine cows of the Swedish Red breed during three consecutive 24-hour periods (recording days 1-3). Overall, cows spent …

Sleep-disordered breathing: Aids to diagnosis without a polysomnogram.

The South African population is suffering from an obesity epidemic. Sleep-disordered breathing (SDB), which includes obstructive sleep apnoea and obesity hypoventilation syndrome, is closely related to obesity. SDB may have serious health consequences if not asked about when taking a history related to sleep and sleep-deprivation symptoms. Unfortunately, a formal polysomnogram is available to very few patients who need the diagnosis confirmed. However, taking a sleep history, measuring the haemoglobin level and using a much smaller device in the comfort of a patient's bed can obviate the need …

Sleep-related hypermotor epilepsy: prevalence, impact and management strategies.

Sleep-related hypermotor epilepsy (SHE), previously called nocturnal frontal lobe epilepsy (NFLE), is a focal epilepsy characterized by asymmetric tonic/dystonic posturing and/or complex hyperkinetic seizures occurring mostly during sleep. SHE fulfills the definition of rare disease with an estimated minimum prevalence of 1.8/100,000 individuals, and it represents about 10% of drug-resistant surgical cases. Although SHE and autosomal-dominant SHE (ADSHE) have been considered benign epileptic conditions for a long time, emerging data have shed light on the severity of this disorder and some pec…

Preliminary Study of Palatal Implant for Sleep Apnea Control.

A fully-implantable device for treating obstructive sleep apnea (OSA) is conceptually suggested using soft palate stimulation. In this research, two in vivo studies were conducted to demonstrate electrical and physical feasibilities of the suggested device. First, electrical stimulation was delivered to the soft palate of a rabbit using a stimulator ASIC. The stimulation frequencies were swept from 20 Hz to 200 Hz to find out the appropriate parameter. Also, threshold level of the current pulse was evaluated to be 1.10 mA with an observance of a C-arm fluoroscopy. Second, a mock-up was fabrica…

Nightmares in United States Military Personnel With Sleep Disturbances.

STUDY OBJECTIVES: Sleep disturbances are common in United States military personnel. Despite their exposure to combat and trauma, little is known about nightmares in this population. The purpose of this study was to describe the prevalence and associated clinical and polysomnographic characteristics of nightmares in United States military personnel with sleep disturbances. METHODS: Retrospective review of 500 active duty United States military personnel who underwent a sleep medicine evaluation and polysomnography at our sleep center. The Pittsburgh Sleep Quality Index and the Pittsburgh Slee…

Association between sleep, childhood trauma and psychosis-like experiences.

Psychosis-like experiences (PLEs), or attenuated positive symptoms of psychosis, present along a severity continuum and have been associated with distressing thoughts and impairments in functioning. Although knowledge of the clinical importance of PLEs is expanding, risk factors for their expression are still poorly understood. Sleep disturbances are one known factor that exacerbate PLEs expression and distress, and trauma exposure is associated with occurrence of PLEs, as well as increased risk of later sleep difficulties. This study examined the joint influences of sleep and trauma on PLEs i…

A 67-Year-Old Man With Palpitations During Sleep.

A 67-year-old man with a history of atrial fibrillation (AF) presented to his physician with symptoms of episodic, nighttime palpitations and excessive daytime sleepiness. Four years prior he underwent radiofrequency ablation after a confirmed diagnosis of AF with subsequent resolution of his palpitations. His palpitations returned approximately 1 year following the ablation. These events would occur only at night and awake him from sleep. Holter monitoring showed baseline sinus rhythm with multiple episodes of AF with rates of 75 to 169 beats/min. These events were all nocturnal and correlate…

Post-Stroke Sleep-Disordered Breathing-Pathophysiology and Therapy Options.

Sleep-disordered breathing (SDB), encompassing both obstructive and central sleep apnea, is prevalent in at least 50% of stroke patients. Small studies have shown vast improvements in post-stroke functional recovery outcomes after the treatment of SDB by continuous positive airway pressure. However, compliance to this therapy is very poor in this complex patient group. There are alternative therapy options for SDB that may be more amenable for use in at least some post-stroke patients, including mandibular advancement, supine avoidance, and oxygen therapy. There are few studies, however, that …

Neuromuscular Dysfunction and Cortical Impairment in Sleep Apnea Syndrome.

PURPOSE: Lower muscle strength and endurance have been reported in severe obstructive sleep apnea (OSA). Increased intracortical inhibition previously reported at rest in OSA suggests central neuromuscular impairments in these patients. We hypothesized that (i) OSA patients demonstrate reduced knee extensor strength and endurance due to central impairments and (ii) continuous positive airway pressure (CPAP) treatment improves neuromuscular function in these patients. METHODS: Twelve OSA patients and 11 healthy controls performed intermittent knee extensions until task failure before and after…

Managing comorbid cardiovascular disease and sleep apnea with pharmacotherapy.

INTRODUCTION: Highly prevalent sleep disordered breathing (SDB) has been recognized as an independent cardiovascular disease (CVD) risk factor. Although these two entities often coexist, there is a shortage of sufficiently-powered studies testing the interplay between the course of sleep apnea and CVD pharmacotherapy. The mutual relationship between treated/untreated obstructive sleep apnea (OSA) with ongoing cardiovascular pharmacotherapies is an evident gap in clinical expertise. AREAS COVERED: In this article, the authors review the available evidence and outline future research directions…

Sleep Apnea and Cardiovascular Disease: An Enigmatic Risk Factor.

Synchronization of molecular, metabolic, and cardiovascular circadian oscillations is fundamental to human health. Sleep-disordered breathing, which disrupts such temporal congruence, elicits hemodynamic, autonomic, chemical, and inflammatory disturbances with acute and long-term consequences for heart, brain, and circulatory and metabolic function. Sleep apnea afflicts a substantial proportion of adult men and women but is more prevalent in those with established cardiovascular diseases and especially fluid-retaining states. Despite the experimental, epidemiological, observational, and interv…

Diagnosis and perioperative management in pediatric sleep-disordered breathing.

Sleep-disordered breathing has a prevalence of 12% in the pediatric population. It represents a spectrum of disorders encompassing abnormalities of the upper airway that lead to sleep disruption, including primary snoring, obstructive sleep apnea, central sleep apnea, and sleep-related hypoventilation. Sleep-disordered breathing is the most common indication for adenotonsillectomy, one of the most common procedures performed in children. In recent years, the American Academy of Otolaryngology-Head and Neck Surgery, American Academy of Pediatrics, and the American Society of Anesthesiologists h…
