Environmental neurology in the tropics.

We address the impact of the tropical environment on the human nervous system using the multifaceted approach characteristic of environmental neurology. First, environmental factors are examined according to their nature (physical, chemical and biological) and in relation to human activity and behavior. Some factors are specific to the tropics (climate and infections), while others are non-specific (chemicals, human communities and their way of life). Second, we examine the major role of human adaptation to the success of Homo sapiens, with emphasis on the linkage between thermoregulation and

Eveningness among late adolescent males predicts neural reactivity to reward and alcohol dependence 2 years later.

Eveningness, a preference for later sleep-wake timing, is linked to altered reward function, which may explain a consistent association with substance abuse. Notably, the extant literature rests largely on cross-sectional data, yet both eveningness and reward function show developmental changes. We examined whether circadian preference during late adolescence predicted the neural response to reward 2 years later. A sample of 93 males reported circadian preference and completed a monetary reward fMRI paradigm at ages 20 and 22. Primary analyses examined longitudinal paths from circadian prefere

Assessment of autonomic symptom scales in patients with neurodevelopmental diagnoses using electronic health record data.

BACKGROUND: Sleep disturbances, gastrointestinal problems, and atypical heart rate are commonly observed in patients with autism spectrum disorder (ASD) and may relate to underlying function of the autonomic nervous system (ANS). The overall objective of the current study was to quantitatively characterize features of ANS function using symptom scales and available electronic health record (EHR) data in a clinically and genetically characterized pediatric cohort. METHODS: We assessed features of ANS function via chart review of patient records adapted from items drawn from a clinical research

Pharmacologic modulation of orexin signaling: Impact on glucose-insulin homeostasis and potential mechanism of antidepressant action.

In addition to their canonical role in sleep-wake regulation, endogenous orexin signaling is implicated in the homeostatic control of reward and motivation, stress and arousal, and glucose-insulin metabolism. Recent research has focused on the development of orexin antagonists, notably seltorexant, as adjunctive treatments in adults with major depressive disorder (MDD). Converging evidence implicates insulin resistance in MDD pathophysiology, highlighting the need to integrate preclinical, pharmacologic, and translational evidence examining the effects of orexin signaling on glucose-insulin ho

A double-blind, randomized, placebo-controlled plus open trial of adjunctive suvorexant for treatment-resistant insomnia in patients with bipolar disorder.

BACKGROUND: Sleep pattern alteration is a core feature of bipolar disorder (BD), often challenging to treat and affecting clinical outcomes. Suvorexant, a hypnotic agent that decreases wakefulness, has shown promising results in treating primary insomnia. To date, data on its use in BD are lacking. This study evaluated the efficacy and tolerability of adjunctive suvorexant for treatment-resistant insomnia in BD patients. METHODS: Thirty-six BD outpatients (19 BDI, 69.4% female, 48.9 [±15.2] years) were randomized for 1 week to double-blind suvorexant (10-20 mg/day) versus placebo. Then, all s

Discrimination and insomnia: Examining depressive symptoms and nicotine withdrawal through a serial mediation model in a sample of smokers.

Insomnia is a common sleep disorder associated with poor health outcomes. Individuals from racially underrepresented groups as well as women tend to report more severe insomnia symptoms, and frequent experiences of discrimination have been found to drive such disparities. Smokers commonly experience sleep problems since nicotine can alter the sleep-wake cycle. Discrimination is associated with increased nicotine dependence, and such discrimination may also intensify tobacco withdrawal, specifically mood and cognitive-related aspects of withdrawal. The potential impact of discrimination on with

The association between sleep problems and perceived health status: a Japanese nationwide general population survey.

OBJECTIVE: Sleep problems in humans have been reported to impact seriously on daily function and to have a close association with well-being. To examine the effects of individual sleep problems on physical and mental health, we conducted a nationwide epidemiological survey and examined the associations between sleep problems and perceived health status. METHODS: Cross-sectional surveys with a face-to-face interview were conducted in August and September, 2009, as part of the Nihon University Sleep and Mental Health Epidemiology Project (NUSMEP). Data from 2559 people aged 20 years or older we

Week-ahead prediction of depressive episodes using wearable-derived circadian biomarkers: A multicenter deep learning study with risk-based operating thresholds.

Early detection of depressive episodes is important because shorter duration of untreated illness is associated with better outcomes, yet routine care remains largely reactive and dependent on retrospective self-report. Wearables enable passive, continuous monitoring, and circadian disruptions in sleep-wake, activity, and diurnal heart rate (HR) patterns may precede mood episodes. However, prior wearable-based models often relied on short prediction horizons, simplistic circadian proxies, and limited clinical interpretability. We evaluated a deep learning approach that integrates two complemen

Rest-activity rhythm profiles associated with manic-hypomanic and depressive symptoms.

Rest-activity rhythm (RAR) disturbances are associated with mood disorders. But there remains a need to identify the particular RAR profiles associated with psychiatric symptom dimensions. Establishing such profiles would support the development of tools that track the 24-h sleep-wake phenotypes signaling clinical heterogeneity. We used data-driven clustering to identify RAR profiles in 145 adults aged 36-82 years (mean = 60, standard deviation = 9). Then we evaluated psychiatric symptom dimensions (including positive and negative affect, depressive, manic-hypomanic, panic-agoraphobic, and sub

Oxidative metabolism in Rett syndrome: 1. Clinical studies.

The etiology of Rett syndrome (RS) remains a mystery. The clinical phenotype has similarities to that of patients with mitochondrial defects of oxidative metabolism. There is evidence of lactate and pyruvate elevations in blood and CSF in some patients. Over the last 10 years we have studied girls with RS looking for evidence of a defect in oxidative metabolism. We present data on lactate and pyruvate blood measurements in 30 patients with RS with repeated measurements performed over time in many. Taken as a whole the means of measurements of lactate and pyruvate fall within the control range,

Sleep in bipolar patients.

BACKGROUND: Sleep disturbance has been described in bipolar disorder (BPD). Specific complaints may include frequent nighttime awakenings, poor quality of sleep, reduction in total sleep time, and nightmares. Most patients with BPD also report insomnia when in depression, but a significant percentage of patients report hypersomnia symptoms with prolonged nighttime sleep, difficulty in wakening, and excessive daytime sleepiness. OBJECTIVES: The present study aims to investigate whether bipolar patients with sleep disorders presented impairment in quality of life, disability, and global functio

Inter- and Intrapersonal Associations Between Physiology and Mental Health: A Longitudinal Study Using Wearables and Mental Health Surveys.

BACKGROUND: More than 1 in 8 people potentially live with a mental health disorder, yet fewer than half receive treatment. Poor mental health awareness may contribute to this treatment gap, and digital health technologies, like wearables and their associated phone- and web-based applications, have the potential to reduce the mental health awareness gap due to their ease of adoption, objective feedback, and high rate of engagement. OBJECTIVE: This study aimed to better understand the relationships between mental health and objective wearable-derived metrics. METHODS: We examined the longitudi

Personality and Sleep Psychopathology: Associations Between the DSM-5 Maladaptive Trait Domains and Multiple Sleep Problems in an Adult Population.

Given the lack of sufficient studies exploring the nature of sleep problems from the perspective of the alternative model of personality disorders (AMPD) proposed by the DSM-5, the present study is aimed at determining the associations between five trait domains such as negative affectivity and sleep problems (insomnia, parasomnia, hypersomnia, circadian rhythm sleep disorder, restless legs syndrome, and sleep-disordered breathing) in an adult population. Adults aged 18-65 from western Iran were invited to the study via virtual platforms (N = 928; 62% female) and responded online to the Brief

Long-term psychopathology rates of children and adolescents and associated factors in the worst-sticken area of 2023 Kahramanmaras earthquake.

INTRODUCTION: Earthquakes are natural disasters that can have significant psychological impacts on individuals, particularly children. While physical injuries are often the immediate focus following an earthquake, the psychological effects can be long-lasting and profound. Understanding the psychiatric effects of traumatic experiences in children following earthquakes is crucial for developing effective interventions and support systems. METHOD: One hundred fifty three children and adolescents who experienced the earthquake were evaluated in the first year of the earthquake. Depression, anxie

Fresh Washed Microbiota Transplantation Alters Gut Microbiota Metabolites to Ameliorate Sleeping Disorder Symptom of Autistic Children.

Accumulating studies have raised concerns about gut dysbiosis associating autism spectrum disorder (ASD) and its related symptoms. However, the effect of gut microbiota modification on the Chinese ASD population and its underlying mechanism were still elusive. Herein, we enrolled 24 ASD children to perform the first course of fresh washed microbiota transplantation (WMT), 18 patients decided to participate the second course, 13 of which stayed to participate the third course, and there were 8 patients at the fourth course. Then we evaluated the effects of fresh WMT on these patients and their

An update of the impact of deep brain stimulation on non motor symptoms in Parkinson's disease.

Deep brain stimulation (DBS) of the subthalamic nucleus (STN) is a well-established therapy for patients with Parkinson's disease (PD), especially those with advanced motor complications. The effect of STN DBS on non motor symptoms (NMS) of PD is less well studied. In this article, we review the pertinent literature on the impact of STN DBS on NMS when they co-exist with disabling motor symptoms in PD patients. We also present evidence that the number and the severity of most NMS decrease after STN DBS which can have a major impact on patients' quality of life.

[Late-onset dysthymic states].

Sixty patients with dysthymic states which had emerged in later age of 60-80 years were examined. Two clinical types of dysthymic states were described: anergic (20 patients) and hypothymic (40 patients). Different comorbid mental disorders--obsessive-phobic (14 cases), somatoform (10), personality deviations (20) and psycho-organic (7)--were found to be characteristic of late-onset dysthymic states. According to developmental features, late dysthymia was primary (first manifested in the elderly) and secondary (develops after several depressive episodes). In diagnostic terms, the former is con

The range of indications for beta blocking substances has extended to various fields of pathology : vascular system, endocrinology... The discovery of some of their side effects give good reasons to try to find out whether they should have a place in psychopharmacotherapy and wheter they could meet its present requirements. The initial target symptoms were anxiety--which, being polymorphous, can be, at various degrees, reactionnal to obviously stressing surroundings, but which is also implied by structural distortions of personality--and psychosomatic manifestations.
