Increased Incidence of Upper Extremity Soft Tissue Injuries and Orthopaedic Surgeries in Patients with Eating Disorders.

BACKGROUND: Despite an established increased fracture risk in eating disorder patients, no studies, to our knowledge, have investigated the association between eating disorders and upper extremity soft tissue injury or surgery incidence. Given the association of eating disorders with nutritional deficiency and musculoskeletal sequelae, we hypothesized that patients with eating disorders would have an increased risk of soft tissue injury and surgery. The aim of this study was to elucidate this link and investigate if these incidences are increased in patients with eating disorders. METHODS: Co

A novel way of treating mallet finger injuries.

Standard treatment protocols following a mallet finger injury involve lengthy periods of immobilization in an effort to ensure the terminal extensor tendon is able to maintain the distal interphalangeal joint in extension. This author describes a technique that utilizes a combination of an orthosis and kinesiotape, thereby creating a treatment protocol that shortens the immobilization phase for these patients. - Victoria Priganc, PhD, OTR, CHT, CLT, Practice Forum Editor.

[Avulsion fracture of the tendon of origin of the long extensor muscle of the toe in a horse and a cat].

An avulsion fracture of the tendon of origin of the extensor digitorum longus muscle is reported in a horse and a cat. This avulsion fracture was due to injury in both animals. There was a striking similarity of radiographic findings in these animals, with involvement of the lateral trochlear ridge and lateral condyle of the femur. The horse was slaughtered because of a poor prognosis as a riding-horse. Fixation of the avulsed fragment with two Kirschner wires resulted in complete recovery in the cat.

Biomechanics of plate fixation following traditional olecranon osteotomy versus novel proximal ulna osteotomy for visualizing a distal humerus injury.

After a distal humeral injury, olecranon osteotomy (OO) is a traditional way to visualize the distal humerus for performing fracture fixation. In contrast, the current authors previously showed that novel proximal ulna osteotomy (PUO) allows better access to the distal humerus without ligamentous compromise. Therefore, this study biomechanically compared plating repair following OO versus PUO. The left or right ulna from eight matched pairs of human cadaveric elbows were randomly assigned to receive OO or PUO and repaired using pre-contoured titanium plates. Destructive and non-destructive mec

Dynadesis for Treatment of Dynamic Scaphoid Instability with 20-Year Results.

Background  Even though the scapholunate interosseous ligament is the most common wrist ligament injury, its treatment remains a challenge for hand surgeons. We report on a surgical treatment (Dynadesis) for dynamic scaphoid instability (DSI) with a 20-year follow-up period. Description of Technique  Dynadesis utilizes antagonist forearm muscles in order to synergistically provide dynamic stabilization to the scaphoid when the wrist is loaded. It is a tendon-to-tendon transfer with the following two components: 1) Dorsal-The extensor carpi radialis longus (ECRL) is passed through a hole in the

Chronic Quadriceps Tendon Rupture After Total Knee Arthroplasty Augmented With Synthetic Mesh.

Tear of the quadriceps tendon after revision or primary total knee arthroplasty is a rare complication, but when it occurs, this injury has serious functional consequences. In complete tears, the outcome of direct repair is unpredictable, and several authors recommend that the suture should be reinforced. Several techniques have been described, including the use of autografts, allografts, and synthetic mesh. The goal of this study was to assess the outcomes of a reconstruction technique augmented with synthetic mesh. A retrospective study was performed involving 3 patients who had chronic part

Single-incision chronic distal biceps tendon repair with tibialis anterior allograft.

PURPOSE: Several techniques for chronic distal biceps tendon repair have been reported; however, the literature is sparse. METHODS: Seven male patients who underwent chronic distal biceps tendon reconstruction were retrospectively evaluated. All patients had significant retraction necessitating the use of an allograft for reconstruction. The procedure was done through a single incision using suture anchors and a tibialis anterior allograft. In each case, the graft was first fixed to the radial tuberosity with suture anchors, and then the allograft was sutured to the remnant of the native bice

Effect of different loads on the shoulder in abduction postures: a finite element analysis.

Load can change the mechanical environment of dynamic and static stable structures of the shoulder joint, increase the risk of tissue damage and affect the stability of the shoulder joint, but its biomechanical mechanism is still unclear. Therefore, a finite element model of the shoulder joint was constructed to analyze the mechanical index changes of shoulder joint abduction under different loads. The stress of the articular side on the supraspinatus tendon was higher than that of the capsular side, with a maximum difference of 43% due to the increased load. For the deltoid muscle and glenohu

Knee sliced open by skate blade: complete patellar tendon rupture in an elite long track speed skater.

A long track speed skater sustained a deep horizontal cut to the right knee just distally to the patella, after he got hit by the skate blade of the pair mate. The injury included a complete patellar tendon rupture from the apex of the patella, a 1 mm deep transverse cut in the femoral condyle and a partial rupture of the anterior cruciate ligament. The tendon rupture was repaired with transosseous suture repair without augmentation. A knee brace was used for 8 weeks, with a gradual decrease in flexion restraints. A rehabilitation programme was overseen by a dedicated physiotherapist. At 6 mon

Zancolli Technique for Delayed Repair of Tibialis Anterior Tendon Rupture: A Case Report and Literature Review.

BACKGROUND: Several techniques for repair of tibialis anterior tendon ruptures (TATRs) are reported, although it is a rare lesion. We describe a case of TATR, discuss our treatment, and review the criteria for the choice of treatment. METHODS: In November 2015, a 61-year-old woman presented to our department 13 days after an injury to her left ankle, with avulsion of TAT. Because of the stump retraction, we performed a Zancolli-like plasty, anchoring the tendon to the navicular bone. Evidence about the timing and type of surgery has been reviewed. RESULTS: The patient was followed up for 16

Surgical technique for repair of acute proximal hamstring tears.

The surgical technique for repair of acute proximal hamstring tears is relatively recent. Installation of the patient is critical. The approach is either vertical under the gluteal fold in case of a complete tear with tendon retraction of more than 6 cm, or horizontal in the gluteal fold for retraction of less than 6 cm or for partial tears. After identification and neurolysis of the sciatic nerve, transosseous tendon reinsertion is performed with three or four metal or resorbable suture anchors. A rigid brace keeping the leg at 30° of flexion is worn for few days, then replaced by an articula

Biceps long head tendon revisited: a case report of split tendon arising from single origin.

A 27-year-old, right-hand-dominant woman with a posttraumatic anterior shoulder dislocation 3 months earlier after traffic accident presented because of pain and limited range of motion in the right shoulder. On physical examination, the patient had negative instability tests and a sulcus sign. On arthroscopic examination, a bifurcate long biceps tendon with two limbs was observed about 1 cm distal to the origin in the supraglenoid tubercle. We found no evidence of a tear in the long biceps tendon on probing, and the margin of each limb was smooth and round. Although this anatomic variant may

Trigger digits: principles, management, and complications.

Stenosing tenosynovitis, or trigger finger, is an entity seen commonly by hand surgeons. This problem generally is caused by a size mismatch between the flexor tendon and the first annular (A-1) pulley. Conservative management includes splinting, corticosteroid injection, and other adjuvant modalities. Surgical treatment consists of release of the A-1 pulley by open or percutaneous techniques. Complications are rare but include bowstringing, digital nerve injury, and continued triggering. Some patients require more extensive procedures to reduce the size of the flexor tendon. Comorbid conditio

Number and morphology of mechanoreceptors in the myotendinous junction of paralysed human muscle.

The mechanoreceptor system of the myotendinous junction (MTJ) of human palmaris longus muscle obtained at autopsy was studied histologically from six patients with flaccid paralysis (complete acute tetraplegia 4-6 weeks before the autopsy, due to a spinal cord injury), eight patients with spastic paralysis (chronic hemiplegia due to cerebral stroke) and ten neurologically normal controls. Four types of nerve endings, Ruffini and Pacini corpuscles, Golgi tendon organs, and free nerve endings, could be identified in the MTJs of the controls. In the MTJs of the patients with flaccid and spastic p

Evaluating the Ability of Hand Surgeons to Reliably Classify the Percentage of Tearing Based on MRI for Patients With a Partial Distal Biceps Tendon Tear.

Background: Patients with partial distal biceps tendon tears (PDBT) have both surgical and nonsurgical treatment options. The percentage of the tendon that has torn plays an important role in decision making, although this can be difficult to reliably measure, even with advanced imaging. The purpose of this study was to assess the reliability of hand surgeons within our practice in determining the percentage of the distal biceps tendon that has torn based on magnetic resonance imaging (MRI) in patients with PDBT. Methods: We retrospectively identified 50 patients at our institution who receive
