Fulminant Eosinophilic Myocarditis in a Young Adult: Successful Recovery After Extracorporeal Membrane Oxygenation (ECMO) Support and Multisystem Complications.

Eosinophilic myocarditis (EM) is a rare inflammatory cardiomyopathy with a highly variable clinical presentation, ranging from mild symptoms to fulminant cardiogenic shock. Its diagnosis is challenging due to non-specific clinical findings and requires a high index of suspicion, with endomyocardial biopsy (EMB) serving as the gold standard. Early initiation of immunosuppressive therapy and, in severe cases, mechanical circulatory support (MCS) are essential to improve outcomes. We report the case of a 21-year-old male who presented with progressive dyspnea, chest pain, and hypoxemia. Echocardi

[Implantable automatic defibrillator. Evaluation after 8 years of use].

The thirty nine patients (30 men and 9 women, mean age 49 +/- 14 years) who were implanted with an automatic cardioverter defibrillator (AICD) between October 1982 and April 1990 were reviewed retrospectively. This group included 22 patients with ischaemic cardiomyopathy (55%), 9 primary cardiomyopathies (23%) 5 ventricular tachycardias in patients with normal hearts (13%), 1 case of arrhythmogenic right ventricular dysplasia and two of mitral valve prolapse. The mean ejection fraction for the whole group was 38 +/- 14%. The indication for implantation of the AICD was life-threatening ventricu

A Rare Case of Multi-System Involvement and Hereditary Pulmonary Hypertension Caused by De Novo Heterozygous CAV1 Mutation in a Pediatric Patient.

Background: Pulmonary arterial hypertension is a rare but life-threatening condition in children, with hereditary forms often being linked to mutations in genes such as bone morphogenetic protein receptor type 2 (BMPR2), caveolin 1 (CAV1), and potassium channel subfamily K member 3 (KCNK3). Among these, CAV1 mutations are associated with severe disease phenotypes, though cases resulting from de novo heterozygous CAV1 mutations with multi-system involvement remain rarely reported. The CAV1 mutation (c.424C > T, p.Q142X) disrupts caveolin-1 function, leading to dysregulated pulmonary vascular re

RETRACTED: 8 F Catheter Percutaneous Mechanical Thrombectomy Plus Anticoagulation Versus Anticoagulation Alone in Intermediate-High Risk Acute Pulmonary Embolism: A Multicenter Real-World Study on Long-Term Complications, Right Heart Function, and Quality of Life.

Источник пометил публикацию как отозванную

BACKGROUND: Intermediate-high risk acute pulmonary embolism (PE) carries a significant long-term burden of post-thrombotic syndrome (PTS) and chronic thromboembolic pulmonary hypertension (CTEPH). Standard anticoagulation does not actively resolve obstructive thrombi, and systemic thrombolysis is limited by high bleeding risk. While percutaneous mechanical thrombectomy (PMT) offers rapid thrombus removal, robust multicenter evidence regarding its efficacy in preventing long-term complications is scarce, leading to its current class IIb recommendation in major international guidelines (2022 ESC

Pulmonary endarterectomy for chronic thromboembolic pulmonary hypertension after bilateral lung transplantation.

A 30-year-old woman suffering from diffuse interstitial lung disease underwent bilateral lung transplantation in 2008. A high-risk bilateral pulmonary embolism (PE) occurred in 2020 after deep venous thrombosis. After lysis (Endovascular System and oral anticoagulation), 1-year follow-up imaging showed a persisting thrombus-load (Fig. 1). Right heart catheterization (RHC) 18 months after PE showed s/m/dPAP of 51/34/25mmHg, PVR of 5.6WU and CO 4.43l/min. Echocardiography demonstrated right ventricular dilation with impaired radial and longitudinal function. She reported worsening dyspnea (NYHA

Association of Right Ventricular Myocardial Blood Flow With Pulmonary Pressures and Outcome in Cardiac Amyloidosis.

BACKGROUND: Cardiac amyloidosis (CA) is a restrictive and infiltrative cardiomyopathy, characterized by increased biventricular filling pressures and low output. Symptoms are predominantly of right heart origin. The role of right ventricular (RV) myocardial blood flow (MBF) in CA has not been studied. OBJECTIVES: This study aimed to first associate RV MBF measured by using positron emission tomography (PET) with reference standards of RV pressures and then to explore its prognostic value in CA. METHODS: Cardiac PET was performed at rest in 52 patients with CA and 9 healthy control subjects.

Use of calcium antagonists for cardiac arrhythmias.

Calcium antagonists have emerged as a new class of antiarrhythmic agents for the control of certain supraventricular and ventricular arrhythmias. Electrophysiologically, these agents are heterogeneous but their main action is mediated through a depressant effect on the slow calcium channel in cardiac muscle, most readily demonstrated in isolated tissue preparations. In vivo, their actions are modulated by their reflex actions and by their interaction with the autonomic nervous system due to the noncompetitive adrenergic-blocking actions that some of the compounds exhibit. The major agents exer

Association of left atrioventricular coupling index with mortality in heart failure with preserved ejection fraction.

BACKGROUND: The left atrioventricular coupling index (LACI) is a novel echocardiographic parameter reflecting atrioventricular interaction and cardiac remodelling. However, its association with mortality in heart failure with preserved ejection fraction (HFpEF) remains unclear. We investigated the relationship between LACI and all-cause mortality in patients with HFpEF. METHODS: This retrospective observational study included 193 patients with HFpEF. Clinical and echocardiographic data were analysed. LACI was calculated as the ratio of left atrial end-diastolic volume to left ventricular end-

Cardiogenic cerebral infarction after Takotsubo cardiomyopathy in a patient with catatonia: A case report.

BACKGROUND: Takotsubo cardiomyopathy (TTC) is a transient cardiac condition often triggered by an emotional or physical stress. TTC usually has a benign clinical course with full recovery. However, in rare cases, TTC is complicated by cardiogenic shock, left ventricular rupture, or ventricular thrombus. We report a case of a patient with catatonia who developed TTC and subsequently experienced extensive cerebral infarction. CASE PRESENTATION: A 71-year-old woman with no prior psychiatric history was admitted for catatonia following a suicide attempt. During hospitalization, she exhibited elec

Clockwise bundle branch re-entrant ventricular tachycardia in a teenage patient as the first manifestation of dilated cardiomyopathy associated with the p.Ile512Leu TNNI3k variant: a case report.

BACKGROUND: Bundle branch re-entrant ventricular tachycardia (BBRVT) typically occurs in patients with structural heart disease and conduction abnormalities. Certain genetic mutations may be responsible for conduction disorders leading to BBRVT, especially in young individuals without apparent structural heart disease. CASE SUMMARY: A 17-year-old male with no pathological history was admitted to our institution due to wide QRS complex tachycardia with right bundle branch block morphology and left superior axis. The patient showed baseline infrahisian conduction abnormalities associated. Altho

[Cardiovascular and hemodynamic changes after artificial pulmonary ventilation].

The cardio-respiratory interactions include mechanical, reflex and humoral mechanisms. However, in the organism they mutually overlap, thus mating their separate investigation is problematic. Mechanical effects of conventional artificial ventilation (AV) and high frequency ventilation (HFV) are elicited by increased intrapulmonary pressure during lung inflation, as well as during application of positive end expiratory pressure (PEEP). The increase in intrapulmonary pressure compresses the pulmonary vessels and increases the pulmonary vascular resistance. These changes deteriorate the right ven
