A brain-based endophenotype for major depressive disorder.

We have identified a brain-based endophenotype for major depressive disorder (MDD) that includes thinning of the cortex of the lateral aspect of the right hemisphere and the medial aspect of the left, as well as bilateral hypoplasia of frontal and parietal white matter. The endophenotype status of these abnormalities is supported by their presence in a multigenerational cohort of persons who themselves do not have MDD but who are at increased familial risk for developing the illness. Those who have the endophenotype but who are not ill nevertheless still suffer from inattention and poor visual…

The brain-gut axis in abdominal pain syndromes.

The importance of bidirectional brain-gut interactions in gastrointestinal (GI) illness is increasingly recognized, most prominently in the area of functional GI syndromes such as irritable bowel syndrome (IBS), functional dyspepsia, and functional chest pain. The brain receives a constant stream of interoceptive input from the GI tract, integrates this information with other interoceptive information from the body and with contextual information from the environment, and sends an integrated response back to various target cells within the GI tract. This system is optimized to assure homeostas…

[Deep brain stimulation: a review on current research].

Recently Deep brain stimulation (DBS) has found continuous use in treatment of neurological movement disorders. However DBS in psychiatric illnesses is less investigated. Its application in depression, obsessive-compulsive disorder, and therapy-resistant Tourette- Syndrome shows positive effects and offers an advanced alternative to neurosurgical therapies of the past. There are also case reports suggesting therapeutic benefits in schizophrenia and addiction. To a large extent, the mechanisms of action appear to be still unknown; the side effects seem partially modulated through the stimulatio…

Brain abscess caused by Cladophialophora bantiana in China.

A case of a 38-year-old male farmer with a brain abscess caused by Cladophialophora bantiana is described. He had a 2 year history of non-insulin-dependent diabetes and myelodysplastic syndrome. A cranial computed tomography scan demonstrated a hypodense ring lesion with peripheral oedema and a midline shift in the left frontal lobe. A darkly pigmented mould was isolated from the brain abscess. The isolate was identified as C. bantiana based on its morphological features and DNA sequence analysis. The patient was unresponsive to burr hole aspiration and irrigation, as well as liposomal amphote…

Hippocampus: a "forgotten" border zone of brain ischemia.

BACKGROUND: Hippocampus is selectively susceptible to ischemic damage and it could be, in some conditions, considered as a border zone of brain ischemia. RESULTS: We illustrate this concept with three cases of hippocampal diffusion-weighted imaging (DWI) restrictions secondary to a carotid artery dissection with an ipsilateral fetal posterior cerebral artery (PCA) in two cases and to global brain hypoperfusion consequent to a cardiac arrest in the last case. CONCLUSION: The hypoperfusion induced by a cardiac arrest or an internal carotid dissection with an incomplete circle of Willis promote…

Diploic epidermoid cyst associated with focal brain pseudohypertrophy.

We present an 82-year-old woman, admitted for generalized status epilepticus, showing an osteolytic lesion of the right parietal bone associated with underlying focal brain pseudohypertrophy. A radiological diagnosis of diploic epidermoid cyst based on MRI characteristics was made. The aspect of focal brain pseudohypertrophy was probably caused by direct involvement of the underlying meningeal structures preventing the brain parenchyma locally from being retracted (i.e., retraction due to age-related brain atrophy). This aspect of focal brain pseudohypertrophy can potentially be seen in all ch…

Mild traumatic brain injury in the occupational setting.

The evaluation and management of mild traumatic brain injury (mTBI) in the occupational setting may pose significant challenges for even the most-seasoned practitioner. Providers must simultaneously address the clinical management of mTBI and be familiar with the systematic and administrative requirements related to the management of injured workers with mTBI who are covered by workers' compensation insurance, including causation, return to work, and the potential of permanent impairment. Given the primarily subjective nature of many mTBI symptoms, an injured worker with a delayed recovery may…

Distress in patients with newly diagnosed brain tumours.

OBJECTIVE: Patients with intracranial tumours often suffer from clinically relevant psychological distress. However, levels of distress and contributing factors have not been systematically evaluated for the early course of the disease. Using the National Comprehensive Cancer Network's Distress Thermometer (DT), we evaluated the extent and sources of distress within a population of patients with intracranial neoplasms. METHODS: One hundred and fifty-nine patients were included who underwent craniotomy for newly diagnosed intracranial tumours at our department. All patients completed the DT qu

[Brain imaging in autism spectrum disorders. A review].

In the past two decades, an increasing number of functional and structural brain imaging studies has provided insights into the neurobiological basis of autism spectrum disorders (ASD). This article summarizes pertinent functional brain imaging studies addressing the neuronal underpinnings of ASD symptomatology (impairments in social interaction and communication, repetitive and restrictive behavior) and associated neuropsychological deficits (theory of mind, executive functions, central coherence), complemented by relevant structural imaging findings. The results of these studies show that al…

Anomia training and brain stimulation in chronic aphasia.

Recent studies have reported enhanced performance on language tasks induced by non-invasive brain stimulation, i.e., repetitive transcranial magnetic stimulation (rTMS), or transcranial direct current stimulation (tDCS), in patients with aphasia due to stroke or Alzheimer's disease (AD). The first part of this article reviews brain stimulation studies related to language recovery in aphasic patients. The second part reports results from a pilot study with three chronic stroke patients who had non-fluent aphasia, where real or placebo rTMS was immediately followed by 25 minutes of individualise…

[Intensive care of traumatic brain injury in children].

Traumatic brain injury among other injuries of human body reaches up to 30-50% and, according to the WHO, it grows by 2%. Severe traumatic brain injury (such as severe brain contusion, epidural, subdural and intracerebral hematoma, intracerebral hygroma, diffuse axonal injury) in the structure of general trauma amounts 4-20%. The prognosis of traumatic brain injury mainly depends on the timeliness of the first aid. The therapeutic measures usually begin at the place of the accident or in the ambulance vehicle (hence the clear role of the specialist team). It is advised for children with severe…

Atypical brain oscillations: a biological basis for dyslexia?

Examining rhythms in the brain reveals a biological basis for dyslexia. A new study provides evidence of atypical oscillatory patterns and hemispheric specialization in dyslexic adults. These patterns inform phonological processing and verbal memory problems, known to be core deficits in dyslexia.

Increased risk of brain injury in IVF babies.

AIM: The aim of this paper was to assess brain injury occurrence among in vitro fertilization (IVF) babies. METHODS: We examined all babies born in our hospital in the triennium 2004-2006, comparing the presence of brain injuries between IVF babies and the rest of the population. RESULTS: In IVF group (180 babies), brain injury was present in 4 babies, while in the rest of population (n=3602) it was present in 23 babies (P=0.042, RR: 3.18). IVF babies have a higher risk of being born with a birthweight less than 2 500 grams (P<0.0001; RR: 5.133). When we considered only babies born with a bi…

John Nash, game theory, and the schizophrenic brain.

This article focuses on John Nash, recipient of the Nobel Prize in Economics in 1994, and subject of the Award winning 2001 film A Beautiful Mind, who was diagnosed with paranoid schizophrenia in 1958 at the age of 29. After presenting an account of the emergence, course, and eventual remission of his illness, the article argues for the relevance of his contribution to game theory, known as the Nash equilibrium, for which he received the Nobel Prize, to research studies of the schizophrenic brain and how it deviates from the normal brain. The case is made that the Nash equilibrium is descripti…

Lateral medullary syndrome due to brain stem tuberculoma.

Brainstem tuberculomas are quite unusual, accounting for less than 5 % of all intracranial tuberculoma. Intracranial tuberculoma is rare form of central nervous system tuberculosis. Lateral medullary syndrome is a combination of clinical features commonly attributed to infarction in territory of posterior inferior cerebellar artery or vertebral artery. We report a case suffering from lateral medullary syndrome resulting from brainstem tuberculoma which has not been reported so far in the literature. This patient was started on antituberculous chemotherapy and repeat magnetic resonance imaging …

Intraoperative brain shift compensation: accounting for dural septa.

Biomechanical models that describe soft tissue deformation provide a relatively inexpensive way to correct registration errors in image-guided neurosurgical systems caused by nonrigid brain shift. Quantifying the factors that cause this deformation to sufficient precision is a challenging task. To circumvent this difficulty, atlas-based methods have been developed recently that allow for uncertainty, yet still capture the first-order effects associated with deformation. The inverse solution is driven by sparse intraoperative surface measurements, which could bias the reconstruction and affect …
