Treatment consideration for US military members with skin disease.

Recent changes to military medicine precipitated by the National Defense Authorization Act for Fiscal Year 2017 are expected to result in civilian specialists playing a larger role in the care of our military population. Medical readiness and deployment eligibility should be taken into consideration when establishing a treatment plan for service members. This article highlights unique factors civilian dermatologists must consider when treating active-duty military patients with acne, atopic dermatitis (AD), psoriasis, dissecting cellulitis of the scalp (DCS), and lupus erythematosus (LE).

[Superantigens in human disease].

Superantigens include viral and bacterial products, mainly of streptococci, staphylococci that stimulate T cells to proliferate nonspecifically through interaction with class II major histocompatibility complex products on antigen-presenting cells and then with variable regions on the beta chain of T cell receptor complex. Superantigens cause symptoms via release of immune cytokines. These proteins should be considered potential cause of illnesses such as rheumatic fever, arthritis. Kawasaki syndrome, atopic dermatitis, and guttate psoriasis because of their potent immune system-altering capac

Editor's Highlights-March 2025.

Managing chronic inflammatory skin diseases such as hidradenitis suppurativa (HS) and psoriasis extends beyond clinical measures; it should also consider patient-reported outcomes, treatment burden, and overall satisfaction. Successful therapy in both conditions is influenced by factors such as residual disease activity, the psychological profiles of patients, and the availability of new treatment options. The coexistence of psoriasis and atopic dermatitis can pose significant diagnostic and therapeutic challenges. In such rare cases, Janus kinase inhibitors may be considered a treatment optio

Paradoxical Psoriasis.

Cytokine blocking therapies have revolutionized the management of psoriasis and atopic dermatitis but can lead to the development of paradoxic psoriasis (PP). Patients treated with biologics should be closely monitored for the development of PP and other paradoxical eruptions (including inflammatory joint disease, inflammatory bowel disease, eczematous eruptions, lupus like eruptions, sarcoidal eruptions, and others) and occasionally the development of cutaneous T-cell lymphoma. Further understanding the immunologic mechanism of these processes will ultimately drive our understanding of and ab

A Review of the Role and Treatment of Biofilms in Skin Disorders.

A biofilm is a diverse community of microorganisms enclosed in an extracellular matrix. Although this organization of cells exists naturally in healthy skin, it is also involved in the pathogenesis of multiple skin disorders, such as acne and atopic dermatitis. Because biofilms provide microorganisms with a survival advantage and increased resistance to traditional antibiotics, they can be very difficult to treat, particularly when the goal is to also preserve the natural skin microbiota. This review aims to provide an overview of the role of biofilms in various dermatological diseases, as wel

Mycosis Fungoides: Awareness in Skin of Color.

Over the past decade, what is known about skin of color in the field of dermatology has gained an increasing amount of attention among both clinicians and the general public. Recent efforts have continued to shed light on the role of skin color in health disparities and the development of actionable strategies to improve outcomes for patients of color. While research has largely focused on conditions such as atopic dermatitis, psoriasis, and skin cancer, uncommon dermatologic conditions have been less emphasized in skin of color discussions thus far.

Resolution of Treatment-Refractory Prurigo Nodularis With Dupilumab: A Case Series.

Prurigo nodularis is a pruritic skin condition that can present therapeutic challenges. We present a series of three patients diagnosed with prurigo nodularis who had failed several commonly trialed therapies, but experienced relief from symptoms and improvement in skin lesions following initiation of dupilumab therapy. All patients in this series lacked a diagnosis of atopic dermatitis and had lesions on the lower extremities, although other locations such as the trunk were also involved. Continued study of dupilumab in patients with prurigo nodularis is advocated.

Dupilumab Treatment for Prurigo Nodularis and Pruritis.

Prurigo nodularis (PN) is a disease in which chronic scratching and picking of the skin due to intense pruritis results in papulonodules, notably in areas that are accessible to the patient. The pathophysiology is hypothesized to be mediated by a Th2 helper cell response, similar to that seen in atopic dermatitis, therefore, treatment of PN with dupilumab would be expected to elicit a therapeutic response. We demonstrated that treatment of PN with dupilumab significantly decreased pruritis and the size and number of new lesions after 2 months of treatment. J Drugs Dermatol. 2019;18(9):940-942.

Immunomodulatory effects of tacrolimus (FK506) for the treatment of allergic diseases.

Tacrolimus has been used to prevent allograft rejection and also used in kidney, liver and heart transplantations. Various preclinical and clinical studies demonstrated that Tacrolimus possess immunomodulatory and anti-inflammatory properties. The mechanism of action of Tacrolimus in allergic diseases involves calcineurin inhibition, and downregulation of T-cell reactivity, IgE degranulation, and its actions on mast cells, dendritic cells, basophils, eosinophils and inhibition of transcription of proinflammatory cytokines. Herein we reviewed the Pharmacotherapeutic mechanism of action of Tacro

Disseminated herpes simplex virus: a case of eczema herpeticum causing viral encephalitis.

Eczema herpeticum is a dermatological emergency causing a mortality of up to 10% if untreated. It frequently presents in a localised form and rarely disseminates via haematogenous spread with pulmonary, hepatic, ocular and neurological manifestations. Although it commonly appears on a background of atopic dermatitis, many other dermatological conditions have been described preceding this disease. Eczema herpeticum can be easily mistaken for folliculitis and is often treated accordingly with antibacterial drugs; therefore patients will often deteriorate before a diagnosis of eczema herpeticum h

Scalp hyperkeratosis in children with skin of color: diagnostic and therapeutic considerations.

Scalp hyperkeratosis is common in childhood and adolescence. Diagnosis is affected by age, race, and history of infectious exposure, and associated symptoms including atopic features, alopecia, inflammatory nodules, presence and type of cutaneous lesions outside of the scalp, and nuchal lymphadenopathy. Tinea capitis is common in children with skin of color, especially black and Hispanic children. In adolescents, seborrheic dermatitis predominates as the cause of scalp hyperkeratosis, but tinea is still of concern. This article aims to help the practitioner comfortably diagnose and treat scalp

Alternative therapies for common dermatologic disorders, part 2.

The exact pathophysiology and curative treatment of many common dermatologic conditions remains unclear. Often, conventional treatments are only partially effective, leading patients to look for alternative therapy. No new promising alternative treatments for aphthous stomatitis could be found in the literature. Acne may be helped by nicotinamide gel and azelaic acid. Atopic dermatitis may be helped by a trial of elimination diet, n-3 fatty acid supplementation, or Chinese 10-herb tea. Psoriasis may be helped by aloe vera or thermal baths combined with sun exposure.

What drives the inflammatory response in rhinosinusitis.

The dilemma of treating chronic sinusitis continues to confuse clinicians. When mucosal thickening does not respond to aggressive antibiotic therapy, other etiologies should be considered. Perhaps the most likely is the inflammatory response. In comparison with asthma, chronic sinusitis exhibits amazing similarities. Atopic dermatitis also exhibits similar inflammatory responses that lead to thickening of the skin in a manner not dissimilar to mucosal thickening. The interactions amid eosinophils, lymphocytes, leukotrienes, interleukins, and epithelial cells serve as a reminder that there is o

Validation of a screening instrument to identify persons for exclusion from smallpox vaccination.

Adults must be screened for atopic dermatitis and other contraindications before smallpox vaccination. We validated the sensitivity of a self-administered Centers for Disease Control and Prevention (Atlanta, GA) screening questionnaire completed by 174 hospital workers, with the workers' medical records as the reference standard. The questionnaire failed to identify one-third of the subjects who had a contraindication, although the incidence of serious adverse events has been low among vaccinees. Further assessment of screening procedures is needed if the administration of smallpox vaccine bec

Ciliated cyst of the vulva.

Ciliated cyst of the vulva is a rare anomaly. The cyst is lined with columnar ciliated epithelium that resembles müllerian epithelium and is located on the superior portion of the labium minus, occasionally associated with pregnancy or exogenous progesterone. We describe a case of an 11-year-old Japanese girl with a ciliated cyst of the vulva and a history of atopic dermatitis. Epithelial cells in our case showed negative staining for estrogen receptor. The origin of the ciliated epithelium is still unknown. However, chronic inflammation of the vulvar vestibulae may contribute to the pathogene

The impact of skin disease on the quality of life of adolescents.

Though relatively well-investigated in adults, the impact of skin disease on the health-related quality of life (HRQL) of adolescents has only recently begun to be systematically evaluated. The primary goal of this article is to critically review previous research that has examined the relationship between HRQL and three types of skin disease commonly experienced by adolescents, namely acne vulgaris, alopecia areata, and atopic dermatitis. A secondary goal is to provide strategies to adolescent medicine physicians that will enable them to more effectively manage the psychological components of

[Allergy investigations in eczema].

Investigation of allergy is of major importance in the diagnosis of eczema. It is mainly based on patient history: personal and familial atopic antecedents, possible contact with allergens at the work-site, in the household or during leisure activities, or in topical medications, cosmetics or clothing. Among complementary examinations, detection of eosinophilia or elevated IgE has no practical application, but sometimes the presence of specific IgE can be useful. In fact, the most important examinations are skin tests, especially patch tests, which are indispensable in contact dermatitis, and

[Eczema and hyper-IgE. Effect of cephalosporins].

Atopic dermatitis is a disease in which staphylococcal cutaneous superinfection and immune disorders, especially hyper IgE, are closely related. Cephalosporins are very effective on exacerbations of superinfections; among them, Cefadroxil diminishes immunoglobulins E in vivo. In vitro, this antibiotic promotes from the macrophage the PgE2 synthesis to which the T suppression is classically sensitive. In DNP-OVA sensitized rats which produce anti DNP IgE, the secondary response is inhibited by Cefadroxil. From clinical to research experiments, the hypothesis is the effect of cephalosporins, and

Stretch garment dermatitis.

A disease of the skin, not hitherto described, is caused by pressure or tension on the skin from the wearing of tight-fitting stretch garments such as "stretch bras," "stretch girdles" and "stretch socks." The condition is not due to chemical sensitization of fabrics, dyes or other additives but is of mechanical origin. The eruption may assume various clinical forms and may be characterized by a nondescript erythematous and eczematous appearance or may consist of an exaggeration, in the areas covered by the stretch garment, of already existing dermatosis such as lichen planus, psoriasis, acne

Nickel Contact Allergy in an Australian Patch Testing Centre Over 2014-2023.

BACKGROUND: Nickel is the most common cause of allergic contact dermatitis (ACD). In certain geographical locations, the prevalence of nickel dermatitis (NiACD) has changed over time; however, there is little data on the prevalence of NiACD in Australia. OBJECTIVES: To investigate the prevalence and trends of both nickel contact allergy and NiACD in Australia. METHODS: A retrospective audit was performed of 3582 patients patch tested to 5% nickel sulphate between 2014 and 2023. Relevance was assessed by consultant dermatologists. Patch test data was analysed using Pearson's χ2 test and Cochr
