Etiology of diverticular disease with classic illustrations.

Diverticulosis is a common colonic disorder and often is found incidentally on colonic endoscopy and contrast enema radiographs. Theories relating to the etiology of the pathologic processes are commonly quoted, although the actual anatomic features are rarely seen during colonoscopic examinations. Here we show classic illustrations that support the widely held theories regarding the etiologies of diverticular diseases.

Consistency enforcement in medical knowledge base construction.

Some aspects of knowledge base creation can be partially or completely automated, resulting in higher quality and smaller effort. Computer assistance is particularly valuable in ensuring the internal consistency of a knowledge base. The article describes several techniques for consistency enforcement in QMR-KAT, an interactive knowledge base editor for the INTERNIST-I/QMR medical knowledge base. Two strategies that improve consistency are applicable to a wide range of situations. The first strategy prevents simple (but common) inconsistencies. The second strategy reveals facts that are potenti

Diverticulosis of the vermiform appendix.

Vermiform appendix is an unusual site of diverticulosis. We present 3 examples of this entity and briefly review its pathogenesis and clinical significance. As a general rule, appendiceal diverticula are multiple, acquired naturally, and rarely associated with complications.

Handedness and the side on which pharyngeal pouches occur.

Pharyngeal pouches present far more commonly on the left side of the neck than the right. Sixty one patients with a history of pharyngeal pouch were questioned about their handedness and about whether they had had symptoms or signs predominantly on one side of the neck before receiving treatment. There was a highly significant association between handedness and pharyngeal pouches on the opposite side of the neck. It is suggested that this is the reason for the rarity of right sided pouches.

Malabsorption in jejunal diverticulosis treated with resection of the diverticula.

Patients with jejunal diverticula usually are asymptomatic unless bacterial overgrowth within the diverticula is sufficient to cause vitamin B12 deficiency, by direct uptake of the vitamin by the bacteria, or malabsorption resulting from bacterial deconjugation of bile salts and impaired lipid digestion. The administration of broad-spectrum antibiotics usually constitutes effective treatment that suppresses bacterial flora, with surgery reserved for complications such as hemorrhage, perforation, and abscess formation, and acute or chronic intestinal obstruction. Our patient had many diverticul
