Surgical lead extraction for total occlusion of the superior vena cava by chronic lead infection after mustard procedure.

We present a case of total occlusion of the superior vena cava (SVC) with extensive thrombosis of the adjacent large veins secondary to multiple abandoned pacemaker leads with a superimposed chronic lead infection by Corynebacterium jeikeium. A surgical lead extraction was performed with an extensive en-bloc resection of the SVC together with the right subclavian vein and the right innomate vein. No venous reconstruction was required because of an unobstructed runoff via a well-developed azygos system.

Vascular endothelial growth factor B prevents the shift in the ocular dominance distribution of visual cortical neurons in monocularly deprived rats.

A key model for examining the activity-dependent development of primary visual cortex (V1) involves the imbalance in activity between the two eyes induced by monocular deprivation (MD). MD early in life causes dramatic changes in the functional and structural organization of mammalian visual cortex. The molecular signals that mediate the effects of MD on the development of visual cortex are not well defined. Neurotrophic factors are important in regulating the plasticity of visual cortex, but the choice of an appropriate growth factor as well as its delivery has proven difficult. Although vasc

Low-frequency stimulation depresses the primate anterior-cingulate-cortex and prevents spontaneous recovery of aversive memories.

Functional abnormalities in the dorsal-anterior-cingulate-cortex (dACC) underlie anxiety disorders and specifically post-traumatic stress disorder (PTSD). Promising and common behavioral approaches have limited effectiveness and many subjects exhibit spontaneous recovery of fear, as also evident in animal models following extinction training. Here, we use low-frequency stimulation (LFS), a protocol shown to induce long-term depression, with the aim of affecting synaptic plasticity induced by fear acquisition and extinction. We use aversive conditioning of either tone or visual stimuli paired w

Validating optimal function of the closed loop stimulation sensor with high right septal ventricular electrode placement in 'ablate and pace' patients.

PURPOSE: The study aim was to validate the closed loop stimulation (CLS) vs. accelerometer (ACC) rate-responsive sensors with electrodes placed in the right ventricular high septal (RVHS) or right ventricular apical (RVA) lead positions in patients following 'ablate and pace' therapy for persistent atrial fibrillation. METHODS: 'Ablate and pace' patients were randomised to either RVHS or RVA electrode placement with a dual sensor device. A double-blind crossover study comparing CLS vs. ACC rate-response pacing modes was undertaken. Subjects undertook cardiopulmonary testing with constant work

[Indications and results of sacral nerve stimulation in faecal incontinence].

BACKGROUND: Sacral nerve stimulation (SNS) is an effective and less invasive treatment of faecal incontinence (FI). Patient selection has evolved from strict criteria to a more liberal approach, since temporary testing reliably predicts the efficacy of permanent stimulation in FI of various aetiologies. PATIENTS AND METHODS: From November 2005 until June 2007, we evaluated 20 consecutive patients (17 females, 3 males) with FI by percutaneous nerve evaluation (PNE), i. e., temporary stimulation. 13 patients proceeded to a permanent implantation of a pulse generator (3 bilateral generators). 11

Correlations between the same motor cortex cells and arm muscles during a trained task, free behavior, and natural sleep in the macaque monkey.

Traditionally, the neural control of movement has been studied by recording cell activity in restrained animals performing repetitive, highly trained tasks within a restricted workspace. However, the degree to which results obtained under these conditions are valid during natural, unconstrained behavior remains unknown. Using an autonomous, implantable recording system, we examined the relationships between the firing of motor cortex cells and forearm muscle activity in primates under three behavioral conditions: performance of a conventional torque-tracking task, unrestrained behavior, and na

Prolonged sacral neuromodulation testing using permanent leads: a more reliable patient selection method?

OBJECTIVE: To assess the effect of prolonged sacral neuromodulation testing using permanent leads comparing the usual evaluation period of 4 to 7 days to a prolonged evaluation period of a minimum of 14 days. PATIENTS AND METHODS: A consecutive series of 20 patients (16 females and 4 males) undergoing prolonged sacral neuromodulation testing using permanent leads between September 2000 and March 2004 were evaluated retrospectively. 10 suffered from urgency-frequency syndrome, 3 from urge incontinence and 7 from non-obstructive chronic urinary retention. Electrochemotherapy: results of cancer treatment using enhanced delivery of bleomycin by electroporation.

Over the last decade a new cancer treatment modality, electrochemotherapy, has emerged. By using short, intense electric pulses that surpass the capacitance of the cell membrane, permeabilization can occur (electroporation). Thus, molecules that are otherwise non-permeant can gain direct access to the cytosol of cells in the treated area.A highly toxic molecule that does not usually pass the membrane barrier is the hydrophilic drug bleomycin. Once inside the cell, bleomycin acts as an enzyme creating single- and double-strand DMA-breaks. The cytotoxicity of bleomycin can be augmented several 1

Evaluation of methods for eliciting somatosensory-evoked potentials in the awake, freely moving rat.

To standardise the method of eliciting somatosensory-evoked potentials (SEPs), SEPs were generated by electrical stimulation of different stimulus sites and recorded bilaterally from the primary somatosensory cortex (S1) and from midline in awake, freely moving rats. Increasing stimulus intensity enhanced amplitudes of all SEPs. At supramaximal stimulation, SEPs following vibrissae and tail stimulation (V-SEP and Ta-SEP, respectively) but not following trunk stimulation (Tr-SEP), fulfilled our criterion of signal-to-noise ratio >or=4. The first V-SEP component coincided with a stimulus artefac

Altered discharge pattern of basal ganglia output neurons in an animal model of idiopathic dystonia.

A decreased activity of basal ganglia output neurons is thought to underlie idiopathic dystonias and other hyperkinetic movement disorders. We found recently a reduced spontaneous discharge rate of entopeduncular neurons (internal globus pallidus in primates) in dt(sz) hamsters, an unique model for idiopathic paroxysmal dystonia in which stress-inducible attacks show an age-dependent severity. Otherwise, it has been suggested that an altered discharge pattern may be more important for the occurrence of dystonia than a reduced discharge rate. Based on qualitative and computerized quantitative e

Long-term electromyogram recording from the posterior cricoarytenoid muscle as a potential biological trigger for phrenic pacing: results of an animal study.

BACKGROUND: On chronic exposure, Lead (Pb) deposits in the skeletal system, replaces calcium ions, and alters the normal physiological processes, which in turn, lead to stunting, delayed fracture healing, and high resorption of collagen molecules. The present study aimed to assess the association of musculoskeletal pain and discomfort with bone turnover markers (BTMs) among long-term Pb-exposed workers. STUDY DESIGN: A cross-sectional study. METHODS: The study recruited 176 male Pb-exposed workers and 80 control subjects who were matched for age, gender, and socio-economic status. Blood lead

Midbrain tremor and hypertrophic olivary degeneration after pontine hemorrhage.

A severe rest tremor arose in a patient's right arm 9 months after a pontine tegmental hemorrhage. Magnetic resonance studies at 4 and 10 months showed residual hemosiderin in the pons and increasing hypertrophic olivary degeneration (HOD) affecting primarily the left olive. The tremor was refractory to pharmacotherapy (clonazepam, propranolol, and levodopa), but was reduced after implantation of a thalamic stimulator device. Although pontine hemorrhage is among several common causes of HOD, it has not previously been appreciated as a cause of midbrain ("rubral") tremor. A disynaptic dentatoru

Autonomic nervous system controlled closed loop cardiac pacing.

A multicenter clinical study is presented, which focuses on the reestablishment of closed loop cardiac control in patients with chronotropic insufficiency. Using the information about sympathetic tone contained in the myocardial contractility, it is possible to reconnect the heart rate to the physiological control mechanisms. Intracardiac impedance is measured with the ventricular electrode and the ventricular inotropic parameter (VIP) is derived from that. The VIP serves directly as input to the control of heart rate by the pacemaker. Over 200 patients have received autonomic nervous system (

[State of the heart conduction system in the sick sinus syndrome (according to the recording of the bundle of His potentials and the atrial stimulation test)].

It was established that the presence of Samoilov-Wenckebach's periods and prolongation of the H--V interval at low frequency of the imposed rhythm as well as periods of asystolia of more than 1,200 msec are most typical for these patients. An attempt was made to determine the type of cardiac stimulation indicated for such patients from the character of the disorders of rhythm observed in them and the initial frequency of their own cardiac contractions.

Carotid sinus nerve stimulation in the management of intractable angina pectoris: four-year follow-up.

Since February 1969 carotid sinus nerve stimulators have been implanted in 13 patients with intractable, incapacitating angina pectoris, unrelieved by medical management and, in some cases, revascularization procedures. Four patients died, one on the third postoperative day, the others at 15, 31 and 49 months postoperatively. Two other patients sustained myocardial infarcts, at two weeks and two months postoperatively. Complications were few and transient. The condition of two patients is now deteriorating. In all cases there was relief of pain and a decrease in blood pressure and heart rate. E

Intensity-dependent microcurrent stimulation enhances BMSC-derived cartilage microtissue formation under defined culture conditions.

Engineering phenotypically favorable cartilage-like tissue from bone marrow mesenchymal stem cells (BMSCs) remains challenging because chondrogenic differentiation can be accompanied by insufficient cartilage-associated extracellular matrix accumulation and unfavorable hypertrophic and catabolic responses. Here, we established a microcurrent-assisted culture system for scaffold-free BMSC-derived cartilage microtissues (CMTs) and investigated the intensity-dependent effects of continuous unidirectional direct-current stimulation at 0, 50, 100, 150, 250, or 500 μA under a defined electrical and

Multimodal validation of temporal interference in a 3D-printed pediatric head phantom.

BACKGROUND: Transcranial temporal interference stimulation (tTIS) is a technique for generating electric-field envelopes within deep brain structures. Yet, its application in human subjects is limited, particularly in pediatric cohorts. OBJECTIVE: To evaluate the validity and accuracy of tTIS envelope reconstruction across beta, gamma, and ripple frequency bands in deep brain structures using a 3D-printed pediatric phantom. METHODS: A phantom resembling the conductivity properties of a 3-year-old child's head was implanted with dipoles at six deep-brain locations. Sinusoidal signals were del
