CD4+ T cell activation and associated susceptibility to HIV-1 infection in vitro increased following acute resistance exercise in human subjects.

Early studies in exercise immunology suggested acute bouts of exercise had an immunosuppressive effect in human subjects. However, recent data, show acute bouts of combined aerobic and resistance training increase both lymphocyte activation and proliferation. We quantified resistance exercise-induced changes in the activation state of CD4+ T lymphocytes via surface protein expression and using a medically relevant model of infection (HIV-1). Using a randomized cross-over design, 10 untrained subjects completed a control and exercise session. The control session consisted of 30-min seated rest

Minimizing right ventricular pacing in patients with sinus node disease and prolonged PQ interval: The impact on exercise capacity.

BACKGROUND: Several clinical and experimental studies have shown that unnecessary right ventricular pacing in sinus node disease can be detrimental. Inter- and intra-ventricular asynchrony imposed by right ventricular pacing may cause reduction in contractility and relaxation of left ventricle, worsening mitral regurgitation, regional redistribution of myocardial perfusion and oxygen consumption, and asymmetrical hypertrophy of left ventricular wall. In some patients, sinus node disease coexists with impaired atrioventricular conduction. The optimal pacing mode in this population is not determ

Myocardial strain analysis by 2-dimensional speckle tracking echocardiography improves diagnostics of coronary artery stenosis in stable angina pectoris.

BACKGROUND: Two-dimensional strain echocardiography detects early signs of left ventricular dysfunction; however, it is unknown whether myocardial strain analysis at rest in patients with suspected stable angina pectoris predicts the presence of coronary artery disease (CAD). METHODS AND RESULTS: In total, 296 consecutive patients with clinically suspected stable angina pectoris, no previous cardiac history, and normal left ventricular ejection fraction were included. All patients were examined by 2-dimensional strain echocardiography, exercise ECG, and coronary angiography. Two-dimensional s

The role of cardiopulmonary exercise test in mitral and aortic regurgitation: it can predict post-operative results.

BACKGROUND: We evaluated the efficacy of the cardiopulmonary exercise test as an objective indicator of functional status and as a pre-operative prognostic indicator in patients with mitral regurgitation (MR) and aortic regurgitation (AR). METHODS: Cardiopulmonary exercise tests and echocardiography were performed in 47 patients (MR: 30, AR: 15, MR + AR: 2) before surgery and repeated one year after surgery. We compared the New York Heart Association (NYHA) functional class, peak oxygen consumption rate (VO2peak), exercise duration, left ventricular dimension and ejection fraction, before and

Intrauterine hyperglycaemia during late gestation caused mitochondrial dysfunction in skeletal muscle of male offspring through CREB/PGC1A signaling.

BACKGROUND: Maternal diabetes mellitus can influence the development of offspring. Gestational diabetes mellitus (GDM) creates a short-term intrauterine hyperglycaemic environment in offspring, leading to glucose intolerance in later life, but the long-term effects and specific mechanism involved in skeletal muscle dysfunction in offspring remain to be clarified. METHODS: Pregnant mice were divided into two groups: The GDM group was intraperitoneally injected with 100 mg/kg streptozotocin on gestational days (GDs) 6.5 and 12.5, while the control (CTR) group was treated with vehicle buffer. On

Potassium-competitive acid blockers for Helicobacter pylori eradication in pediatrics: a narrative review of pharmacologic rationale, clinical evidence, and practical considerations.

Helicobacter pylori (H. pylori) infection remains highly prevalent worldwide and is strongly associated with chronic gastritis, peptic ulcer disease, and an elevated risk of gastric cancer. In pediatric clinical practice, eradication therapy is challenged by rising antibiotic resistance, inadequate acid suppression, and poor adherence to complex multidrug regimens. Potassium-competitive acid blockers (P-CABs) represent a newer class of antisecretory agents that competitively and reversibly inhibit gastric H+/K+-ATPase, resulting in a more rapid onset and more sustained control of intragastric

Long-Term Efficacy and Safety of ARRY-371797 (PF-07265803) in Patients With Lamin A/C-Related Dilated Cardiomyopathy.

Dilated cardiomyopathy associated with lamin A/C (LMNA) gene variants (LMNA-related dilated cardiomyopathy [DCM]) is a life-threatening condition with a high unmet need, accounting for approximately 6% of idiopathic DCM cases. Currently, no disease-specific treatments target the underlying disease mechanism. ARRY-371797 (PF-07265803), a potent, selective, oral, small-molecule inhibitor of the p38α mitogen-activated protein kinase pathway, improved 6-minute walk test (6MWT) distance in 12 patients with symptomatic LMNA-related DCM in a 48-week, open-label, phase 2 study. Acute vestibular neuritis: A rare complication after the adenoviral vector-based COVID-19 vaccine.

Vestibular neuritis was first reported in 1952 by Dix and Hallpike, and 30% of patients reporting a flu-like symptom before acquiring the disorder. The most common causes are viral infections, often resulting from systemic viral infections or bacterial labyrinthitis. Here we presented a rare case of acute vestibular neuritis after the adenoviral vector-based COVID-19 vaccination. A 51-year-old male pilot awoke early in the morning with severe vertigo, nausea, and vomiting after receiving the first dose of the ChAdOx1 nCoV-19 vaccine 11 days ago. The post-thrombotic syndrome: a 2012 therapeutic update.

Post-thrombotic syndrome (PTS) refers to chronic manifestations of venous insufficiency following a deep-vein thrombosis (DVT). It is a frequent, chronic, burdensome and costly disease for which therapeutic options are limited. Above all, the optimal management of PTS consists of preventing its occurrence: first, by preventing DVT, and second, by preventing development of PTS after a DVT. Prevention of DVT is challenging, particularly in the case of nonsurgical hospital inpatients, where physician's adherence to recommended thromboprophylaxis is often low. In our opinion, this adherence should

Efficacy of traditional swallowing therapy plus neuromuscular electrostimulation in multiple sclerosis: A randomized controlled trial.

BACKGROUND: Dysphagia is a common and potentially life-threatening complication in people with multiple sclerosis (pwMS), even in early disease stages. Although compensatory strategies may reduce aspiration risk, they do not restore normal swallowing. Neuromuscular Electrical Stimulation (NMES) has shown promise in dysphagic populations, but evidence in pwMS is scarce. OBJECTIVES: This double-blind, randomized, placebo-controlled trial aimed to evaluate the feasibility and added benefit of NMES combined with standard swallowing therapy in pwMS with dysphagia. RESULTS: Of 151 dysphagic pwMS s

Advances in the management of obstructive hypertrophic cardiomyopathy: The role of mavacamten.

INTRODUCTION: Hypertrophic cardiomyopathy (HCM) is a common inherited cardiovascular disorder characterized by asymmetric left ventricular hypertrophy and increased risk of sudden cardiac death. While current treatments like beta-blockers, calcium channel blockers, and invasive procedures provide symptom relief, they do not prevent disease progression. Mavacamten, a first-in-class cardiac myosin inhibitor, offers a promising alternative by directly targeting the underlying cause of left ventricular outflow tract (LVOT) obstruction in obstructive HCM. This drug reduces myosin ATPase activity, d

Oral Rehydration Salt Solutions for Children: A Review.

Dehydration continues to be a major contributor to morbidity and mortality in children globally. Oral rehydration salts (ORS) solutions can successfully treat mild to moderate dehydration. However, the uptake of this simple and cost-effective intervention remains low in both high-income and low-to-mid-income countries. The low-osmolality ORS solution recommended by the World Health Organization (WHO) contains an appropriate balance of electrolytes and glucose that helps effectively absorb water and electrolytes, irrespective of the cause of gastroenteritis. ORS solution can be administered ora
