A Scoping Review of the WRIST Trial and Its Subanalyses: Implications for Care of Elderly Distal Radius Fractures.

BACKGROUND: Distal radius fractures are common injuries in older adults. There is disagreement on the optimal treatment options for distal radius fractures in this patient population. The Wrist and Radius Injury Surgical Trial (WRIST) was a randomized clinical trial comparing the most common treatment options for distal radius fractures in this population (casting, closed reduction and percutaneous pinning [CRPP], external fixation [EFP], and volar locking plate systems [VLPS]). The objective of this review is to summarize the findings across all articles stemming from the data collected in th

Clearance of hypermutator XDR Pseudomonas osteomyelitis and hardware infection with adjunctive bacteriophage therapy.

BACKGROUND: Bacteriophages (phages), viruses ubiquitous in the environment infecting bacteria, are an increasingly attractive option for adjunct therapy with conventional antibiotics in complicated, refractory multidrug-resistant infections. Bacteria ("hypermutators") with defects in methyl-directed mismatch repair (MMR) have been described in Pseudomonas aeruginosa, with the resultant defect in DNA repair responsible for rapid development of antimicrobial resistance. CASE SUMMARY: A 72-year-old male with mild chronic obstructive pulmonary disease, diagnosed with resectable, advanced right fr

Accordion Maneuver for Delayed Regenerate Formation Following Pediatric Osteosarcoma Resection: A Case Report and Literature Review.

Background and Clinical Significance: Reconstruction of large segmental bone defects following oncologic resection in pediatric patients remains a major challenge. Although distraction osteogenesis with bone transport is a well-established biological reconstructive option, regenerate formation may be compromised in patients receiving chemotherapy. The accordion maneuver, consisting of alternating cycles of compression and distraction, has been described as a method to stimulate bone regeneration, primarily in association with external fixation systems. However, its use in internal bone transpo

Can we predict postoperative dysphagia after anterior cervical discectomy and fusion based on lateral x-ray? An external validation of two outcome scores.

Postoperative dysphagia following Anterior cervical discectomy and fixation (ACDF) impacts quality of life and risks airway complications. Identification of retropharyngeal oedema with a lateral Xray has gained interest as screening method. We prospectively evaluated the relation between dysphagia and two radiological scores to determine the external predictive value. A prospective study (2021-2022) held in a tertiary centre, including a sequential sample of patients operated of ACDF for degenerative conditions. Postoperative (day + 1) evaluation of dysphagia was systematically performed by a

Development of a radiation-free magnetic guidance device for distal screw placement in intramedullary nails.

This study developed a practical device for guiding distal interlocking screws during intramedullary nail fixation, aiming to address issues associated with the conventional free-hand approach using fluoroscopy. The objectives were to attain precision, reduce radiation exposure, and ease of operation. A small cylindrical magnet was embedded in the distal screw hole to emit signals for positioning. Due to the magnet's relatively weak magnetic force, especially in clinical settings with obese patients, the required long sensing distance is significant, making it susceptible to interference from

A modified Yamane technique with a posterior approach in a case report of intraocular lens dislocation.

INTRODUCTION: The Yamane intrascleral fixation technique is a widespread surgical approach used in ophthalmology to secure three-piece intraocular lenses (IOLs) without the use of sutures. This technique is particularly beneficial in cases where the capsular bag is not suitable for IOL implantation. By creating a flange at the tip of each haptic this procedure innovatively eliminates the need for additional securing materials. We report an alternative management of posterior intraocular lens (IOL) dislocation with a modified Yamane technique, performed in vitreous chamber. CASE PRESENTATION:

Bone regeneration during osteoporosis: a translational in vivo monitoring of callus mechanical parameters.

INTRODUCTION: Regenerating osteoporotic bone remains challenging due to healing complications such as non-unions. Distraction osteogenesis is a promising technique for bone repair, but its efficacy under osteoporotic conditions is poorly understood. This study provides in vivo quantitative translational knowledge on the influence of osteoporosis in distraction callus mechanics. METHODS: Fifteen Merino sheep were induced with osteoporosis. A 15 mm bone defect in the right hind metatarsus was treated with distraction osteogenesis and stabilized with an instrumented external fixator. Callus dist

The use of regenerative technologies for treatment of delayed fracture union in patient with menopausal osteoporosis.

Delayed fracture union is a common complication in orthopedic practice, particularly in patients with osteoporosis. Advances in regenerative medicine, including mesenchymal stem cells (MSCs) within the stromal-vascular fraction, have introduced novel therapeutic approaches to address this challenge. This case describes a patient with postmenopausal osteoporosis and a congenital anomaly of the left lower limb who sustained a proximal tibial fracture with fragment displacement. Osteoporosis therapy was initiated, and the fracture was managed using external fixation and compression. Despite an im

Outcomes of Tibiotalocalcaneal Fusion After Partial Talectomy in the Charcot Ankle.

BACKGROUND: Whether a sequela of neuropathy or a result of infectious processes to dysvascular changes, talar degeneration in the Charcot ankle proves difficult to treat. Total resection followed by bone void filler or grafts is costly, with varying levels of success. Although tibiocalcaneal fusion allows stabilization, this approach remains a procedure of last resort before amputation. This series presents outcomes of partial talectomy with tibiotalocalcaneal arthrodesis. METHODS: Nineteen patients with Charcot's neuroarthropathy necessitating a tibiotalocalcaneal fusion were included in thi

Distances and angles in standing long-leg radiographs: comparing conventional radiography, digital radiography, and EOS.

OBJECTIVE: Distances and angles measured from long-leg radiographs (LLR) are important for surgical decision-making. However, projectional radiography suffers from distortion, potentially generating differences between measurement and true anatomical dimension. These phenomena are not uniform between conventional radiography (CR) digital radiography (DR) and fan-beam technology (EOS). We aimed to identify differences between these modalities in an experimental setup. MATERIALS AND METHODS: A hemiskeleton was stabilized using an external fixator in neutral, valgus and varus knee alignment. Ten

Resolved Chronic Non-Healing Ulcer After Distal Radius Giant Cell Tumor Resection: Nursing Experience and Wound Care.

BACKGROUND Giant cell tumors of bone typically occur in early adulthood, when the growth plate has closed. The distal radius is the second most common location affected, accounting for 10% to 12% of cases. Complications of poor soft tissue healing are rare in the distal radius, owing to its rich blood supply. However, the curettage procedure and use of bone cement and external fixation can affect the local blood supply. CASE REPORT We present a rare case of a 24-year-old woman with no significant medical history who underwent surgery at a local hospital to treat a giant cell tumor of the radiu

Vascular rehabilitation in children with chronic intestinal failure reduces the risk of central-line associated bloodstream infections and catheter replacements.

BACKGROUND & AIMS: Children with chronic intestinal failure (IF) require a long-term central venous catheter (CVC) for provision of parenteral nutrition. Vascular, mechanical and infectious complications such as central line-associated bloodstream infections (CLABSI) may lead to progressive loss of venous access sites. Handling and management of CVCs therefore play an important role. Our vascular rehabilitation concept (VRC) is a core component of our intestinal rehabilitation program (IRP) and consists of an education program, optimization of skin care, catheter fixation and lock solution, an

The Center-Center Image Closely Approximates Other Methods for Syndesmosis Reduction Clamp Placement.

BackgroundThe optimal placement for a syndesmosis reduction clamp remains an open question. This study compared the center-center axis, which localizes clamp placement using only an internally rotated lateral ankle X-ray, with other common approaches, whose accuracy can only be confirmed using computed tomography (CT).MethodsBone models of anatomically aligned (n = 6) and malreduced (n = 48) limbs were generated from CT scans of cadaveric specimens. Four axes for guiding clamp placement (center-center, centroid, B2, and trans-syndesmotic) were then analyzed, using digitally reconstructed radio
