[Scapho-trapezio-trapezoidal arthrodesis for scapho-trapezio-trapezoidal osteoarthritis].

Scapho-trapezio-trapezoid arthrodesis was originally performed for the treatment of scapho-lunate instability. However, only a few publications have described this technique for treatment of osteoarthritis of the scapho-trapezio-trapezoid (STT) joint. The purpose of this paper is to analyze the results of triscaphoid arthrodesis for STT osteoarthritis with a long-term follow-up. Thirteen cases of osteoarthritis of the STT joint in twelve patients, all treated by STT arthrodesis, were reviewed with an average follow-up of 60 months. Pain was classified according to Alnot's classification: eight

[Functional results after proximal row carpectomy (PRC) in patients with SNAC-/SLAC-wrist stage II].

The proximal row carpectomy (PRC) is a motion preserving procedure which creates a new joint without arthrosis. It is a frequently used procedure in stage II of a posttraumatic degenerative arthrosis of the wrist after scaphoid nonunion or scapholunate ligament instability (SNAC-/SLAC-wrist). In this retrospective analysis the functional postoperative results of this operation are compared in light of a homogenous indication (SNAC-/SLAC-wrist stage II). In 38 patients PRC was performed for a stage II SNAC- (n = 29) or SLAC-wrist (n = 9) between June 1994 and March 2002. Postoperative examinati

Factors affecting the outcome in 50 cases of scaphoid nonunion treated with Herbert screw fixation.

Our experience in the treatment of 50 scaphoid nonunions treated with Herbert screw fixation is reviewed. Success rates fell off as the duration of nonunion before treatment increased. However, this appeared to be related to the increased incidence of avascular necrosis with time rather than simply the factor of time alone. In this series the major adverse determinants for outcome were avascular necrosis and a history of previous surgery for nonunion. Failure to graft in a selected group with an intact cartilaginous envelope or a stable firm fibrous union did not affect results. The heterogene

Optimizing clinical assessment of trapeziometacarpal arthroplasty: Introducing the buffalo score.

BACKGROUND: Numerous patient-reported outcome measures are used to assess thumb and hand function, but none are specific to the trapeziometacarpal (TMC) joint. The purpose of this study was to describe and validate a TMC-specific patient-reported outcome measure: the Buffalo Score. METHODS: The Buffalo Score was compared with validated functional scores (QuickDASH, Kapandji score, Trapeziometacarpal Arthrosis Symptoms Disability (TASD) questionnaire) and with objective hand measurements (lateral pinch strength and subjective strength evaluation by number of paper folds a patient can do). Nine

Assessing the causal influence of biomechanical factors on osteoporosis risk: A multivariable Mendelian randomization investigation.

While observational studies indicate correlations between multiple biomechanical traits and osteoporosis, causal inference has been constrained, impeding the clinical application of these measures in risk stratification. This Mendelian randomization (MR) study examines causal associations of ankle spacing width (ASW), height, body mass index (BMI), grip strength, and usual walking pace with bone mineral density (BMD) across skeletal sites and age groups. Genetic proxies for the exposures were derived from the Medical Research Council Integrative Epidemiology Unit and UK Biobank resources. Gene

Combined home and clinic-based therapy versus home-based exercise programme after distal radial fracture: a randomized controlled study.

We investigated outcomes of clinic-based hand therapy combined with a home-based exercise programme after anterior plating for distal radial fractures. A total of 102 patients were randomly assigned to one of three groups: a home-based exercise programme alone; a home-based exercise programme combined with four hand therapy sessions in the clinic; and a home-based exercise programme with seven sessions in the clinic. Mean Patient-Rated Wrist Evaluation scores at 6 weeks were significantly better for the group of patients with seven sessions in the clinic than in those with only home exercises

Effect of Time-To-Surgery on Distal Radius Fracture Outcomes: A Systematic Review.

PURPOSE: It remains unclear whether time-to-surgery for distal radius fractures affects clinical, functional, or radiographic outcomes or health care costs/use. This systematic review investigated the outcomes of early versus delayed surgery for closed, isolated distal radius fractures in adult patients. METHODS: A comprehensive search of MEDLINE, Embase, and CINAHL databases was completed for all original case series, observational studies, and randomized controlled trials reporting clinical outcomes of both early and delayed surgically-treated distal radius fractures from database inception

Bone mineral density, limb muscle mass, muscle strength, and exercise capacity are reduced in female patients with distal radius fractures when the unaffected side grip strength is less than 18 kg.

BACKGROUND: Prevention of domino effects after distal radius fractures is important for improving life expectancy. Fragility fractures secondary to falls are associated with decreased bone mineral density, muscle strength, and exercise capacity. Grip strength is one of the simplest and most useful tests to comprehensively judge muscle strength. The purpose of this study was to examine whether grip strength is associated with bone mineral density, limb muscle mass, muscle strength, and exercise capacity, by comparing patient backgrounds based on the presence or absence of grip weakness in femal

Smoking and Alcohol Intake but Not Muscle Strength in Young Men Increase Fracture Risk at Middle Age: A Cohort Study Linked to the Swedish National Patient Registry.

We aimed to determine the relationship between handgrip strength, smoking, and alcohol consumption in young men and fracture risk at middle age. Thus, we carried out a cohort study including young men undergoing conscription examination in Sweden from September 1969 to May 1970 at a typical age of 18 years. Data on muscle strength, height, weight, and lifestyle factors were linked to the National Patient Register 1987-2010. Handgrip strength was considered the main exposure and smoking and alcohol consumption as secondary exposures. Outcomes were all fractures (except face, skull, digits), maj

A 52-Year-Old Man With a Tuft Fracture and Hand Cellulitis.

A 52-year-old man presented to the emergency department (ED) 1 week after getting his right index finger shut in a car door. The patient complained of right index finger pain. His entire hand was edematous and reddened. After evaluation in the ED and x-ray, the patient was diagnosed with a tuft fracture, right index finger/hand cellulitis, and possible osteomyelitis. The patient received tetanus diphtheria i.m., vancomycin 1 g i.v., and ceftriaxone (Rocephin) 2 g i.v. while in the ED and was admitted with referral to a hand specialist. The patient was discharged after 10 days of parental antib

Hermann Friedberg's case report: an early description of CLOVES syndrome.

CLOVES syndrome is a recently described overgrowth disorder with complex vascular anomalies. Careful analysis of the case report by the German physician Hermann Friedberg "gigantism of the right lower limb" published in 1867 revealed that the report probably represents one of the first written accounts of CLOVES syndrome.

[Littler tenodesis for correction of swan neck deformity in rheumatoid arthritis].

PURPOSE: The aim of this study was to assess the results of operative treatment for rheumatoid swan neck deformity using Littler's technique consisting in the reconstruction of the oblique retinacular ligament. PATIENTS AND METHOD: From 2004 to 2007 twenty rheumatoid patients with 30 PIP-joints affected by swan neck deformity underwent surgical correction. In all cases the tenodesis described by Littler was used. Modification of the operative procedure because of insufficiency of the Cleland ligament or the A2-pulley was in no case necessary. Twenty six PIP-joints in 17 patients could be exam

Paralysis of the first dorsal interosseous muscle after external fixation of a distal radial fracture treated by transfer of the extensor indicis proprius tendon.

We describe a patient in whom the motor branch to the first dorsal interosseous muscle was injured by the pins of an fixator used to treat an unstable fracture of the distal radius. She was successfully treated by extensor indicis proprius transfer to the base of the proximal phalanx of the index finger.

Corrective osteotomy for malunited extra-articular fractures of the distal radius: a follow-up study of 33 patients.

We present a series of 33 consecutive patients treated with lengthening osteotomy due to malunited extra-articular fractures of the distal radius. Thirty-one patients were able for long-term follow-up a median (range) of seven (2-20) years after the procedure. The indication for reconstruction was mainly impaired function of the wrist. Both the anatomy and function were improved significantly postoperatively. The median radial length improved 5 mm, the radial tilt 25 degrees, and the radial inclination improved 9 degrees. The median improvement of forearm supination was 20 degrees, pronation 1

Extra help required by litigants after simple fractures--a questionnaire based study.

A questionnaire based interview of 100 patients under fracture clinic review was undertaken to assess the help required with the activities of daily living (ADL) in the first 2 weeks following fractures of the hand, wrist, neck of humerus, foot or ankle. The amount of help patients required was correlated with whether or not the patients were claiming compensation for their injuries. Twenty-five percent were litigating and this was shown to influence the amount of help a patient reported having received with dressing, shopping, cooking, housework, personal hygiene and travelling (p<0.0001) and

Adult hand injuries on artificial ski slopes.

Artificial ski slope skiing is a popular sport in Great Britain with significant risk of injury. We reviewed 36 patients who had been treated for hand injuries excluding the wrist in our plastic surgery department over a 14-month period after skiing on an artificial ski slope. Specifically, they were 15 fractures of the digits, 3 dislocations of the finger joints, 7 ulnar collateral ligament injuries of the metacarpophalangeal joint of the thumb, and 11 bruises and abrasions. The mean age of the patients was 28 years, and there were 20 men and 16 women. It is concluded that the hardness of the

Hand and wrist injuries: Part II. Emergent evaluation.

Primary care physicians must be able to recognize wrist and hand injuries that require immediate attention. A complete history and physical examination, including assessment of distal limb function, are essential. Hemorrhage control is necessary in patients with vessel lacerations and amputations. Amputations require an understanding of the indications and contraindications in the management of the amputated limb. High-pressure injection injuries and compartment syndromes require a high index of suspicion for early recognition. Infectious entities include "fight bite," open fractures, purulent

Non-vascularized free toe phalanx transfers in congenital hand deformities--the Great Ormond Street experience.

Many options of varying complexity are available for the management of congenital short digits resulting from aphalangia in symbrachydactyly and constriction ring syndrome. We have used non-vascularized free toe phalanx transfers for these children when a vascularized toe transfer has been contraindicated. We describe our technique and experience with 22 children who underwent a total of 64 transfers of the proximal (35) or middle (29) toe phalanges (average 3 per child). The mean age at initial surgery was 15 months, and the mean follow-up was 5 years. Duration of time until epiphyseal closur
