Simultaneous Transfemoral Mitral and Tricuspid Valve in Ring Implantation: First Case Report with Edwards Sapien 3 Valve.

Patients with advanced valvular disease may be at high or prohibitive risk for surgical management. We describe a patient with previous mitral and tricuspid valve repair and recurrent admissions for New York Heart Association Class IV heart failure symptoms due to severe mitral stenosis and severe tricuspid regurgitation. Due to her comorbidities and two previous sternotomies, the patient was at high risk for surgery. We performed a simultaneous transfemoral mitral and tricuspid valve-in-ring implantation. This is the first report of its kind using a Sapien 3 valve (Edwards Lifesciences).

A novel method to manage a dislodged self-expanding transcatheter aortic valve.

Despite advancements in technology, operator experience, and procedural planning, transcatheter aortic valve replacement (TAVR) procedures are complex, and complications remain inevitable. Valve embolization may prove to be fatal and conventional rescue techniques are dependent on the anatomy of the aorta. We describe a case of postimplant embolization of a self-expanding valve during valve-in-valve application where the valve could not be stabilized due to the anatomy of the aorta and a novel technique was utilized to stabilize the valve in the aortic arch using a wire fixed to the left axill

Valve sparing-root replacement with the reimplantation technique to increase the durability of bicuspid aortic valve repair.

OBJECTIVES: To assess root replacement and annular stabilization in bicuspid aortic valve repair, we compared results of reimplantation technique versus subcommissural annuloplasty or no annuloplasty. METHODS: Between 1995 and 2010, 161 consecutive patients underwent bicuspid aortic valve repair. Patients undergoing subcommissural annuloplasty or no annuloplasty (group 1, n = 87) had larger root dimensions and less aortic insufficiency than did patients undergoing reimplantation technique (group 2, n = 74). We matched groups 1 to 1 on basis of those criteria. After matching (n = 106, n = 53 p

Transfemoral Edwards-Novaflex valve implantation in a patient with aorto-iliac endoprosthesis and severely tortuous bilateral external iliac arteries-"Railing track".

Transcatheter aortic valve implantation (TAVI) has nowadays been introduced as an alternative for surgical aortic valve replacement as a treatment for high risk aortic stenosis patients. This procedure is not free of complications: the SOURCE registry, indeed, showed that vascular complications are more frequent with the transfemoral approach. We present the case of an 82-year-old man with known history of severe aortic stenosis at high-risk for surgery. Pre-TAVI screening shows bilateral severely tortuous iliac arteries and aorto-bi-iliac endoprosthesis. Transapical TAVI as a first choice was

Unruptured, isolated giant aneurysm of the sinus of valsalva resulting from medial mucoid degeneration.

We report a quite rare case of unruptured, isolated giant aneurysm of the sinus of Valsalva resulting from medial mucoid degeneration in a young adult woman. A 29-year-old Japanese female diagnosed as having an aneurysm of the sinus of Valsalva and severe aortic regurgitation with no clinical findings of Marfan's syndrome or Ehlers-Danlos syndrome. A modified Bentall's operation was performed successfully, and she was discharged with no complications. A pathological examination revealed marked medial mucoid degeneration of the aneurismal wall. In the literature, most giant aneurysms resulting

Aortic Stenosis and Small Aortic Annulus.

The presence of a small aortic annulus poses a considerable challenge in the management of patients with severe aortic stenosis, especially in elderly women, where it is a very frequent finding. The optimal approach for treating these patients remains controversial, and several surgical strategies such as aortic root enlargement, supra-annular stented prosthetic valves, stentless bioprosthesis, and sutureless bioprostheses have been proposed to improve valve hemodynamics and clinical outcomes. More recently, transcatheter aortic valve replacement has emerged as a valid alternative for the trea

Safety and Efficacy of Transcatheter Aortic Valve Replacement in the Treatment of Pure Aortic Regurgitation in Native Valves and Failing Surgical Bioprostheses: Results From an International Registry Study.

OBJECTIVES: The aim of this study was to evaluate the use of transcatheter heart valves (THV) for the treatment of noncalcific pure native aortic valve regurgitation (NAVR) and failing bioprosthetic surgical heart valves (SHVs) with pure severe aortic regurgitation (AR). BACKGROUND: Limited data are available about the "off-label" use of transcatheter aortic valve replacement (TAVR) to treat pure severe AR. METHODS: The study population consisted of patients with pure severe AR treated by TAVR at 18 different centers. Study endpoints were device success, early safety, and clinical efficacy a

Transcatheter mitral valve regurgitation treatment: State of the art and a glimpse to the future.

Since the first transcatheter heart valve implantation in the pulmonary position in 2000 and in the aortic position in 2002, a large number of transcatheter heart valves have reached the clinical arena and thousands of high-risk patients have been treated successfully, in particular those with severe aortic stenosis. In contrast, the experience of transcatheter mitral valve repair or implantation started relatively more recently, and only a few devices are available at the moment. The aim of this review is to describe the different percutaneous systems for the treatment of mitral regurgitation

Transapical aortic valve and mitral valve in ring prosthesis implantation - a new advance in transcatheter procedures.

Transcatheter valve implantation offers a new treatment modality to those patients whose general condition makes conventional surgery very risky. However, the transcatheter option has only been available for the aortic valve. We describe a case of a successful implantation of two Edwards SAPIEN(®) 26 and 29 mm transapical valves, respectively, in aortic and mitral positions, on a 74-year-old patient with severe aortic and mitral stenosis. The procedure progressed uneventfully. Predischarge echocardiogram showed a peak aortic gradient of 20 mmHg, mild periprosthetic regurgitation, peak and mean

Hemodynamic outcomes of transcatheter aortic valve implantation with the CoreValve system: an early assessment.

BACKGROUND AND AIMS: Transcatheter aortic valve implantation (TAVI) is an established method for the treatment of high-risk patients with aortic stenosis (AS). The beneficial effects of TAVI in cardiac hemodynamics have been described in recent studies, but those investigations were mostly performed after an interval of more than 6 months following aortic valve implantation. The aim of this study is to investigate the acute and short-term alterations in hemodynamic conditions using the echocardiography outcomes in patients undergoing TAVI. METHODS AND RESULTS: A total of 60 patients (26 males

Small aortic annulus: a functional definition.

BACKGROUND & OBJECTIVE: Small aortic annulus is conventionally associated with poor outcome after aortic valve replacement (AVR). Contrarily, several patients have excellent follow-up results after AVR with 19, 20 or 21 size Medtronic Hall (MH) or Sorin Carbocast (SC) prostheses. This disparity prompted a relook at the semantics of a small aortic annulus. METHODS: Available survivors of isolated AVR with #19, #20 or #21 prostheses - 13 with 19 SC or 20 MH valves (Group A) and 29 with 21 SC or MH valves (Group B) were studied. Disparity between actually implanted prostheses versus predicted pr

Leaflet Splitting to Prevent TAVR-Induced Coronary Obstruction in Stented vs Stentless Surgical Bioprosthetic Valves.

BACKGROUND: Transcatheter electrosurgical leaflet splitting prevents coronary artery obstruction (CAO) following transcatheter aortic valve replacement (TAVR), particularly in valve-in-valve interventions. Morphologic and fluoroscopic differences between stented and stentless valves may affect outcomes, but comparative data are limited. OBJECTIVES: The aim of this study was to compare procedural characteristics and outcomes of BASILICA (bioprosthetic or native aortic scallop intentional laceration to prevent iatrogenic CAO) and TAVR in stented vs stentless surgical bioprosthetic valves. Redo Surgical Aortic Valve Replacement vs Valve-in-Valve Transcatheter Aortic Valve Replacement for Degenerated Bioprosthetic Valves.

BACKGROUND: Valve-in-valve transcatheter aortic valve replacement (ViV-TAVR) is associated with improved perioperative safety compared with redo surgical aortic valve replacement (redo-SAVR), but long-term outcomes remain uncertain. We therefore compare long-term outcomes of ViV-TAVR and redo-SAVR. METHODS: The study included 1:1 propensity score-matched Medicare beneficiaries with degenerated bioprosthetic valves admitted between September 29, 2011, and December 30, 2020, undergoing either redo-SAVR or ViV-TAVR. Exclusion criteria included endocarditis, other concomitant cardiac surgery, and

Mitral valve anatomy: pre-procedural screening and imaging techniques.

Mitral regurgitation is a source of significant morbidity and mortality with growing prevalence in an increasingly ageing population. The development of reparative percutaneous techniques allows offering treatment to patients ineligible for surgery. The mitral valve is a complex structure and these procedures are limited by anatomical variability and technical complexity. The most established mitral valve repair technique currently is the MitraClip, which requires a careful preoperative evaluation of the mitral valve apparatus to assess the patient's suitability for the intervention, where ech

Aortic annulus diameter measurement: what is the best modality?

PURPOSE: The purpose of this study was to clarify the most suitable method to measure the aortic annulus diameter. PATIENTS AND METHODS: Fifty-five patients, who had undergone aortic valve replacement at Miyazaki University Hospital between April 2008 and May 2011, were included in this study. The maximum diameter of the sizing tool that could be inserted into the left ventricle through the annulus had been predicted, based on the diameter measured by each modality. Agreement with surgery and each imaging modality, namely transthoracic echocardiography, multidetector computed tomography and c
