Morphologic classification and incidence of hematopoietic neoplasms in the Sprague-Dawley rat.

Hematopoietic and related neoplasms were morphologically studied in a total of 1,765 male and 1,765 female Sprague-Dawley rats from 6 chronic toxicity studies conducted at a large toxicology testing laboratory. The most common types of lymphoid neoplasms seen included the lymphoblastic lymphoma (0.65%) and the large granular lymphocyte lymphoma (LGL, Fischer or mononuclear cell leukemia) (0.60%). The most common type of nonlymphoid neoplasm with morphologic features similar to lymphoid tumors was histiocytic sarcoma (1.1%). A small number of cases of myelogenous leukemia were also seen. Spread according to histology in non-Hodgkin's lymphomata.

The spread of non-Hodgkin's lymphomata was investigated in a group of 323 cases. Differences were sought between 4 histological groups: lymphocytic vs histiocytic lymphomata and diffuse vs follicular types. Histiocytic and follicular types were rare in childhood. Histiocytic lymphomata, compared with lymphocytic ones, were more often confined to sites above the diaphragm. They exhibited a higher rate of loco-regional recurrences within the first 3 months and also of late recurrence after 4 years. Lymphocytic lymphomata of the follicular type more commonly presented with nodal involvement below

Genomic signatures in B-cell lymphoma: How can these improve precision in diagnosis and inform prognosis?

Current genomic technologies have immensely improved disease classification and prognostication of major subtypes of B-cell lymphomas. This novel genetic information has not only aided in diagnosis, but has also revealed a landscape of critical molecular events that determine the biological and clinical behavior of a lymphoma. In this review, we summarized the genetic characteristics of major subtypes of B-cell lymphomas, including diffuse large B cell lymphoma (DLBCL), follicular lymphoma (FL), Burkitt lymphoma (BL), and mantle cell lymphoma (MCL). We illustrated how genomic profiling had ide

Treatment and Outcome Analysis of 639 Relapsed Non-Hodgkin Lymphomas in Children and Adolescents and Resulting Treatment Recommendations.

Despite poor survival, controversies remain in the treatment for refractory or relapsed pediatric non-Hodgkin lymphoma (r/r NHL). The current project aimed to collect international experience on the re-induction treatment of r/r NHL, hematopoietic stem cell transplantation (HSCT), risk factors associated with outcome, and to suggest treatment recommendations. Inclusion criteria were (i) refractory disease, disease progression or relapse of any NHL subtype except anaplastic large cell lymphoma, (ii) age < 18 years at initial diagnosis, (iii) diagnosis in/after January 2000. Data from 639 eligib

Three coexisting lymphomas in a single patient: composite lymphoma derived from a common germinal center B-cell precursor and unrelated discordant lymphoma.

Composite lymphoma (CL) is a rare disorder defined as the coexistence of two or more distinct lymphoma subtypes at a single anatomic site. Discordant lymphoma (DL), which is the simultaneous occurrence of two or more distinct lymphoma subtypes at different sites, is also rare. CL complicated with DL involving three distinct subtypes of lymphoma in the same patient is an extremely rare disease. Clonal relationships in CL and DL are commonly investigated by molecular analysis using mutational status with t(14;18)BCL2/IgH translocation and immunoglobulin heavy chain variable-region (IgVH) gene re

Acquisition of a t(11;14)(q13;q32) in clonal evolution in a follicular lymphoma with a t(14;18)(q32;q21) and t(3;22)(q27;q11.2).

Chromosome translocations involving an immunoglobulin (IG) locus and another gene, either BCL or MYC, are common events in B-cell lymphoma. Occasionally, two IG loci, one with BCL and the other with MYC, are simultaneously involved; such cases are classified as double-hit (DH) lymphomas. These tumors often show intermediate histologic features between those of diffuse large B-cell lymphoma and those of Burkitt lymphoma. Patients with DH lymphoma have a poor prognosis. Rarely, lymphomas in which three IG loci are simultaneously involved with two different BCL genes and MYC have been reported. T

Aggressive B-cell lymphomas with a primary bone marrow presentation.

Aggressive B-cell lymphomas present as a heterogeneous spectrum of disease. A primary diagnosis in the bone marrow (BM) may be challenging in terms of diagnostic classification and clinical handling, owing to limited architectural information. Aggressive B-cell lymphomas can be subdivided into entities that typically present primarily in the BM, and cases with BM involvement in which the bulk of disease is present in other organs. One main topic at the 2018 BM workshop of the European Association of Haematopathology/Society of Hematopathology was therefore aggressive B-cell lymphomas with a pr

Mycosis Fungoides With Large Cell Lymphoma Transformation: A Report of a Rare Case.

BACKGROUND Mycosis fungoides, the most common cutaneous T-cell lymphoma, can transform into large cell lymphoma in some cases, leading to worse outcomes and reduced life expectancy. CASE REPORT We report the case of a 38-year-old Brazilian man with a history of multiple skin lesions refractory to medical treatment, requiring repeated hospitalizations, procedures, chemotherapy, phototherapy, and immunotherapy. The patient was followed by the same hospital's pathology and dermatology services for 17 years, from the diagnosis of mycosis fungoides to subsequent transformation to large cell lymphom

Selection of bispecific antibody therapies or CAR-T cell therapy in relapsed lymphomas.

Patients with relapsed and refractory (R/R) aggressive B-cell non-Hodgkin lymphomas have historically poor survival outcomes, with chimeric antigen receptor T-cell (CAR-T) therapy now presenting a curative option for a subset of those patients. However, with the approval of several novel bispecific monoclonal antibody (BsAb) therapies with considerable activity in R/R aggressive large B-cell lymphomas (LBCL), patients and oncologists will be faced with decisions regarding how to sequence CAR-T and BsAb therapies based on patient- and disease-related factors. In this review, we compare CAR-T an

Custom CARs: Leveraging the Adaptability of Allogeneic CAR Therapies to Address Current Challenges in Relapsed/Refractory DLBCL.

Cellular therapies have transformed the treatment of relapsed/refractory diffuse large B-cell lymphoma (r/r DLBCL), which typically does not respond well to salvage chemotherapy. Recently, approximately 40% of r/r DLBCL patients across three different trials achieved a complete remission at 1 year after receiving treatment with autologous chimeric antigen receptor (CAR) T cells (auto-CARs). These successes have prompted studies of auto-CARs in second-line settings, in which axicabtagene ciloleucel and lisocabtagene maraleucel both showed improved event-free survival over autologous hematopoiet

CRP and ferritin in addition to the EASIX score predict CAR-T-related toxicity.

The Endothelial Activation and Stress Index (EASIX) score, defined as [(creatinine × lactate dehydrogenase [LDH])/platelets], is a marker of endothelial activation that has been validated in the allogeneic hematopoietic stem cell transplant setting. Endothelial activation is one of the mechanisms driving immune-mediated toxicities in patients treated with chimeric antigen receptor-T (CAR-T)-cell therapy. This study's objective was to evaluate the association between EASIX and other laboratory parameters collected before lymphodepletion and the subsequent onset of cytokine release syndrome (CRS

[New treatment for patients with therapy-resistant lymphoma: CD19-targeted chimeric antigen receptor (CAR) T-cell therapy].

The prognosis of patients with diffuse large B-cell lymphoma a primary refractory disease or relapsed within 12 months after autologous hematopoietic cell transplantation is poor with a median survival of only 6 months. With the new CD19-directed CAR T-cell therapy, 40% of the patients still achieve a long-term remission. However, this new treatment does bring new challenges such as bridging the time during the CAR T-cell product time, and recognition of treatment-related side effects such as cytokine release syndrome or neurotoxicity. Therefore, treatment by a dedicated, multidisciplinary tea

[Richter's syndrome and acute transformation: two terminal haematological complications of chronic lymphoid leukaemia (author's transl)].

Two cases of CLL terminating in Richter's syndrome and in blastic crisis raise the problem of the physiopathological signification of these unusual complications. Richter's syndrome is described as a "histiocytic" lymphoma or a Hodgkin's disease supervening on a CLL. It does not seem that these two pictures have actually to be distinguished since clinical and haematological aspects are not different, and histopathologic studies show always atypical aspects with difficulty of an accurate diagnosis. Cases of blastic crisis of CLL are very uncommon. They correspond to the transformation of a CLL
