How Much Change Is Meaningful in Global Impression Anchors? Evidence From 5 Rare Diseases.

OBJECTIVES: Rare diseases pose methodological challenges in determining meaningful change thresholds for clinical outcome assessments (COAs) because of small sample sizes and disease heterogeneity. This article presents qualitative approaches for establishing patient-informed meaningful change thresholds on global impression anchor measures for rare diseases. METHODS: Two Food and Drug Administration-funded studies used novel cognitive debriefing methods to determine within-patient meaningful change on custom-developed anchors. B-cell repertoire sequencing reveals frequent rearrangements of IGHD5-5 in patients with systemic sclerosis.

Systemic sclerosis (SSc) is an autoimmune disorder marked by fibrosis of the skin and internal organs, with B cells increasingly recognized as key players in its pathogenesis. However, the characteristics of the B cell receptor (BCR) repertoire in SSc remain insufficiently defined. In this study, we performed high-throughput sequencing of immunoglobulin heavy chain genes in 15 female anti-centromere antibody (ACA)-positive SSc patients and five age-matched healthy female controls to explore disease-specific repertoire biases. A total of 2,597,460 in-frame sequence reads and 384,111 unique read

Machine Learning-Based Prediction of Lower Extremity Deep Vein Thrombosis in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease.

OBJECTIVE: Patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) have an increased risk of lower-extremity deep vein thrombosis (DVT) due to inflammation, immobility, and coagulation abnormalities. This study aimed to develop a machine learning model for predicting DVT risk in AECOPD patients and identify important predictors. METHODS: This single-center retrospective study included 1,500 patients with AECOPD who were classified into DVT (n = 126) and non-DVT groups (n = 1,374) according to lower-limb venous ultrasonography findings. Clinical characteristics and l

Blunted Cortisol Awakening Response in Patients with Inflammatory Bowel Disease: A Cross-Sectional Case-Control Study.

BACKGROUND: Inflammatory bowel disease (IBD) involves chronic immune activation, yet alterations in the cortisol awakening response (CAR) remain poorly defined. We compared cortisol dynamics in patients with IBD and healthy controls and explored associations with phenotype, endoscopic activity, and inflammation. METHODS: This single-centre, cross-sectional case-control study enrolled 100 patients with IBD (51 with ulcerative colitis [UC] and 49 with Crohn's disease [CD]) and 78 frequency-matched controls. Salivary cortisol was measured at awakening (C0), 30-45 min later (C1), and bedtime (C2)

Abundance and balance of circulating leukocyte subsets and colorectal cancer survival.

BACKGROUND: Cancer immunology research has traditionally focused on tumor-infiltrating leukocytes, but the role of the peripheral leukocytes remains understudied. Since tumor-infiltrating leukocytes are recruited from the blood, circulating immune profiles may provide prognostic insights obtainable before surgery, reflecting overall immune competence. METHODS: We analyzed treatment-naïve blood samples from 134 stage I-III colorectal cancer (CRC) patients using a nested case-control design (33 recurrences, 45 deaths; median follow-up: 7.4 years). Circulating leukocyte subsets were estimated us

Mapping the Mandibular Lingual Foramina for Safer Chin Surgery: CT Morphometry and Predictive Modeling.

BACKGROUND: Submental hematoma after genioplasty is rare but can be life-threatening, and current imaging data give limited patient-specific guidance for chin osteotomy design. METHODS: We combined targeted cadaveric dissection of two perfused heads with cranial CT analysis of 211 adults (70 long chin, 71 retrogenia, 70 controls; 621 lingual foramina). Standardized three-dimensional measurements described lingual foramen (LF) number, diameter, location, and canal trajectory, and their distances to tooth apices, genial spines, the mental foramen, and the mandibular border. Longitudinal retinal microvascular changes and their association with white matter hyperintensities in neuromyelitis optica spectrum disorder.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune astrocytopathy characterized by recurrent inflammatory attacks within the central nervous system. Beyond optic neuritis, diffuse retinal and cerebral alterations may occur. Optical coherence tomography angiography (OCTA) enables the non-invasive assessment of retinal microvasculature and may provide insight into longitudinal retinal changes and their association with cerebral white matter hyperintensities (WMHs). METHODS: In this longitudinal observational study, 45 aquaporin-4 antibody-positive NMOSD patients and 45

The prognostic value of skin temperature variability analysis in clinical settings: a systematic review of current evidence.

Objective. While core body temperature in humans remains relatively stable within a narrow range, peripheral skin temperature fluctuates dynamically. This variability reflects complex interactions between heat loss (e.g. via vasodilation) and heat gain (e.g. through metabolic activity), all modulated by an integrated thermoregulatory network. Emerging evidence suggests that skin temperature variability (STV) may offer insight into the integrity of thermoregulatory, vascular, and autonomic control systems. This systematic review evaluates the prognostic utility of STV as a dynamic physiological

Research progress on continuous nursing care for children after radical surgery for Hirschsprung's disease.

Hirschsprung's disease is a common congenital gastrointestinal malformation in children, for which radical surgery remains the primary treatment modality. Postoperatively, patients are prone to complications such as Hirschsprung-associated enterocolitis, defecation dysfunction, anal stenosis, and psychological or behavioral abnormalities, all of which significantly impair growth, development, and quality of life. As a critical bridge linking inpatient treatment with home-based rehabilitation, continuous nursing care has demonstrated efficacy in enhancing post-discharge outcomes, reducing compl

AI-Assisted Diagnosis of Trichomonas vaginalis from Routine Gram-Stained Vaginal Smears.

Background/Objectives: Trichomonas vaginalis is one of the most prevalent non-viral sexually transmitted infections worldwide. Although Gram staining is routinely performed in clinical microbiology laboratories for the evaluation of vaginal samples, it is not considered a diagnostic method for T. vaginalis, which represents a missed diagnostic opportunity in routine practice. This study aimed to evaluate an artificial intelligence (AI)-assisted diagnostic approach for the identification of T. vaginalis directly from routine Gram-stained vaginal smears. Methods: A retrospective dataset of Gram-

Information gain-based molecular ecological network reveals core microbial taxa involved in gastrointestinal recovery following H. pylori infection and intervention with Weizmannia coagulans BC99 in mice.

Helicobacter pylori infection is closely linked to gastric diseases such as ulcers and cancer. Recent studies have suggested that probiotics may help regulate H. pylori-induced dysbiosis. In this study, 25 female C57BL/6 mice were randomly assigned to five groups (n = 5 per group), including a control group, an H. pylori model group, and three BC99 intervention groups receiving low, medium, and high doses, after a 1-week acclimation period. The mice were infected for 1 week and then received BC99 intervention for 4 weeks, examines gastrointestinal microbiome dynamics from infection through rec

A novel approach for the EEG-driven assessment of divided attention through mutual information theory: A case study at the wheel.

Monitoring how attention is distributed across concurrent task demands is fundamental in daily life contexts such as mobility, education and industrial control. However, existing neurophysiological measures, typically based on univariate EEG markers, are sensitive to generic cognitive load, visual complexity, or motor activity, and therefore lack specificity for attentional splitting. Here, we introduce the Attentional Split Index (ASI), a novel EEG-based metric that employ Mutual Information (MI) theory, designed to quantify the coordinated modulation of multiple neurometrics that emerges whe

Near‑Infrared Photobiomodulation in White‑Matter Disease: From Microglial States to Measurable Endpoints.

White-matter (WM) injury contributes to disability across multiple sclerosis, traumatic brain injury, Alzheimer's disease and related dementias, and small-vessel disease. We use microglial state programs as an organizing axis for WM injury-to-repair logic, while emphasizing that WM outcomes are multicellular and involve oligodendrocyte-lineage cells, astrocytes, axons/neurons, and vascular factors. Microglia span an injury-repair continuum, from inflammatory programs that increase oxidative stress and debris burden to repair-competent programs that support debris handling, remyelination, and a
