Pericatheter leak in a peritoneal dialysis patient: SPECT/CT diagnosis.

A 62-year-old patient with end-stage renal disease who is managed with continuous ambulatory peritoneal dialysis developed anterior abdominal wall swelling and pain that extended from just above peritoneal catheter site caudally. CT of the abdomen suggested pericatheter abscess with panniculitis. Tc-99m sulfur colloid peritoneal distribution planar scintigraphy suggested the diagnoses of pericatheter leak. The addition of SPECT/CT confirmed the latter diagnosis and pinpointed the exact location of the peritoneal leakage that was around insertion site. SPECT/CT was useful in establishing unequi

Chemical and physiological relevance of glucose degradation products in peritoneal dialysis.

Fibrosis and vascular sclerosis are main complications that limit the long-term application of peritoneal dialysis (PD). Low biocompatibility has been largely attributed to the presence of glucose degradation products (GDPs), which are formed during the heat sterilization of PD fluids. GDPs readily modify proteins in the peritoneum, leading to a decline of their biological function. After absorption, GDPs can also promote systemic protein glycation. Additionally, GDPs may augment DNA glycation, a process enhanced in uremia. Apart from their glycating activity, GDPs induce cytotoxicity and inte

Impact of intraperitoneal pressure of a CO2 pneumoperitoneum on the surgical peritoneal environment.

BACKGROUND: Animal experiments have suggested that a high intraperitoneal pressure (IPP) might adversely affect the surgical peritoneal environment. The present experimental study investigates the impact of IPP of a CO(2) pneumoperitoneum on human peritoneum. METHODS: Patients undergoing laparoscopic surgery were subjected to either low (8 mmHg) or standard (12 mmHg) IPP. Normal peritoneum was collected from the parietal wall at the beginning of surgery and every 60 min thereafter. Expression levels of 168 genes that encode extracellular matrix proteins, adhesion molecules or inflammatory cyt

A two-hit approach in the development of an experimental peritoneal sclerosis model.

Models of encapsulating peritoneal sclerosis (EPS) are often based on local administration of chemical irritants. Our aim was to develop a clinically relevant "two-hit" model with incorporation of renal failure and exposure to conventional dialysis solutions. We randomly allocated 36 male Wistar rats that had undergone catheter implantation and a 70% nephrectomy to 3 peritoneal infusion groups. The experimental group was exposed to a 3.86% glucose-based conventional dialysis solution for 8 weeks, after which the animals received a second hit of intraperitoneal blood administration. Two weeks l

Phagocytosis of dying tumor cells by human peritoneal mesothelial cells.

Peritoneal carcinomatosis is an advanced form of metastatic disease characterized by cancer cell dissemination onto the peritoneum. It is commonly observed in ovarian and colorectal cancers and is associated with poor patient survival. Novel therapies consist of cytoreductive surgery in combination with intraperitoneal chemotherapy, aiming at tumor cell death induction. The resulting dying tumor cells are considered to be eliminated by professional as well as semi-professional phagocytes. In the present study, we have identified a hitherto unknown type of 'amateur' phagocyte in this environmen

[Current possibilities of cytological assay in the study of pleural and peritoneal exudates].

Serous exudates were studied by immunoperoxidase and immunofluorescence immunocytochemical assays and by laser flow cytofluorimetry. The current high-tech techniques detect sparse tumor complexes in the exudates at the early metastatic stages, which are undetectable by the routine cytological assay, and also reduce the hyperdiagnosis of the tumor process. The use of a set of immunocytochemical markers can specify the source of metastatic spread when the primary tumor focus is not found. Fluorescence immunocytochemistry of the exudates has been shown to be highly effective and rapid during emer

Splenic localization of the peritoneal mesothelioma: case report and literature revue.

Peritoneal mesothelioma is a rare form of malignant mesothelioma, making up <30% of diagnosed mesothelioma cases. Because of a lack of specific symptoms (abdominal pain, abdominal swelling), normally it is diagnosed in advanced stages, sometimes in a surgical emergency (intestinal obstruction, severe ascites) and occasionally during image procedures or laparoscopy which can show a mass developing from peritoneal mesothelium surfaces, or an accumulation of small irregularities that may be tumors or plaques. The reported case refers to a particular localization of a peritoneal mesothelioma, the

Energy intake and energy expenditure profiles in peritoneal dialysis patients.

Protein-energy wasting is a highly prevalent complication in patients with end-stage renal disease receiving peritoneal dialysis therapy, and it powerfully predicts clinical outcomes of these patients. The etiology is usually multifactorial. In this article, we aim to present the energy intake and energy expenditure profiles and review some of the important causes that have a negative effect on them in patients on peritoneal dialysis.

Laparoscopic management of early primary peritoneal pregnancy: a case report.

Peritoneal pregnancy is an implantation in the peritoneal cavity exclusive of tubal, ovarian, or intra-ligamentary implantations. This is a rare obstetric complication with high maternal mortality and even higher perinatal mortality, and secondary type was most common. Risk factors for peritoneal pregnancy are previous history of extrauterine pregnancy or tubal surgery pelvic post-inflammatory status or presence of an intra-uterine device. As it is a life-threatening condition, expectant management carries a risk of sudden life-threatening intra-abdominal bleeding and a generally poor fetal pr

An update on RAAS blockade and peritoneal membrane preservation: the ace of art.

Peritoneal membrane changes over time in long-term peritoneal dialysis (PD) patients lead to dialysis failure and increased morbidity as well as mortality. Bio-incompatable PD solution, peritonitis, and uremia are hypothesized in causing membrane damage. Fibrous organization and angiogenesis of peritoneum are crucial morphological alterations which can diminish the efficacy of exchange and cause ultrafiltration failure. Pathophysiologic mechanisms of membrane damage have been extensively studied to innovate therapeutic strategies. One of the potential mechanisms is a presence of local renin-an…

Peritoneal keratin granuloma associated with endometrioid adenocarcinoma of the uterine corpus.

We present a 69-year-old woman with a chief complaint of postmenopausal bleeding. She was diagnosed as having an endometrioid adenocarcinoma by biopsy, and underwent a total abdominal hysterectomy. At the time of surgery, granulation tissue-like nodules were found on the peritoneal serosa of the uterus. In the intraoperative cytology of peritoneal washing, atypical cells were noted. The intraoperative frozen section of the peritoneal nodule revealed granulation tissue with proliferating mesothelial cells. Microscopic examination of the permanent section showed keratin granulomas without viable

A rare case of benign multicystic peritoneal mesothelioma: a clinical dilemma.

Following the publication of the above article, an interested reader raised several comments on the study that required clarification on the part of the authors in order to confirm the reporting of certain of the results, and to make the paper fully comprehensible. First, in the Materials and methods section, the single‑cell RNA sequencing (RNA‑seq) source did not appear to match the presented groups for the scRNA‑seq data that were obtained from the GSE116222 dataset, specifically using 'three human UC samples', where three distinct groups, namely the 'Normal', 'Non‑inflamed' and 'Inflamed' g

Imaging ovarian cancer and peritoneal metastases--current and emerging techniques.

Peritoneal metastases are often the first presentation of ovarian malignancy. Evaluating the extent of disease critically determines tumor resectability and can also predict outcome. Standard CT, however, frequently fails to identify small sites of peritoneal spread. Moreover, it does not provide a quantitative index of disease response to cytotoxic therapy as it relies on macroscopic morphological changes in tumor volume, and does not reflect preceding molecular events in the microenvironment of the tumor. We describe the emerging role of functional imaging techniques, such as radioimmunoscin

Adrenocortical carcinoma: is the surgical approach a risk factor of peritoneal carcinomatosis?

CONTEXT: Peritoneal carcinomatosis (PC) is a rare site of distant metastases in patients with adrenocortical cancer (ACC). One preliminary study suggests an increased risk of PC after laparoscopic adrenalectomy (LA) for ACC. OBJECTIVE: The objective of the study was to search for risk factors of PC including surgical approach. DESIGN: This was a retrospective cohort study conducted in an institutional practice. PATIENTS: Sixty-four consecutive patients with ACC seen at our institution between 2003 and 2009 were included. Mean tumor size was 132 mm. Patients had stage I disease in 2 cases, s

A patient with refractory nephrotic syndrome withdrawn from peritoneal dialysis.

A 67-year-old woman was admitted to our hospital because of anasarca due to refractory nephrotic syndrome and chronic renal insufficiency. Laboratory data indicated serum total protein of 4.8 g/dl, albumin of 1.5 g/dl, creatinine of 1.9 mg/dl and BUN of 17 mg/dl. Urinary protein excretion was 7.8 g/day. Because of severe atrophy of both kidneys, neither renal biopsy nor immunosuppressive treatment was performed. Since conservative management including bed rest, diet therapy, limitation of water intake and administration of diuretics was not effective, peritoneal dialysis therapy using icodextr
