Smoking crushed hyoscine/scopolamine tablets as drug abuse.

BACKGROUND: Hyoscine N-butyl bromide/scopolamine (H/S) is a type of anticholinergic agent that is commonly used as an antispasmodic drug. We have evaluated the effects of crushed H/S smoking in prisoners who illicitly abused this drug. METHODS: All imprisoned cases with at least a 3-month history of HS were evaluated from April 2012 to September 2012. Demographic information, history, and clinical findings were studied. RESULTS: In total, 36 male cases with a mean (SD, min-max) age of 33.3 (3.6, 27-42) years were included. All subjects were cigarette smokers with a history of substance abuse

Adverse childhood experiences among Hawai'i adults: Findings from the 2010 Behavioral Risk Factor Survey.

The prevalence of adverse childhood experiences (ACEs) among Hawai'i adults and their impact on the health of affected individuals are unknown. Aiming to provide Hawai'i State baseline information on ACEs and their associations with health conditions and risk behaviors, the 2010 Hawai'i Behavioral Risk Factor Surveillance System (BRFSS) included the ACE module. Using 5,928 survey respondents who completed the module, demographic attributes were estimated and multivariate logistic regression analysis was performed to examine the association between ACEs and sixteen selected health indicators. Male imprisoned firesetters have different characteristics than other imprisoned offenders and require specialist treatment.

OBJECTIVE: This study investigated whether a group of firesetters (n = 68) could be distinguished, psychologically, from a matched group of non-firesetting offenders (n = 68). METHOD: Participants completed measures examining psychological variables relating to fire, emotional/self-regulation, social competency, self-concept, boredom proneness, and impression management. Official prison records were also examined to record offending history and other offense-related variables. A series of MANOVAs were conducted with conceptually related measures identified as the dependent variables. Follow-u

Trends of Zambia's tuberculosis burden over the past two decades.

OBJECTIVES: To study trends in Zambia's TB notification rates between 1990 and 2010 and to ascertain progress made towards TB control. METHODS: Retrospective review of TB notification returns and TB programme reports for the period from 1990 to 2010. RESULTS: Two distinct TB trend periods were identified: a period of rising trends up to a peak between 1990 and 2004 and a period of moderately declining trends between 2004 and 2010. Treatment outcomes improved over the two decades. Data on trends in paediatric TB, TB in prisoners and TB in pregnant women remain scanty and unreliable owing to p

[Guideline 'Medicinal care for drug addicts in penal institutions'].

In the Netherlands, the policy on care for prisoners who are addicted to opiates is still heterogeneous. The recent guidelines entitled 'Medicinal care for drug addicts in penal institutions' should contribute towards unambiguous and more evidence-based treatment for this group. In addition, it should improve and bring the care pathways within judicial institutions and mainstream healthcare more into line with one another. Each rational course of medicinal treatment will initially be continued in the penal institution. In penal institutions the help on offer is mainly focused on abstinence fro

Getting to equal: strategies to understand and eliminate general and orthopaedic healthcare disparities.

The 2001 Institute of Medicine report entitled Unequal Treatment: Confronting Racial and Ethnic Disparities in Health Care pointed out extensive healthcare disparities in the United States even when controlling for disease severity, socioeconomic status, education, and access. The literature identifies several groups of Americans who receive disparate healthcare: ethnic minorities, women, children, the elderly, the handicapped, the poor, prisoners, lesbians, gays, and the transgender population. Disparate healthcare represents an enormous current challenge with substantial moral, ethical, poli

Active case finding of tuberculosis in Europe: a Tuberculosis Network European Trials Group (TBNET) survey.

Tuberculosis control depends on successful case finding and treatment of individuals infected with Mycobacterium tuberculosis. Passive case finding is widely practised: the present study aims to ascertain the consensus and possible improvements in active case finding across Europe. Recommendations from national guidelines were collected from 50 countries of the World Health Organization European region using a standard questionnaire. Contacts are universally screened for active tuberculosis and latent tuberculosis infection (LTBI). Most countries (>70%) screen those with HIV infection, prisone

[History of the control of acute infectious diseases in Poland after the World War I--until the year 1924 (including big cities)].

Acute infectious diseases of high intensity, i.e. typhus fever, typhoid fever, dysentery, followed by scarlet fever, measles, malaria, relapsing fever, whooping cough, diphtheria, smallpox and Asiatic cholera spreading after the World War I in Poland posed one of the most significant problems in the reviving country. Their incidence resulted not only from bad living conditions of the population but also from poor personal and environmental hygiene and lack of access to bacteriologically healthy drinking water. The Polish-Bolshevik war (1919-1920) as well as repatriation of war prisoners and th

Risk factors and algorithms to identify hepatitis C, hepatitis B, and HIV among Georgian tuberculosis patients.

OBJECTIVES: To determine prevalence, risk factors, and simple identification algorithms for HIV, hepatitis B, and hepatitis C co-infection; factors that may predispose for anti-tuberculosis therapy-induced hepatotoxicity. METHODS: We recruited 300 individuals at in-patient tuberculosis hospitals in three cities in Georgia, administered a behavioral questionnaire, and tested for antibody to HIV, hepatitis C (HCV), hepatitis B core antigen (anti-HBc), and the hepatitis B surface antigen (HBsAg). RESULTS: Of the individuals tested, 0.7% were HIV positive, 4.3% were HBsAg positive, 8.7% were ant

Acute hepatitis C virus infection in incarcerated injection drug users.

BACKGROUND: The Centers for Disease Control and Prevention has emphasized the need for interventional programs regarding hepatitis C virus (HCV) infection for injection drug users, the group of persons who are at highest risk of acquiring acute infection. METHODS: We designed a pilot study to assess the feasibility of identifying injection drug users with acute HCV infection in correctional and detoxification facilities. On-site medical providers were educated regarding risk factors and signs and symptoms of infection and were instructed to refer all patients with hepatitis to our specialty c

Sexually transmitted infection and blood-borne virus screening in juvenile correctional facilities: a review of the literature and recommendations for Australian centres.

BACKGROUND: Juveniles in custody are disproportionately affected by sexually transmitted infections (STI) and blood-borne viruses (BBV) due to high rates of risk behaviours. METHODS: A literature review was undertaken with the aim of providing evidence-based recommendations on STI/BBV screening in Australian juvenile correctional facilities. Relevant research was identified using Premedline and Medline databases, followed by a manual search of reference lists in relevant articles identified in the database search. A total of 36 relevant publications were identified and reviewed. RESULTS: STI

Correlates of hepatitis C virus infection in homeless men: a latent variable approach.

Homeless individuals are at risk for numerous health problems including Hepatitis C virus (HCV). HCV is primarily caused by sharing of equipment associated with injection drug use (IDU). In the current study, we assessed differences among HCV-negative and HCV-positive homeless men residing in Los Angeles (N = 198; about 50% HCV positive) on a number of risk factors and behaviors. Findings revealed several significant correlates of HCV-positive status. HCV-positivity was significantly and positively associated with a history of substance use (IDU and non-IDU), recent IDU-related behaviors inclu

Possible association between serotonin transporter promoter region polymorphism and impulsivity in Koreans.

Serotonin has become the major focus of biological studies of suicidal behavior and impulsive-aggressive behavior in humans. The serotonin transporter (5-HTT) gene is one of the important genes involved in the regulation of serotonin transmission. We examined the association of impulsivity in Korean populations with a functional polymorphism of the promoter region of the 5-HTT gene (5-HTTLPR). We recruited 186 adolescent prisoners and 64 medical students. Impulsivity was measured using the Barratt Impulsiveness Scale and we divided all subjects into three groups: impulsive subjects (IS, N=121)

Impact of HIV vaccination on laboratory diagnosis: case reports.

BACKGROUND: It has not been clearly demonstrated whether HIV vaccination can complicate routine HIV testing. In this report, we describe the laboratory data of two prisoners who received rgp120 vaccine in a phase III trial underway in Thailand. These data indicate that previous vaccination may complicate the interpretation of screening HIV diagnostic tests. CASE PRESENTATION: The participants were identified from a cohort study on "Health factors related to HIV-1 and other viral infections among incarcerated people" that was approved by The Ethical Committee for Research in Human Subjects, Mi

Damon or Pandora?

Americans have recently had thrust into their faces multiple media borne medical and social ethics dilemmas ranging from Dr. Kevorkian's euthanizing a patient on national television to payments by managed care providers for experimental medical treatments, to the nationally telecast situation which this paper will attempt to address. The case at hand concerns a minor in need of a repeat (third) kidney transplant who has been offered a kidney by her father. He also provided a kidney for her second kidney transplant some years ago. The teenage girl went into her current kidney failure/organ reje

Rationing organs using psychosocial and lifestyle criteria.

The role of patient psychosocial and lifestyle characteristics in decisions about the allocation of scarce health care resources has not been examined. In this national survey using the Criteria for Selection of Transplant Recipient (CSTR) Scale, organ transplant coordinators (N = 559) identified the psychosocial and lifestyle criteria they believe should be considered in patient selection/rejection for organ transplant. Using factor analysis to reduce the data, six factors were identified: current lifestyle/psychiatric problems, family/socioeconomic issues, habits, controlled lifestyle/psychi

[The fight against dysentery, typhus and typhoid by Polish underground between 1941-1944].

Between 1941 and 1944 the Polish Home Army sanitary service was well prepared for the fight against infections diseases. The underground in so called General Government was both well organised and unimerous (nearly 350,000). The medical staff consisted of army doctors not taken prisoner, and civilians who as reserve officers were sworn to serve in the Home Army. Students from both elementary and secondary schools acted as undercover nurses having been prepared for the job by girl guides organisation. The main infections diseases were dysentery typhus and typhoid. Other diseases were rather rar
