Does instrumented fusion improve clinical outcomes in posterior surgery for cervical ossification of the posterior longitudinal ligament? A systematic review and meta-analysis.

BACKGROUND: Posterior decompression for cervical ossification of the posterior longitudinal ligament (OPLL) is commonly performed using laminoplasty (LAMP) or posterior decompression with fusion (PDF). However, the relative benefits and risks of these procedures remain uncertain. This study aimed to compare clinical and radiographic outcomes between LAMP and PDF using a systematic review and meta-analysis. METHODS: A systematic search of PubMed, Embase, and the Cochrane Library was conducted to identify studies comparing LAMP and PDF for cervical OPLL. Extracted data included neurological fun

Outcomes of an enhanced recovery after surgery (ERAS) program to limit perioperative opioid use in outpatient minimally invasive GI and hernia surgeries.

BACKGROUND: Perioperative pain management is important for patient satisfaction while returning to homeostasis in the safest way possible. Studies show that patients don't require as much opioids as once thought. The benefits of ERAS pathways extend beyond enhancement of patients' perioperative experience, and include reducing opioid prescriptions in the face of the ongoing nationwide opioid crisis and evidence of prescription opioids as a contributor. METHODS: We performed a retrospective cohort study of patients undergoing same day minimally invasive surgery (MIS) procedures for GI and hern

Is Kidner procedure necessary during subtalar arthroereisis for pediatric flexible flatfoot that combined with symptomatic type 2 accessory navicular? A retrospective comparative study.

BACKGROUND: Kidner procedure is thought to be able to eliminate the medial foot pain and contribute to restoring the medial longitudinal foot arch, making it particularly suitable for surgical treatment of pes planus that combined with symptomatic type 2 accessory navicular (AN). However, controversy remains, and the clinical evidence is still lacking. The aim of the current study is to verify the necessity of Kidner procedure during subtalar arthroereisis (STA) for pediatric flexible flatfoot (PFF) that combined with symptomatic type 2 AN. METHODS: Forty pediatric patients (72 feet) who had

Comparison of anterior palatoplasty and uvulopalatal flap placement for treating mild and moderate obstructive sleep apnea.

We prospectively compared the efficacy of anterior palatoplasty and the uvulopalatal flap procedure for the treatment of patients with mild and moderate obstructive sleep apnea syndrome (OSAS). Our study group was made up of 45 patients who had been randomly assigned to undergo one of the two procedures. Palatoplasty was performed on 22 patients-12 men and 10 women, aged 28 to 49 years (mean: 39.2)-and the flap procedure was performed on 23 patients-14 men and 9 women, aged 28 to 56 years (mean: 41.3). Our primary outcomes measure was the difference in pre- and postoperative apnea-hypopnea ind

Inguinal hernia repair in day surgery: the role of MAC (Monitored Anesthesia Care) with remifentanil.

BACKGROUND: The extension of indications for procedures in a Day Surgery (DS) setting has led to changes in the anesthetic and surgical treatment of Inguinal Hernias (IH). According to the recommendations of the European Hernia Society, the treatment of IH in DS units should be performed under Monitored Anesthesia Care (MAC). PATIENTS AND METHODS: 960 patients underwent IH repairs over a period of 24 months. The patients were randomly divided into two groups: R (remifentanil) and F (fentanyl); the group F was considered as a control group. The exclusion criteria in both group were: morbid obe

Surgery for advanced and metastatic pancreatic cancer--current state and perspectives.

Advanced disease, defined as vascular invasion or invasion into adjacent organs, in pancreatic ductal adenocarcinoma still remains a major diagnostic and therapeutic challenge. In most cases, only exploratory laparotomy will ultimately ensure surgical resectibility. A physician is ill-advised to make any decision regarding palliation relying on CT-scan, MRI, ultrasonography or angiography, since vascular invasion is difficult to diagnose because of peritumoral pancreatitis mimicking vascular invasion. Only in the case of complete vascular encasement of the mesenterico-portal axis or celiac tru

[Functional results after proximal row carpectomy (PRC) in patients with SNAC-/SLAC-wrist stage II].

The proximal row carpectomy (PRC) is a motion preserving procedure which creates a new joint without arthrosis. It is a frequently used procedure in stage II of a posttraumatic degenerative arthrosis of the wrist after scaphoid nonunion or scapholunate ligament instability (SNAC-/SLAC-wrist). In this retrospective analysis the functional postoperative results of this operation are compared in light of a homogenous indication (SNAC-/SLAC-wrist stage II). In 38 patients PRC was performed for a stage II SNAC- (n = 29) or SLAC-wrist (n = 9) between June 1994 and March 2002. Postoperative examinati

Cervical laminaplasty: its role in the treatment of cervical radiculopathy.

Krita in 1968 described the use of laminaplasty for the treatment of cervical myelopathy. Since then, several authors have modified this technique, settling on the "expansive open door laminaplasty" as the technique of choice for cervical myelopathy. There have been no reports to date on the use of the cervical laminaplasty procedure for the treatment of cervical radiculopathy. The purpose of this paper is to report on the initial 16 patients undergoing this procedure for the surgical treatment of cervical radiculopathy due to cervical spondylosis and/or cervical spinal stenosis. There were 16

Spontaneous Contralateral Adrenal Hemorrhage during Periduodenal Abscess Drain Placement.

A spontaneous adrenal hemorrhage is a rare complication occurring in the setting of interventional radiology procedures. Here, we present the case of a 73-year-old male who underwent CT-guided drainage of a periduodenal collection. During the procedure, he developed sudden onset left back pain and hypertension, which revealed a left adrenal gland hemorrhage on CT imaging. Potential mechanisms for this complication include a physiological stress response, procedure-associated hypercoagulability, and direct trauma to the adrenal gland. Moreover, an adrenal hemorrhage should be monitored closely

Hindfoot endoscopy for posterior ankle impingement syndrome and flexor hallucis longus tendon disorders.

Hindfoot endoscopic surgery is an alternative to conventional open surgery for treatment of posterior ankle pain. This procedure can be applied not only for accurate diagnosis under direct visualization but also for low-invasive therapy. Common indications for hindfoot endoscopy are posterior ankle impingement syndrome and damaged soft tissue. Several studies have reported good clinical outcomes of hindfoot endoscopy with lower complication rates than in the conventional open procedure. Nerve injury remains a common complication. To avoid such injury, make a posterolateral portal just lateral

Intraoperative Bier's block as supplement to insufficient axillary block anesthesia in upper extremity surgery.

The authors reviewed 52 patients who underwent Bier's block, as supplementary anesthesia for insufficient axillary block in upper extremity surgical procedures. Prior to proceeding to the Bier's block, the mean value of pain using the visual optical analogue scale (VAS) was 7.0. In 48 of the patients supplementation with the Bier's block was sufficient (mean VAS score of 1.0) and all patients were comfortable throughout the procedure. In the remaining four patients supplementation with narcotics and sedatives via the other i.v. line was required. Intraoperative Bier's block provides a safe and

Non-surgical interventional treatment of cervical and thoracic radiculopathies.

Cervical and thoracic radiculopathies are among the most common causes of neck pain. Outcome studies of conservative treatments have shown varying results and have not been well controlled or systematic. Although there has not yet been a non-surgical interventional procedure developed with the therapeutic efficacy of open surgery, conservative procedures can offer substantial benefits, are less invasive, and avoid surgical complications. While more invasive procedures may be appropriate when conservative treatment fails, prospective studies evaluating cervical and thoracic radiculopathy treatm

Surgical treatment of unresolved Osgood-Schlatter lesion.

The Osgood-Schlatter lesion is considered a traction apophysitis. Treatment is generally conservative, and only rarely does a surgical treatment become necessary for the persistence of pain and swelling over the tibial tubercle. The most widely used procedure consists of the excision of all intratendinous ossicles with or without removal of the prominent tibial tubercle. In 15 cases treated in this manner compared with 11 cases treated with various procedures before 1975, the results were clearly better with the former. There was only one fair and no poor results. Removal of all loose intraten

Patellofemoral arthritis resulting from malalignment. A long-term evaluation of treatment options.

This study examines and compares three treatment options for patellofemoral arthritis resulting from patellofemoral malalignment: longitudinal semipatellectomy, total patellectomy, and a modified Trillat procedure. Patients returned for examination in a 3- to 12-year follow-up period. Results from longitudinal semipatellectomy were unsatisfactory, with a reoperation and failure rate of 70%. Good long-term effects were found with total patellectomy (87% good or excellent). The modified Trillat procedure was effective in preventing recurrent dislocation (94%); however, the procedure was less eff

Consolidation rate in ulnar shortening osteotomy with the APTUS Wrist plate.

INTRODUCTION: Ulnocarpal impaction syndrome causes pain on the ulnar side of the wrist. Various surgical techniques have been described. Ulnar shortening osteotomy is now a standard treatment. However, it is associated with complications such as non-union of the osteotomy site. The main study objective was to report the rate of radiographic consolidation after ulnar shortening osteotomy with a cutting guide. MATERIAL AND METHODS: This multicenter retrospective study of 30 cases reported clinical and radiographic criteria at a minimum 6 month's follow-up. RESULTS: The non-union rate was 3.4%.

Pyrocarbon interposition implant after lunate resection in Kienböck's disease: A case series.

In advanced stages of Kienböck's disease, the lunate is no longer conservable. One of the surgical options is to resect the lunate and replace it with a prosthesis. The procedure consisted in lunate resection and interposition of a free APSI® or Pi2® pyrocarbon implant through a dorsal approach. Follow-up was clinical and radiological on QuickDASH and PRWE scores. At a median follow-up of 3 years, 12 patients were reviewed, with a median age of 56 years. Flexion significantly decreased from 42° to 28° (p < 0.01). Extension and pronation-supination were conserved. Strength was 94% compared to t
