The Pathology of the Vestibular System in CANVAS.

OBJECTIVE: To describe the site of lesion responsible for the severe, bilateral, symmetrical, selective loss of vestibular function in Cerebellar Ataxia with Neuronopathy and Vestibular Areflexia Syndrome (CANVAS), an adult-onset recessively-inherited ataxia, characterized by progressive imbalance due to a combination of cerebellar, somatosensory, and selective vestibular impairment with normal hearing. METHODS: Histologic examination of five temporal bones and the brainstems from four CANVAS patients and the brainstem only from one more, each diagnosed and followed from diagnosis to death by…

Functional assessment of stretch hyperreflexia in children with cerebral palsy using treadmill perturbations.

BACKGROUND: As hyperactive muscle stretch reflexes hinder movement in patients with central nervous system disorders, they are a common target of treatment. To improve treatment evaluation, hyperactive reflexes should be assessed during activities as walking rather than passively. This study systematically explores the feasibility, reliability and validity of sudden treadmill perturbations to evoke and quantify calf muscle stretch reflexes during walking in children with neurological disorders. METHODS: We performed an observational cross-sectional study including 24 children with cerebral pa…

Low startle magnitude may be a behavioral marker of vulnerability to cocaine addiction.

Cocaine addicted men have low startle magnitude persisting during prolonged abstinence. Low startle rats show greater cocaine self-administration than high startle rats. Low startle may be a marker of a vulnerability to heightened cocaine-related behaviors in rats and similarly may be a marker of vulnerability to cocaine addiction in humans.

Neonatal hyperekplexia: the Stiff-Baby syndrome.

Neonatal hyperekplexia is a rare autosomal dominant startle disorder. Presenting soon after birth, it is often mistakenly diagnosed as spastic quadriparesis, epilepsy etc. While the long-term prognosis is relatively benign, sudden death due to severe spasms have been seen in sporadic cases. We report a case of hyperekplexia with some typical and some unusual findings.

Familial congenital vestibular areflexia.

Three cases in one family are presented with oscillopsia due to vestibular areflexia, but without hearing loss. There was no history of other neurological or otological diseases (including infectious diseases) or use of neuro-ototoxic drugs. Laboratory tests, including tests for autoimmune diseases, were undisturbed. Petrosal radiographs and brain CT scans were normal. The pedigree suggested autosomal recessive inheritance.

[Primary localization of the pathologic process in amyotrophic lateral sclerosis].

The clinico-electromyographical study was conducted in 120 patients with motor disturbances in amyotrophical lateral sclerosis. Comparing the clinical and electromyographical data in different variants of this disease the authors make a conclusion on the simultaneous damage of peripheral and central mononeurons from the very onset of the disease. Along with this the latter is damaged not only primarily, but secondarily as a result of trans-synaptic degerneration due to pathology of the segmental-nucleus motor cells.
