Upper extremity injuries in the adolescent athlete.

There has been a significant increase in youth sports participation and athletic activities over the past 3 decades. With the increase in participation, there has been a commensurate rise in the number of sports-related injuries. A majority of these injuries are due to overuse as athletes frequently compete in multiple sports with year round competition and training. As higher demands are placed on these young athletes, the likelihood of injury during and individual's playing career continues to increase. This review will focus on both overuse and traumatic injuries of the upper extremity in t

Lymphoma masquerading as infection.

During surgical training, medical students and residents constantly are reminded to culture every suspected tumor and send tissue for pathologic evaluation for every suspected abscess. A diagnosis of cancer can be missed easily if this procedure is not followed, delaying the diagnosis and possibly adversely affecting the patient's prognosis. The confusion also may be compounded by a sterile abscess, positive culture results or a negative biopsy specimen. Therefore it is imperative to do a biopsy and a culture on any suspect lesion. An additional workup and possible biopsy may be warranted for

Long term results after bipolar radial head arthroplasty.

Radial head fractures are common injuries, occurring mostly in active young people 85%. Treatment of the more complex radial head fractures (Mason type III and IV), appear to be challenging and remain a subject of controversy. Replacement of the radial head with a metal prosthesis imitates the stabilizing role of the radial head and has been considered as the treatment of choice. However, long-term results are scarce. We report our mid to long-term (mean 9.4 year) clinical and radiographic results after insertion of a Judet Bipolar Radial Head prosthesis. 34 patients were treated with the Jude

Differing perceptions of intervention thresholds for fracture risk: a survey of patients and doctors.

UNLABELLED: This survey suggests that patients are prepared to accept higher absolute fracture risk than doctors, before considering pharmacological therapy to be justified. Patients require that drug treatments confer substantial fracture risk reductions in order to consider long-term therapy. INTRODUCTION: Absolute fracture risk estimates are now incorporated into osteoporosis treatment guidelines. At present, little is known about how patients regard fracture risk and its management. We set out to describe and compare the views of patients and doctors on the level of fracture risk at which

Isolated radial neck delayed union/nonunion after conservative treatment in adults: two case reports and a literature review.

BACKGROUND: Treatment recommendations for isolated radial neck delayed union/nonunion are sparse, but include mainly conservative treatment, electro-stimulation, bone grafting, open reduction and internal fixation (ORIF) and radial head resection. The purpose of this study was to perform a literature review on isolated delayed union/nonunion of radial neck fractures and to evaluate the reported outcomes of proposed treatment strategies. Furthermore, we aimed to generate data-based recommendations for this rare pathology. In the second part of this paper, we report two clinical cases of delayed

Comparison of distal radius fracture plating surgery under wide-awake local anesthesia no tourniquet technique and balanced anesthesia: a retrospective cohort study.

BACKGROUND: Distal radius fractures (DRF) are frequently treated with internal fixation under general anesthesia or a brachial plexus block. Recently, the wide-awake local anesthesia with no tourniquet (WALANT) technique has been suggested as a method that results in higher patient satisfaction. This study aimed to evaluate the functional outcomes, complications, and patient-reported outcomes of DRF plating surgery under both the WALANT and balanced anesthesia (BA). METHODS: Ninety-three patients with DRFs who underwent open reduction and plating were included. Regarding the anesthetic techni

Imaging Injuries in Throwing Sports Beyond the Typical Shoulder and Elbow Pathologies.

This review article describes injuries that occur in the upper extremities of athletes less commonly than those typically discussed with shoulders and elbows. A survey of osseous, musculotendinous, ligamentous, and neurovascular injuries is presented along with associated imaging findings and standard treatment options. This article does not focus on the classic throwing injuries of the shoulder or elbow; the goal is to survey injuries in throwing sports that involve structures away from the glenohumeral, acromioclavicular, or elbow joints. The goal of this article is to introduce readers to t

Occupant kinematics and shoulder belt retention in far-side lateral and oblique collisions: a parametric study.

In far-side impacts, head contact with interior components is a key injury mechanism. Restraint characteristics have a pronounced influence on head motion and injury risk. This study performed a parametric examination of restraint, positioning, and collision factors affecting shoulder belt retention and occupant kinematics in far-side lateral and oblique sled tests with post mortem human subjects (PMHS). Seven PMHS were subjected to repeated tests varying the D-ring position, arm position, pelvis restraint, pre-tensioning, and impact severity. Each PMHS was subjected to four low-severity tests

[What information do we need to treat the injured shoulder of a hockey player?].

Successful patient care of the injured shoulder of a hockey player involves interdisciplinary cupertino. Images allow an interpretation of a static dimension but may not be sufficient enough to develop a surgical treatment strategy, since all injuries to the shoulder have its origin in a dynamic pathology with some kind of external impact. This article classifies various traumatic shoulder problems and the mechanism of injury, it outlines clinical tests and stresses key facts an imaging report should include in order to plan conservative or surgical treatment. Basic treatment options as perfor

The neurovascular outcome of scapulothoracic dissociation.

PURPOSE: The purpose of this study was to define the role of acute revascularization in the management of scapulothoracic dissociation (SD), a rare injury of the upper extremity in which severe traction to the shoulder girdle results in combined arterial, brachial plexus, and musculoskeletal injury. METHODS: The results of a retrospective study of 11 patients with SD who were compared with 41 patients from the literature are presented. RESULTS: All 11 patients were involved in high-speed accidents. All patients had chest wall hematoma, absent radial pulse, and complete brachial plexus palsy.

Skiing injuries in children and adults: a comparative study from an 8-year period.

The investigation is a retrospective analysis of 2756 skiing injuries from two centers in a Swedish skiing area during the years 1972 to, and including, 1979. The relative frequency of lower extremity injuries successively diminished, while a corresponding increase of upper extremity injuries was found. The relative frequency of ankle and foot injuries declined, while the frequency of knee injuries did not change, knee injuries still being the most common skiing injury. The relative frequency of fractures of the lower leg showed a statistically significant decrease, but, in children, constitut

Social determinants of health disparities are associated with lower total shoulder arthroplasty utilization and increased medical complications: A propensity-matched analysis.

BACKGROUND: Total shoulder arthroplasty (TSA) is an effective treatment for glenohumeral osteoarthritis (GHOA), with utilization increasing over time. While prior studies have examined racial and ethnic disparities, the impact of broader social determinants of health (SDOH) on TSA utilization and outcomes remains unclear. METHODS: The TriNetX database was used to identify patients with GHOA and stratify them by SDOH disparities. Patients were matched 1:1 using propensity scores. The primary outcome was TSA utilization within 5 years. Secondary outcomes included 90-day and 1-year postoperative

Antegrade intramedullary nail and plate combination technique for proximal humeral non-union: A case report.

Proximal humeral nonunion remains a challenging condition, particularly after failed plate fixation, because of the compromised bone stock within the humeral head and loss of medial calcar support. Under these conditions, it is often difficult to achieve sufficient mechanical stability while preserving the humeral head. We report a case of proximal humeral nonunion successfully treated using the nail and plate combination technique. A 61-year-old man sustained a high-energy proximal humeral fracture in a motorcycle accident and underwent initial plate fixation at a different institution. The p

Outcomes and Complications of Total Joint Arthroplasty in Heart Transplant Recipients: A Systematic Review.

Background: Heart transplantation has evolved into a life-saving intervention for thousands of patients with end-stage disease worldwide. Advances in surgical techniques and immunotherapy have increased life expectancy in this specific patient group, resulting in the emergence of degenerative diseases such as osteoarthritis. Concurrently, due to the frequent administration of corticosteroids to transplant recipients, the rates of joint osteonecrosis remain high. This review aims to evaluate the final functional outcomes as well as the complications of patients with a history of heart transplan

Human use conditions in fatigue of intravenous leads used in cardiac implanted electronic devices.

BACKGROUND: Transvenous lead conductors are subject to fatigue fracture from repetitive bending stresses, which are proportional to alternating curvature. A prerequisite for developing a valid preclinical lead-testing standard is knowledge of the stresses on implanted leads - their fatigue environment, but little is known about it. OBJECTIVE: The Human Use Condition Study (HUCS) quantified this fatigue environment, including the relationship between lead stiffness and alternating curvature. METHODS: HUCS was a prospective, observational, multicenter clinical study. Leads from 4 manufacturers
