Neck pain: common complaint, uncommon diagnosis--symptomatic clival chordoma.

Patients presenting with neck complaints, such as pain or stiffness, are not uncommon in the Emergency Department. Complaints of neck instability, however, are unusual. We report the case of a 30-year-old woman who presented with multiple neck complaints that included having a "wobbly" sensation of her neck on flexion, feeling as if it were unstable. Our patient indeed had atlanto-occipital instability secondary to a locally destructive tumor of the cranial base, known as a clival chordoma. Chordomas are rare and unique bony tumors that arise along the neural axis and are thought to originate

Foramen jugulare: a local investigation and a review of the literature.

The jugular foramen varies in size and shape. Because of the variations and the surrounding structures, successful surgery is difficult in this region. For this reason, numerous studies have investigate the detailed anatomy of the foramen. The purpose of the present study is to investigate the possible local variations of the foramen jugulare. In this work, Eastern Anatolian skulls were examined in order to investigate the local variety. Extracranial opening of the foramen was only measured in diameters as maximum anteroposterior (AP) and mediolateral diameter (ML) of both right and left foram

SynthStrip: skull-stripping for any brain image.

The removal of non-brain signal from magnetic resonance imaging (MRI) data, known as skull-stripping, is an integral component of many neuroimage analysis streams. Despite their abundance, popular classical skull-stripping methods are usually tailored to images with specific acquisition properties, namely near-isotropic resolution and T1-weighted (T1w) MRI contrast, which are prevalent in research settings. As a result, existing tools tend to adapt poorly to other image types, such as stacks of thick slices acquired with fast spin-echo (FSE) MRI that are common in the clinic. While learning-ba

Suicide by the intraoral blast of firecrackers - experimental simulation using a skull simulant model.

Suicides committed by intraorally placed firecrackers are rare events. Given to the use of more powerful components such as flash powder recently, some firecrackers may cause massive life-threatening injuries in case of such misuse. Innocuous black powder firecrackers are subject to national explosives legislation and only have the potential to cause harmless injuries restricted to the soft tissue. We here report two cases of suicide committed by an intraoral placement of firecrackers, resulting in similar patterns of skull injury. As it was first unknown whether black powder firecrackers can

Extended transsphenoidal approach for pituitary adenomas invading the anterior cranial base, cavernous sinus, and clivus: a single-center experience with 126 consecutive cases.

OBJECT: The standard transsphenoidal approach has been successfully used to resect most pituitary adenomas. However, as a result of the limited exposure provided by this procedure, complete surgical removal of pituitary adenomas with parasellar or retrosellar extension remains problematic. By additional bone removal of the cranial base, the extended transsphenoidal approach provides better exposure to the parasellar and clival region compared with the standard approach. The authors describe their surgical experience with the extended transsphenoidal approach to remove pituitary adenomas invadi

Endoscopic repair of high-flow cranial base defects using a bilayer button.

OBJECTIVES/HYPOTHESIS: Repair of the skull base still begins with a direct repair of the dural defect. We present a new button closure for primary repair of the dura for high flow defects. STUDY DESIGN: Retrospective review. METHODS: We reviewed our 20 cases of primary button grafts and compared the results to the previous 20 high-flow open-cistern cerebrospinal fluid (CSF) cases. Subjects were excluded if they had no violation of the arachnoid space or potential for low-flow CSF leak. The button is constructed so that the inlay portion is at least 25% larger than the dural defect, and the o…

Management Strategies of Cranial Encephaloceles: A Neurosurgical Challenge.

BACKGROUND: Encephalocele is defined as herniation of cranial contents beyond the normal confines of the skull through a defect in the calvarium either along the midline or at the base of skull. These anomalies should be repaired in the first few months of life to prevent neurological deficits and facial disfigurement. The aim of the surgery is water tight dural closure at the level of internal defect, closure of skull defect, and reconstruction of external bony deformity. MATERIALS AND METHODS: Fifty-four cases of encephalocoeles were studied in our hospital over a 6-year period from 2010 to

Skull morphometry and sexual dimorphism in the Kyrgyz horse: A multivariate size-corrected analysis.

This study aimed to evaluate sexual dimorphism in Kyrgyz horse skulls by separating size and shape components using Mosimann log-shape ratios, based on 18 linear cranial measurements recorded from 39 specimens. Raw linear data were analyzed alongside size-corrected Mosimann log-shape ratios to distinguish overall size from size-independent shape. Males showed larger raw means than females for all 18 variables, and 14 measurements remained significant after false-discovery-rate correction. The strongest raw differences involved skull length to nasale, total skull length, nasal length, breadth b

Machine learning for prediction of schizophrenia based on identifying the primary and interaction effects of minor physical anomalies.

In the neurodevelopmental model of schizophrenia, minor physical anomalies (MPAs) are considered neurodevelopmental markers of schizophrenia. To date, there has been no research to evaluate the interaction between MPAs. Our study built and used a machine learning model to predict the risk of schizophrenia based on measurements of MPA items and to investigate the potential primary and interaction effects of MPAs. The study included 470 patients with schizophrenia and 354 healthy controls. The models used are classical statistical model, Logistic Regression (LR), and machine leaning models, Deci

Rigid fixation in reconstruction of craniofacial fractures.

Ninety-four patients had either isolated fractures of the skull or midface, or combined fractures of the skull, periorbit, and/or midface. Thirty-five of these patients were treated by conventional methods, including maxillomandibular fixation (MMF) 4 to 6 weeks postoperatively, wire osteosynthesis, suspension ligatures, or a head frame. The remaining 59 patients were treated with either mini-, low-profile, micro-, or 3-D titanium plate fixation (rigid internal fixation [RIF]) and comprise the patient population for this study. Of the 59 patients, 11 were female and 48 male, ranging in age fro

Levine-Sekhar grading system for prediction of the extent of resection of cranial base meningiomas revisited: study of 124 cases.

INTRODUCTION: Skull base meningiomas comprise an intricate kingdom in neurological surgery. Due to their proximity to critical neurovascular structures, these tumours impose a cumbersome burden on the surgeon regarding surgical intervention and the clinical outcome. Preoperative prediction of the meningioma resectability will help the surgeon seek a rational result from surgery. This study tries to re-examine and promote the Levine-Sekhar (LS) grading system proposed to predict the resectability of basal meningiomas. PATIENTS AND METHODS: A retrospective study was performed on 124 eligible pa…

Impact of adjuvant radiation therapy on outcomes in grade 2 conventional chondrosarcoma.

OBJECTIVE: While histological subtype and grade are important for guiding treatment of skull base chondrosarcomas, the role of adjuvant radiation therapy (RT) and the optimal radiation modality for grade 2 conventional chondrosarcoma (cCS) are not well defined. The aim of this study was to further elucidate optimal treatment strategies for grade 2 cCS. METHODS: Multi-institutional data from the Skull Base Chondrosarcoma Consortium, including patient demographics, radiological features, and outcomes, were analyzed retrospectively. Clinical and radiological follow-up data were used to assess di…

Distinct genomic subclasses of high-grade/progressive meningiomas: NF2-associated, NF2-exclusive, and NF2-agnostic.

BACKGROUND: Genomic studies of high-grade/progressive meningiomas have reported a heterogeneous mutation spectrum, identifying few recurrently mutated genes. Most studies have been underpowered to detect genomic subclasses of aggressive meningiomas due to relatively small number of available samples. Here, we present a genomic survey of one of the largest multi-institutional cohorts of high-grade/progressive meningiomas to date. METHODS: 850 high-grade/progressive meningiomas, including 441 WHO grade 2 and 176 WHO grade 3 meningiomas and 220 progressive WHO grade 1 meningiomas, were tested as

Pattern of local relapse of maxillary sinus carcinoma.

Maxillary sinus carcinoma (MSC) is a rare disease with a variety of treatment options. The present study was undertaken to review the outcome of patients with treated MSC in order to clarify the factors related to local recurrence by analyzing CT findings. The study group comprised of 47 cases, 40 males and 7 females with a median age of 61 years (range, 40- 84 years) treated between 1988 to 1996 at the department of radiotherapy. CT was taken with a slice thickness of 5 mm and contrast material was routinely used. The mean follow-up period for the group was 45.0 months (range, 3-125 months).

The effectiveness and factors related to treatment results of gamma knife radiosurgery for meningiomas.

A retrospective analysis was conducted on 80 patients with intracranial mengiomas treated with Gamma Knife radiosurgery between 1993 and 1996. The purpose was to analyze the efficacy of the treatment and to assess appropriate treatment parameters. The results were assessed by regular MR examinations, and tumor volume was measured at 6-month intervals. Mean follow-up duration was 21 months (range 6-45 months). 63 meningiomas were at the skull base and 17 were distal from the skull base. Tumor volumes <5 ml (n=38), 5-10 ml (n=21), 10-15 ml (n=14), 15-20 ml (n=7). The patients were divided into 3…
