Associations between clinical or polysomnographic features and the risk of continuous positive airway pressure therapy termination in patients with obstructive sleep apnea: a 5-year cohort study.

BACKGROUND: Continuous positive airway pressure (CPAP) is the first-line treatment for obstructive sleep apnea (OSA). Despite CPAP recognized benefits, the 3-year CPAP termination rate in France is 47.7 %, with sex, age, and comorbidities identified as significant predictors of therapy termination. The contribution of symptoms, OSA severity and objective sleep parameters to CPAP termination remains to be investigated. METHODS: This single-center study used real-life data from a tertiary university for patients diagnosed with OSA and started on CPAP. Data were prospectively collected and linke

Effects of Continuous Positive Airway Pressure on Central and Peripheral Blood Pressure in Patients with Uncontrolled Hypertension and Obstructive Sleep Apnea: The Randomized Controlled MORPHEOS Clinical Trial.

Rationale: Previous studies evaluating the effect of continuous positive airway pressure (CPAP) on blood pressure (BP) among patients with obstructive sleep apnea (OSA) showed variable results. Moreover, several studies recruited patients with normal or controlled BP, and compliance with antihypertensive drug treatment was not monitored. In addition, very few studies investigated central BP in this scenario. Objectives: To evaluate whether OSA treatment is able to reduce central and peripheral BP in patients with uncontrolled hypertension (HTN) despite well-documented use of antihypertensive t

Efficacy of continuous positive airway pressure on cardiac remodeling and ventricular function in obstructive sleep apnea: a systematic review and updated meta-analysis of speckle-tracking echocardiography.

Obstructive sleep apnea (OSA) is associated with structural heart disease, with continuous positive airway pressure (CPAP) potentially improving cardiac remodeling. This meta-analysis provides an updated assessment of CPAP's effects on myocardial strain and remodeling in patients with OSA. A systematic review and meta-analysis of clinical studies evaluating CPAP's effects on speckle-tracking echocardiographic parameters in OSA patients was conducted. PubMed, Embase and Cochrane Central were searched. A random-effects model analyzed pooled data. Studies assessing right ventricular global longit

TRIM24 Up-Regulates ORM2 to Alleviate Abnormal Lipid Metabolism, Inflammation, and Oxidative Stress in Mice with Obstructive Sleep Apnea Syndrome and Metabolic Dysfunction-Associated Steatotic Liver Disease.

Obstructive sleep apnea syndrome (OSAS) is associated with the development and progression of metabolic dysfunction-associated steatotic liver disease (MASLD). Tripartite motif containing 24 (TRIM24) deficiency causes hepatic lipid accumulation and hepatitis. However, the expression, function, and mechanism of TRIM24 in OSAS and MASLD remain unclear. Herein, an OSAS and MASLD mouse model was established by intermittent hypoxia (IH) and high-fat diet. IH- and 1% free fatty acid-induced mouse liver cells served as an in vitro model. TRIM24 and HIF1A were up-regulated under the IH condition. HIF1

"The terrible dryness woke me up, I had some trouble breathing"-Critical situations related to oral health as described by CPAP-treated persons with obstructive sleep apnea.

Continuous positive airway pressure is a common and effective treatment for obstructive sleep apnea, but adherence remains an issue. Both obstructive sleep apnea and oral diseases are associated with cardiovascular diseases, and as oral dryness contributes to treatment abandonment, oral health is of importance for this patient group. The aim was therefore to explore how persons with continuous positive airway pressure-treated obstructive sleep apnea experience situations associated with their oral health, and which actions they take to manage these. An explorative and descriptive design was ad

The effect of continuous positive airway pressure therapy on arterial stiffness and endothelial function in obstructive sleep apnea: a randomized controlled trial in patients without cardiovascular disease.

BACKGROUND: Obstructive sleep apnea (OSA) is associated with increased cardiovascular morbidity and mortality which may be mediated by increased arterial stiffness and endothelial dysfunction. Continuous positive airway pressure (CPAP) therapy improves excessive daytime somnolence (EDS), but its effect on vascular function in patients without preexisting cardiovascular disease (CVD) is unclear. METHODS: Fifty-three patients with OSA defined as an apnea-hypopnea index (AHI) of ⩾15 and without CVD were recruited into a double-blind, randomized, placebo-controlled, crossover trial of 12weeks of
