Nightmare after trauma as paradigm for all dreams: a new approach to the nature and functions of dreaming.

Nightmares, far from being unsuccessful dreams or exceptions to rules about dreams, can be considered paradigms for all dreaming. They allow us to follow exactly how a disturbance or perturbation is handled by the processing systems in our minds. The data considered here consists of dream series in the weeks and months immediately following trauma in adults--in other words, nightmares and dreams occurring as the trauma resolves. It appears the traumatized person may dream first about the actual trauma (though not always), then, very quickly, the dreams appear to deal with the dominant emotion.

Reversibility of disturbed pituitary function in pediatric conditions with psychological stressors: implications for clinical practice.

The complex communication network between the central nervous system and the hypothalamic-pituitary axis forms the basis of endocrine functional plasticity, which facilitates adaptation to changing internal and external conditions, but also makes it vulnerable to the negative effects of stressful psychological factors. Herein, clinical conditions such as functional hypothalamic amenorrhea, eating disorders, growth faltering, post-traumatic stress disorder, and pubertal disorders that may emerge during childhood or adolescence, their origin possibly including psychological stressors, are analyz

Fear generalization immediately after contextual fear memory consolidation in mice.

Fear generalization is a symptom of anxiety-related disorders, including acute stress disorder and post-traumatic stress disorder. Using a contextual fear conditioning paradigm, we found that mice exposed to a similar neutral context but not a different neutral context soon after training showed fear generalization immediately after contextual fear memory consolidation (i.e., 6 h after training). This fear generalization was reflected by a change not only in the total amount but also the pattern of freezing between conditioned and generalized contexts. These results provide insight into the fa

Intimate partner violence and women's health and wellbeing: impacts, risk factors and responses.

Women have approximately a one in four chance of experiencing intimate partner violence (IPV). Those who do are at increased risk of developing physical and mental health problems including traumatic brain injury, chronic pain, gastrointestinal disorders, depression, post-traumatic stress disorder, and substance-related disorders. Nurses, in whatever situation they work, are therefore highly likely to encounter women who are victims of IPV. This paper explores the prevalence of physical and mental health issues for women with an experience of IPV. Factors that influence a woman's experience of

Is there a physiological role for the neurosteroid THDOC in stress-sensitive conditions?

Endogenous neurosteroids affect brain excitability during physiological states such as pregnancy and the menstrual cycle, and during conditions of acute and chronic stress. The neurosteroid allotetrahydrodeoxycorticosterone (THDOC) is an allosteric modulator of the GABA(A) receptor. Although the role of THDOC within the brain is undefined, recent studies indicate that stress induces THDOC to levels that can activate GABA(A) receptors. These results might have significant implications for human stress-sensitive conditions such as epilepsy, post-traumatic stress disorder and depression.

Parent and Family Well-Being and Associated Risk Factors as Children with Neonatal Seizures Reach Preschool and School-Age: A Longitudinal Cohort Study.

OBJECTIVE: To assess parent/family well-being when children with neonatal seizures reach 3-8 years of age and examine factors associated with parent/family well-being. METHODS: One parent per surviving infant in the Neonatal Seizure Registry-II was invited to complete validated surveys annually when children were between 3 and 8 years of age. Three outcomes were examined: (1) parent well-being (anxiety, depression, and quality of life); (2) parent post-traumatic stress symptoms; and (3) impact on the family. We used mixed model regression with random intercepts and guided backward elimination

Sociodemographic predictors of postpartum post-traumatic stress symptoms-cross-sectional study.

BACKGROUND: Postpartum psychological distress, including depression and post-traumatic stress disorder (PP-PTSD), poses serious risks for maternal and child well-being. The role of sociodemographic predictors remains less understood, particularly in Poland. METHODS: An observational, cross-sectional study was conducted among 273 Polish women. Sociodemographic and obstetric information was obtained using a self-designed online questionnaire. Symptoms of depression, anxiety, and stress were measured using the DASS-21, and post-traumatic stress symptoms were assessed with the PTSD-8 scale. Multi

The manifestation of anxiety disorders after traumatic brain injury: a review.

The development of anxiety disorders after a traumatic brain injury (TBI) is a strong predictor of social, personal, and work dysfunction; nevertheless, the emergence of anxiety has been largely unexplored and poorly understood in the context of TBI. This article provides an overview of the limited published research to date on anxiety disorders that are known to develop after TBI, including post-traumatic stress disorder, generalized anxiety disorder, obsessive-compulsive disorder, panic disorder, specific phobia, and social anxiety disorder. This review also examines diagnostic criteria, the

Acute stress disorder and posttraumatic stress disorder: a prospective study of prevalence, course, and predictors in a sample with major burn injuries.

This is one of the largest prospective studies of patients with major burn injuries to use psychometrically sound methods to track and predict posttraumatic stress disorder (PTSD) across 2 years after burn. The principal objectives were to investigate the utility of self-report measures in detecting acute stress disorder (ASD) and PTSD, and in tracking and predicting PTSD. Participants were adult patients admitted for treatment of a major burn injury. The Stanford Acute Stress Reaction Questionnaire (SASRQ) was used to assess ASD symptomatology at discharge (n = 178), and the Davidson Trauma S

Depression, anxiety, and post-traumatic stress symptoms among wildland firefighters and local residents in recurrent wildfire areas.

BACKGROUND: Recurrent wildfires are an emerging public health concern, yet little is known about their mental health impact on both wildland firefighters and local residents in low‑ and middle‑income countries. This study aimed to compare depression, anxiety, stress, and post‑traumatic stress symptoms between firefighters and residents living in a recurrent wildfire area in southern Thailand, and to identify factors associated with these outcomes. METHODS: A cross‑sectional survey was conducted among 108 wildland firefighters and 1,090 residents in communities surrounding Kuan Kreng Peat Swam

Post-traumatic stress in cancer patients: prevalence and psychosocial correlates.

AIMS: Post-traumatic stress symptoms (PTSS) are a substantial yet underrecognised mental health concern in oncology. Large-scale epidemiological data across diverse cancer types are scarce. This study aimed to estimate the prevalence of post-traumatic stress disorder (PTSD) and to identify socio-demographic and psychosocial correlates in a large, unselected outpatient cohort. METHODS: In this cross-sectional study, 4,103 adult outpatients with solid tumours or haematologic malignancies were screened during routine care using the PTSS-8-Cancer scale. Socio-demographic, clinical and psychosocia

When good news falls short: Prospective role of positive belief updating in predicting post-trauma distress.

Adaptive functioning relies on the ability to update beliefs in response to new information. However, belief updating is often optimistically biased, with individuals more readily revising their beliefs in response to desirable (good news) than to undesirable (bad news) information. Deficiencies in positive updating have been linked to psychological distress. However, the temporal association between positive belief updating and psychological distress remains unclear. Given that trauma exposure strongly challenges prior beliefs and places heightened demands on belief updating, the present stud

Exploring within and between associations of momentary mindfulness and emotion regulation and the moderating effects of mental health among adolescents.

Greater mindfulness is thought to give rise to more positive psychological health through more adaptive emotion regulation. While there is extensive evidence linking higher average mindfulness to lower average emotion regulation difficulties, less is known about the momentary-level patterns that occur within individuals. Additionally, it is unclear if the relationship between mindfulness and emotion regulation changes as a function of adolescents' mental health status. This study sought to fill these gaps by investigating the between- and within-person concurrent (same moment) and prospective

After the storm, the calm never came: Hurricane Maria-related stressors and PTSD symptoms among Puerto Rican youth.

OBJECTIVE: Youth may develop posttraumatic stress disorder (PTSD) following a hurricane. Triaging of mental health services is crucial to effectively deliver trauma-focused interventions following natural disasters. Given the increased likelihood of hurricanes due to the current climate crisis, this study sought to examine the dose-response effect between hurricane-related stressors and PTSD, identify a cumulative stressor cutoff score based on the number of hurricane-related stressors experienced, and identify important individual hurricane-related stressors in explaining PTSD symptoms among

Initial development of the Stress Monitoring and Response Tool (SMART): A holistic measure of stress following trauma.

In the immediate aftermath of trauma exposure, individuals may experience an acute stress reaction (ASR). ASRs may be transient but for individuals operating in high-stakes occupations, these reactions can potentially endanger themselves and those around them. Thus, a better understanding of ASRs could facilitate development of early interventions that help prevent longer-term sequelae. Although existing measures (e.g., PCL-5, CAPS-5) target symptoms that occur in the weeks following trauma, they do not encompass the range of ASR symptoms identified in emerging research. Using data from a larg

Gender differences in the associations of psychosocial trauma and acute medical stressors with immune system activation and dementia risk.

Objective: The purpose of this article is to provide a narrative review synthesizing the literature on differences between women and men in relationships among certain stressors associated with immune system activation and their relationship to cognitive dysfunction and dementia. Method: We review the cycle of stress leading to neuroinflammation via cortisol and neurochemical alterations, cell-mediated immune system activation, and pro-inflammatory cytokines, and how this is implicated in the development of dementia. We follow this by discussing sex differences in stress physiology and immune

Potential benefits of intranasal neuropeptide Y include sustained extinction of fear memory.

Compelling evidence in animals and humans from a variety of approaches demonstrate that neuropeptide Y (NPY) in the brain can provide resilience to development of many stress-elicited symptoms. Preclinical experiments demonstrated that delivery of NPY by intranasal infusion to rats shortly after single exposure to traumatic stress in the single prolonged stress (SPS) rodent model of post-traumatic stress disorder (PTSD) can prevent development of many relevant behavioral alterations weeks later, including heightened anxiety and depressive-like behavior. Here, we examined responses to intranasa

Childbirth as an anticipated trauma during pregnancy: pretraumatic stress symptoms in primiparous women.

INTRODUCTION: Women can develop anticipated traumatic reactions related to the forthcoming delivery through "pretraumatic stress" symptoms. The present study aims at: a) exploring the frequency of probable pretraumatic stress disorder in primiparous pregnant women, b) evidencing associated features of pretraumatic stress symptoms and c) exploring which specific components of antenatal anxiety are associated with pretraumatic stress symptoms. METHODS: A sample of 100 primiparous pregnant women completed an online questionnaire assessing pretraumatic stress, fear of childbirth, depressive and a
