The role of clinical pharmacology in molecular genetics.

PROBLEM: Discovering the causes of unusual phenotypes in human subjects is an important aspect of patient-oriented research. MATERIAL: The tools of clinical pharmacology are uniquely useful in addressing these problems. PATIENTS, SUBJECTS, OR CASE HISTORIES: We evaluated a 42-year-old patient with lifelong orthostatic hypotension and ptosis of the eyelids. He underwent a series of biochemical, physiological, and pharmacological tests outlined in this article. RESULTS: These studies indicated that sympathetic innervation was intact but that the sympathetic neurotransmitter was dopamine rather

Orthostatic hypotension in spinal cord injured patients.

Treatment of orthostatic hypotension is an important consideration for many patients with spinal cord injuries, especially those with an injury to the cervical cord. Delay in attainment of an upright sitting posture can cause a delay in the completion of rehabilitation. Although most trials of medications used in the treatment of orthostatic hypotension have been performed on non-spinal cord injured patients, this paper provides a review of both pharmacological and non-pharmacologic means of therapy. Normally, regulation of mean arterial pressure occurs through several different mechanisms; lo

Sympathetic withdrawal and forearm vasodilation during vasovagal syncope in humans.

Our aim was to determine whether sympathetic withdrawal alone can account for the profound forearm vasodilation that occurs during syncope in humans. We also determined whether either vasodilating beta 2-adrenergic receptor or nitric oxide (NO) contributes to this dilation. Forearm blood flow was measured bilaterally in healthy volunteers (n = 10) by using plethysmography during two bouts of graded lower body negative pressure (LBNP) to syncope. In one forearm, drugs were infused via a brachial artery catheter while the other forearm served as a control. In the control arm, forearm vascular re

Percutaneous transluminal mitral valvuloplasty normalizes baroreflex sensitivity and sympathetic activity in patients with mitral stenosis.

BACKGROUND: In patients with mitral stenosis, reduced cardiac output or altered pulmonary hemodynamics may increase sympathetic nerve activity. However, the magnitude of the increase in sympathetic activity in such patients and the effect of valvuloplasty on this activity are unknown. METHODS AND RESULTS: We microneurographically measured muscle sympathetic nerve activity before and after mitral valvuloplasty in 10 patients (mean+/-SEM age, 48+/-2 years) with mitral stenosis and in 10 healthy volunteers (47+/-4 years); hemodynamic variables were also measured. Baroreflex sensitivity was asses

Reduction of 0.1 Hz microcirculatory fluctuations as evidence of sympathetic dysfunction in insulin-dependent diabetes.

OBJECTIVE: Loss of spontaneous fluctuations in resting microcirculatory flow has been described in diabetes mellitus, but its mechanism remains unexplained. METHODS: The autonomic control of forearm skin microcirculation was investigated in 23 insulin-dependent diabetic human subjects (median age 39 years, range 27-50) and in 23 age-matched controls (median age 38 years, range 20-57), by laser-Doppler flowmetry. Using spectral analysis of spontaneous microvascular fluctuations, we measured the power of 0.1 Hz ('10-second rhythm') fluctuations, dependent on sympathetic control, and of respirat

Effects of aging and of chronic obstructive pulmonary disease on RR interval variability.

Clinical and experimental investigations suggest that spectral analysis of RR interval variability might be employed to assess changes in autonomic regulation of the SA node occurring in various chronic, cardiac or respiratory conditions. We studied three groups of subjects: young healthy, older healthy, and old patients with chronic obstructive pulmonary disease (COPD), considering RR interval and breathing, i.e. tidal volume, variability in the frequency domain (AR algorithms), in order to obtain markers of the sympathetic and vagal modulation of the SA node [by way, respectively, of the low

The evidence for a pathophysiologic significance of the sympathetic overactivity in hypertension.

Interest for the role of the sympathetics in the genesis of hypertension has come full circle from early enthusiasm, through a period of neglect, to present understanding that strong evidence cannot be ignored. Sympathetic overactivity starts in childhood and is easily evident in 30% of patients with incipient hypertension. Later, in advanced hypertension, altered cardiovascular responsiveness obscures the obvious signs of sympathetic overactivity but the brain maintains its decisive pathophysiologic role. The early onset, the bimodal distribution of the neurogenic "hyperkinetic" pattern and f

Spontaneous flow waves detected by laser Doppler in human skin.

The contribution of spontaneous variations in sympathetic nervous activity and perfusion pressure to the laser Doppler flux signal in human skin was studied in nine healthy subjects. Simultaneous recordings were made of laser Doppler flux, mean blood pressure, and blood flow in the radial artery. In the skin of the palm and sole, there was a significant and strong correlation between fluctuations in flux and radial artery velocity, which indicated a high degree of neural control of microcirculatory blood flow. This correlation decreased progressively toward the trunk and face. In the skin of t

Inhibitory effect of cilnidipine on vascular sympathetic neurotransmission and subsequent vasoconstriction in spontaneously hypertensive rats.

We reported previously that cilnidipine inhibited increases in blood pressure and plasma norepinephrine (NE) level in response to cold stress in spontaneously hypertensive rats (SHRs). In the present study, we investigated the effect of cilnidipine on sympathetic neurotransmission and subsequent vasoconstriction in SHRs. In pithed SHRs, electrical sympathetic nerve stimulation (ESNS) elevated blood pressure, and this pressor response was abolished by guanethidine. Cilnidipine at 10 micrograms/kg, i.v. and phentolamine at 1 mg/kg, i.v. suppressed the pressor response to ESNS by 28 +/- 6% and 67

The vasodilator and secretory effects elicited by sympathetic nerve stimulation in cat submandibular gland.

The effects of electrical stimulation of the peripheral cut ends of the ascending cervical sympathetic trunk on vasomotor, particularly vasodilator, response were studied in relation to salivary secretion in the cat submandibular gland. The vasodilator and salivary responses were compared by stimulating the peripheral cut ends of the sympathetic nerve at various intensities (1-8 V), durations (5-60 s) and frequencies (1-100 Hz) using a 2 ms pulse duration. Electrical stimulation of the cervical sympathetic nerves caused vasoconstriction followed by vasodilatation and salivation. There were cer

Inhibitory sympathetic action on the carotid body responses to sustained hypoxia.

The mammalian carotid bodies receive sympathetic innervation from the superior cervical ganglion. The purposes of the present study were: (1) to investigate whether sympathetic innervation influences the carotid body response to hypoxia, and, if so, (2) to determine the involvement of adrenoceptors in these influence. Chemo-sensory activity was recorded from clearly identifiable action potentials from the carotid sinus nerve in 20 anaesthetized, paralyzed and artificially ventilated cats. Chemoreceptor responses to sustained isocapnic hypoxia (30 min, duration) were compared before and after c

Hemodynamic changes during prolonged laparoscopic surgery.

7 healthy pigs, anesthetized with ketamine/azaperon/thiopentone and ventilated with O2/N2O by volume control, underwent anterior resection of the descending colon by laparoscopic view. During operation of pneumoperitoneum by inflating CO2 to an abdominal pressure of 14 mm Hg was installed. Immediately (+2 min) after the onset of insufflation, both systemic and pulmonary arterial pressure increased. However, pulmonary artery pressure started to decrease after 10 min, whereas systemic arterial pressure remained elevated until the end of the experimental protocol. Left ventricular (LV) pressure a

Origin of cardiac-related synchronized cardiac sympathetic nerve activity in anaesthetized cats.

To study the origin of cardiac-related rhythm in cardiac sympathetic nerve activity (CSNA), ECG, aortic pressure and CSNA were recorded when cardiac interval was changed by artificial pacing, or when the aortic nerve was stimulated after baroreceptor denervation in anaesthetized cats. CSNA was averaged by using the R-wave of ECG, or stimulus pulse as a trigger. Delay times from arterial pulse or stimulus pulse to the onset and half amplitude of inhibition and to the maximal inhibition were measured from the averaged data. The delay of inhibition in CSNA was constant and independent of pacing i

Age-related changes in melatonin modulation of sympathetic neurotransmission.

An age-related reduction in the maximal acetylcholine (ACh)-induced contraction in the prostatic portion of rat vas deferens during the day was observed. This contraction is due to the release of norepinephrine and adenosine triphosphate from sympathetic nerve terminals. Male Wistar rats (4 and 24 months-old) were housed on a light/dark cycle (12:00 h/12:00 h, lights on at 6:00). The diurnal variation of ACh-induced contraction was determined on animals sacrificed every 3 h during the day. Aging reduced the maximal contraction and shortened the length of the nocturnal response. In both young a

Innervation of the ciliary process vasculature and epithelium by nerve fibers containing catecholamines and neuropeptide Y.

The ciliary processes (CPs) in the rat were investigated for the presence of neuropeptide Y (NPY)-like immunoreactivity and for histofluorescence indicating catecholamine (CAM)-ergic innervation in the rat, rabbit and cynomolgus monkey. Special attention was paid to those CP vessel segments which in previous studies showed distinct reactions on application of CAM or NPY, and on the respective sections of the CP epithelium. In the rat and cynomolgus monkey most CAM-ergic nerve fibers concentrated along the terminal arterioles and the epithelium of the anteriormost portion of the major CPs. In c

Effect of L-dopa on human muscle sympathetic nerve activity.

This study was undertaken to clarify the mechanism of orthostatic hypotension, one of the side effects frequently encountered after administration of levodopa (L-dopa). Muscle sympathetic nerve activity (MSNA) was measured concomitantly with blood pressure (BP), heart rate (HR) and calf blood flow (calf BF). The responses of these variables to head-up tilting (HUT) were analyzed before and after intravenous administration of L-dopa (50 mg) in eight healthy subjects. L-dopa induced significant increases in MSNA, with concomitant decreases in BP and no changes in calf BF in the supine position.

Block of calcium channels in rat neurons by synthetic omega-Aga-IVA.

We investigated block of voltage-dependent Ca channels in freshly dissociated rat central and peripheral neurons by the synthetic peptide omega-Aga-IVA. Synthetic omega-Aga-IVA blocked approximately 90% of the high-threshold Ca current in cerebellar Purkinje neurons with an estimated Kd of approximately 1.5 nM, slightly higher than that determined for block by toxin purified from Agelenopsis aperta venom. At 200 nM, the synthetic peptide blocked a small fraction of current in dorsal root ganglion neurons but had no effect on identified components of current carried by low-threshold T-type chan

Angiotensin II facilitates sympathetic transmission in rat hind limb circulation.

We developed a novel method to stimulate the sympathetic innervation of the isolated, perfused rat hind limb to investigate whether a subpressor concentration of angiotensin II (Ang II) facilitates noradrenergic transmission in the vascular bed to skeletal muscle. We electrically stimulated the lumbar sympathetic trunk while perfusing the preparation with artificial medium. Seventy-five percent of the resulting frequency-dependent increases in perfusion pressure were mediated by alpha 1-adrenergic receptors. Ang II (10 nM) significantly enhanced the effects of nerve stimulation at 1 and 10 Hz

Effects of gamma-aminobutyric acid on the compound action potential of the rat superior cervical ganglion.

The effects on the ganglionic transmission of gamma-aminobutyric acid (GABA) and related drugs were studied in the rat isolated superior cervical ganglion. The extracellularly recorded postganglionic compound action potential was used as an index for ganglionic transmission. GABA reversibly inhibited the ganglionic transmission by blocking the action potential. The effect of GABA is markedly antagonized by bicuculline (20 microM) or picrotoxin (200 microM). Furosemide (1 mM) was also effective in antagonizing the ganglionic effect of GABA. The antagonism by bicuculline was not affected in the
