Current trends in anterior cruciate ligament surgery. A worldwide benchmark study.

OBJECTIVES: To benchmark current trends on anterior cruciate ligament reconstruction (ACLR) surgery. METHODS: The largest worldwide ACLR survey to date was performed during May 2020, targeted to reach representation of all continents. It was submitted electronically to all International Society of Arthroscopy, Knee Surgery and Orthopaedic Sports surgeons (n = 3,026), asking those who perform ACLR to respond. RESULTS: With a final sample size of 2,107, the overall response rate was 69.6%. Median years of practice as orthopaedic surgeon was 15 (range 1-52) and 49.6% of all respondents were def

Neuroarthropathy: an overuse injury of the shoulder in quadriplegia.

The shoulder joint of the spinal cord injured patient is the primary joint used for transfer and wheelchair propulsion. In addition to ordinary use, the shoulder is subject to overuse injury with resultant functional impairment. Frequent overuse injuries of the shoulder include subdeltoid bursitis, bicipital tendonitis, rotator cuff tear and secondary degenerative arthritis. Neuroarthropathy of the shoulder, an overuse injury of an insensate shoulder girdle, has been rarely described in the literature. The case presented is that of an active 47 year old, right hand dominant, C7 complete quadri

Osteochondroma of the bicipital tuberosity causing an avulsion of the distal biceps tendon.

Osteochondromas are one of the most common benign bone tumors. They usually arise from the metaphyses of long bones. Involvement of the bicipital tuberosity is rare. To date, no reports have described avulsed rupture of the distal biceps tendon caused by an osteochondroma of the bicipital tuberosity. This article presents a case of avulsion of the distal biceps tendon secondary to sessile osteochondroma of the bicipital tuberosity in a 65-year-old right-handed sedentary worker who presented with insidious pain and limited motion in his left elbow for 2 months. Intraoperative findings showed a

Device for zone-II flexor tendon repair. A multicenter, randomized, blinded, clinical trial.

BACKGROUND: The stainless-steel Teno Fix tendon-repair device has improved biomechanical characteristics compared with those of suture repair, and it was well tolerated in a canine model. The purpose of this study was to compare the Teno Fix with suture repair in a clinical setting. METHODS: Sixty-seven patients with isolated zone-II flexor tendon injury were randomized to be treated with a Teno Fix or a four-stranded cruciate suture repair. There were eighty-five injured digits: thirty-four were treated with the Teno Fix, and fifty-one served as controls. A modified Kleinert rehabilitation t

Isolated Proximal Tibiofibular Joint Reconstruction With Semitendinosus Tendon Autograft in a Collegiate Athlete.

BACKGROUND: Proximal tibiofibular joint (PTFJ) instability is a rare and often underrecognized cause of lateral knee pain and dysfunction, most commonly presenting as anterolateral subluxation of the fibular head after trauma. While conservative management is first-line, persistent instability may require operative intervention. INDICATIONS: We present the case of a 20-year-old male athlete who sustained a PTFJ injury during sport and noted instability. After a successful PTFJ diagnostic taping trial, he elected for surgical reconstruction after completing his competitive season. Imaging conf

Soft Tissue Reconstruction in the Extremities: Principles, Techniques, and Outcomes.

Soft tissue reconstruction in the extremities is complicated by the fact that injuries often involve composite defects of skin, muscle, tendon, nerve, and bone. In both the upper and lower limbs, achieving reliable soft tissue coverage, restoring function, and reducing complications are critical goals. The upper extremity requires precision and fine manipulation, whereas the lower limb should be reconstructed to support weight-bearing and mobility. Free flaps have become a routine part of the surgical armamentarium, offering flexible solutions tailored to different needs. Donor sites in the lo

Peripheral Lesions of the Triangular Fibrocartilage Complex.

The recently introduced CUP classification of the triangular fibrocartilage complex divides the lesions into central (C), ulnar (U), and peripheral (P). The periphery of the triangular fibrocartilage complex consists of the meniscus homologue, the ulnocarpal joint capsule including the extensor carpi ulnaris tendon sheath, and the ulnotriquetral and ulnolunate ligaments. Peripheral triangular fibrocartilage complex lesions can occur in isolation or in combination with injuries of the ulnar insertions and/or the articular disk. Most commonly the meniscus homologue and dorsal capsule are affecte

Bilateral Quadriceps Tendon Rupture in a Healthy Individual Following a Motor Vehicle Accident: A Case Report.

Bilateral quadriceps tendon rupture (QTR) is a rare injury that typically affects middle-aged men presenting underlying medical conditions, while only a few cases have been reported in healthy individuals. The gold standard treatment of such injuries is prompt surgical repair, followed by postoperative immobilization and physiotherapy. We present the case of a 51-year-old previously healthy man who experienced bilateral, simultaneous, and complete QTR following a high-velocity motor vehicle accident. Physical examination revealed bilateral extensor mechanism disruption and palpable defects at

Dorsal Impaction Injuries of the Distal Radius: Operative Technique Through Dorsal Approach to the Wrist.

Distal radius fractures vary widely in fracture pattern and displacement. Impaction injuries involving the dorsal articular surface of the distal radius can present challenges when anatomic reduction and fixation is attempted through a standard volar approach. Dorsal approach to the distal radius can provide direct visualization of these fracture patterns, greatly facilitating anatomic reduction and stabilization. In this technique video, surgical approach, fracture reduction, and operative fixation of a dorsally impacted, intra-articular distal radius fracture through a dorsal approach is pre

Application of cryopreserved autologous skin replantation in the treatment of degloving injury of limbs.

Degloving injury is a common and intractable injury with the bone and tendon exposed and contamination, the stripped skin cannot be replanted immediately and will be discarded, although auto-graft is needed for subsequent wound repair. In this study, autologous skin cryopreservation technique was applied to the treatment of severe limb degloving injuries. The clinical data of 9 patients from January 2016 to December 2018 were analyzed retrospectively. Among the 9 cases, 1 case developed necrosis due to wound infection, and the rest survived 60-100%. The replanted cryopreserved skin were soft a

Surgical Management of Lateral Ankle Instability in Athletes.

Ankle sprains are common injuries involving the lateral ankle ligaments and affect athletes of all levels. Most patients heal uneventfully, but those with symptoms persisting past 3 months should be evaluated for chronic ankle instability and its associated conditions as well as for the presence of varus malalignment. Chronic ankle instability is initially treated nonoperatively, with surgical management reserved for those who have failed to improve after 3 to 6 months of bracing and functional rehabilitation. Anatomic repair using a modification of the Broström procedure is the preferred tech

Multiple retentive means for prosthetic restoration of a large facial defect - a case report.

Rotator cuff tears (RCTs) are common in shoulder disease and disability. Despite significant advances in surgical repair techniques, 20-70% of patients still have postoperative rotator cuff dysfunction. These functional defects may be related to retear or rotator cuff quality deterioration due to tendon retraction and scar tissue at the repair site. As an effective delivery system, hydrogel scaffolds may improve the healing of RCTs and be a useful treatment for irreparable rotator cuff injuries. Although many studies have tested this hypothesis, most are limited to laboratory animal experiment

Free flap choice for soft tissue reconstruction of the severely damaged upper extremity.

Reconstruction of complex wounds of the hand associated with severe bone, tendon, nerve and soft-tissue injuries has been a major problem in hand surgery. Early definitive soft-tissue coverage of this kind of extensive wound with well-vascularized tissue is one of the most important stages of reconstruction for salvage of the extremity and restoration of function. Although multiple free flap donor sites have been described for complex upper extremity wounds, the authors think that anterolateral thigh (ALT) and lateral arm (LA) flaps are good choices for soft-tissue reconstruction in the upper

Kinematic analysis of the distal radioulnar joint after a simulated progressive ulnar-sided wrist injury.

A kinematic analysis of a progressive, ulnar-sided wrist injury was performed using a cadaver model to study the static and dynamic stabilizers of the distal radioulnar joint with a 3-dimensional motion tracking system. Anatomically based loads were applied to achieve pronation and supination and then the specimens were evaluated after serial transection of the triangular fibrocartilage, the extensor carpi ulnaris sheath, and the ulnocarpal ligaments. Statistically significant increases in translation of the radius relative to the ulna occurred with sectioning of both the triangular fibrocarti

The island pedicle rotation advancement gastrocnemius musculocutaneous flap for complete coverage of the popliteal fossa.

The medial head of the gastrocnemius flap is a large, versatile flap that can be used to cover areas above the popliteal fossa, thereby minimizing the necessity for amputation in many traumatic injuries to this area. By advancing the medial head of the gastrocnemius muscle and rotating it on the neurovascular pedicle of the medial sural vessels, we have covered a 17 cm X 20 cm defect of crushed muscle, tendon, and bone in a leg that was traumatically amputated proximal to the knee. This procedure makes possible muscular coverage for bone, stainless steel plate, and reanastomosed arteries and v

Regenerative Trifecta: A Multimodal Regenerative Approach to Soft Tissue Injury Recovery and Return to Sport.

Soft tissue injuries - affecting tendons, ligaments, and muscles - are the leading cause of missed games, lost seasons, and early career endings in competitive sports. Injury rates across major professional leagues have risen significantly over the past decade, with the knee, groin, hip, and thigh bearing the greatest burden. The financial impact is enormous at the high school and collegiate levels alone, while at the professional level, soft tissue failures have decided playoff outcomes and derailed championship-caliber teams. Standard treatments - rest, physical therapy, nonsteroidal anti-in
