Cost Impact of a Novel Pre-transplant Risk Assessment Tool for Early Acute Rejection in Kidney Transplant Patients.

Background: Accurately predicting outcomes and stratifying risks remains a challenge in kidney transplant patients. The Pre-Transplant Risk Assessment (PTRA) predicts risk of early acute rejection (EAR) in deceased donor transplant recipients, enabling clinicians to tailor immunosuppression strategies using individual immune response risk. Objectives: We developed a cost-impact analysis of adding PTRA to the current standard of care to predict the risk of EAR compared with the current protocol-driven immunosuppression in standard-risk kidney transplant patients. Methods: A decision tree model

Impact of pre-transplant induction cycles on post-transplant outcomes in patients with ALL: a study from the ALWP EBMT.

The impact of the number of induction cycles required to achieve first complete remission (CR1) on transplant outcomes in adult acute lymphoblastic leukemia (ALL) patients remains unknown. We conducted a retrospective EBMT registry analysis (2000-2022) of ALL patients who underwent transplantation in CR1 after one (n = 2038), two (n = 296), or three or more (n = 110) induction cycles. Median age was 40 years (range 18-73); 79% had B-ALL. At 2 years, relapse incidence was 23%, 31%, and 32%, while non-relapse mortality was 17%, 18%, and 16%, for those achieving CR1 after one, two, and ≥3 cycles,

Impact of distance from liver transplant centre on outcomes following liver transplantation: an Australian single-centre study.

BACKGROUND: Access to liver transplantation (LT) is affected by geographic disparities. Higher waitlist mortality is observed in patients residing farther from LT centres, but the impact of distance on post-LT outcomes is unclear. AIMS: To evaluate whether the distance LT recipients reside from their LT centre affects graft and patient outcomes. METHODS: We retrospectively studied consecutive adult patients who received deceased donor LT at a statewide LT referral centre, Royal Prince Alfred Hospital (RPAH), 2006-2021. The primary outcome was overall patient survival. RESULTS: A total of 97

Trends in the Perioperative Practices for Immunological Assessment and Immunosuppression Strategies for Patients Undergoing Intestinal Transplantation at American Transplant Centers.

BACKGROUND: Intestinal transplantation (IT) is a complex procedure that requires nuanced immunosuppressive strategies to optimize patient ou … logies. Desensitization was reported by 44% of centers for isolated IT and by 22% for multivisceral transplants. Induction therapy predominantly involved antithymocyte globulin (89%) and rituximab (44%). Tacrolimus was universally used for maintenance, with varying trough level targets across centers. Post-transplant DSA testing was performed by all centers, …

The impact of tacrolimus levels on acute GVHD and transplant outcomes in haploidentical hematopoietic stem cell transplantation: A retrospective analysis.

The impact of early tacrolimus (TAC) blood levels on acute graft-versus-host disease (aGVHD) and transplant outcomes in adults undergoing haploidentical hematopoietic stem cell transplantation (haplo-HSCT) with posttransplant cyclophosphamide (PTCy) is incompletely investigated. We retrospectively analyzed 161 T-cell-replete haplo-HSCT with PTCy, TAC, and mycophenolate-mofetil. TAC trough levels from weeks 1-2 (w1/2) and weeks 3-4 (w3/4) posttransplant were categorized as "Low" or "High" using a threshold of 10 ng/ml. Outcomes assessed included grade III-IV acute graft-versus-host-disease (aGV

Post-Transplant Lymphoproliferative Disorder following Heart Transplantation Presenting as a Retroperitoneal Plasmacytoma: A Case Report and Review of the Literature.

INTRODUCTION: Plasma cell neoplasms (PCNs) are a rare type of post-transplant lymphoproliferative disease (PTLD). Extramedullary plasmacytoma is a rare type of PCN, and is infrequently located in the retroperitoneum. According to the current literature, heart transplant recipients are considered at intermediate risk for developing PTLD. CASE PRESENTATION: We report a case of a 70-year-old patient with a history of heart transplantation, who was admitted to the hospital with back pain and swelling of the left lower limb. Imaging revealed a large retroperitoneal mass involving the left kidney,

Ultrasound for monitoring different stages of post-transplant lymphoproliferative disorder in a transplanted kidney: A case report and review of the literature.

RATIONALE: Post-transplant lymphoproliferative disorder (PTLD) is a well-recognized, but uncommon complication in patients with kidney transplantation, which poses challenges in diagnosis and poor prognosis due to its low incidence and nonspecific clinical manifestations. As a routine follow-up examination method for kidney transplant patients, ultrasound (US) plays a significant role in the diagnosis of PTLD. Therefore, it is critical to evaluate the ultrasonic characteristics of PTLD in transplanted kidney patients for early detection and diagnosis. PATIENT CONCERNS: A 59-year-old female pa
