Esophageal atresia, choanal atresia, and dysautonomia.

Patients with esophageal atresia (EA) or choanal atresia (CA) manifest similar clinical and pathophysiological features. To determine the significance of this observation, the clinical records of 80 patients with EA and 57 with CA were reviewed. This survey showed that similarities between the two conditions included inspiratory and expiratory dyspnea, episodes of reflex apnea and/or bradycardia, oropharyngeal dysphagia, vomiting, convulsions, hyperhydrosis, hyperthermia, sialorrhea, and sudden death. After the second year of life most symptoms disappeared spontaneously. In both conditions, re

Inhibition of activity-induced parotid growth by submandibular gland ablation or autonomic denervation.

When solid chow was reintroduced for 2 days to rats previously maintained on an all-liquid diet, [3H]thymidine incorporation into rat parotid gland was 2.4-4.4 times that of chow-fed or liquid-fed animals. When the submandibular-sublingual glands were removed before the change in diet from liquid to solid food, values for [3H]thymidine incorporation of parotid gland were not statistically different from those of chow-fed rats: the increase was completely prevented. The increase in gland size that accompanied the change in dietary consistency was not prevented by prior submandibular gland ablat

Rat salivary glands as a model for the study of adrenergic receptor modulation.

The properties (kD and Bmax) of alpha-1 and alpha-2 adrenoceptors in rat submaxillary gland have been determined by the binding process of radioactive ligands. Data of dissociation constants compiled from several tissues by this technique closely agree with physiological measurements. The radioactive ligand binding method has afforded evidence for changes in receptor number elicited by procedures that induce either hyper or hyposensitivity of the response. Chemical sympathectomy with reserpine increases 10 fold the number of alpha-2 receptors in rat submaxillary gland, as determined by the bin

Immunoreactive erythropoietin in rodent submaxillary gland. Autonomic control of exocrine secretion and factors influencing its concentration.

The SMG of mice and rats contain a heterologous group of biologically active factors. Some are well known, can be obtained at high purity and are well-characterized. There is strong evidence for the presence of others although they have not been purified. Finally, some of them are questionable and/or have not yet been characterized. EPO would be one of the factors whose presence in the SMG is strongly suspected, although its biological activity has not been demonstrated yet. Its presence in the gland, therefore, is only supported by radioimmunoassay data and immunocytochemical methods. Immunor

Posttraumatic gustatory rhinorrhea.

We describe a case of gustatory rhinorrhea in which gustatory stimuli caused nasal obstruction and secretion simulating cerebrospinal rhinorrhea. This disorder was presumably caused by faulty regenerated parasympathetic nerve fibers reaching the nasal mucosa or, possibly, by a congenital condition. The characteristics of this disorder are compared with other autonomic disorders of the head and neck.

Presence of non-cholinergic motor transmission in human isolated bladder.

Atropine-sensitivity of the motor transmission in the isolated detrusor preparation from human bladder has been examined. The preparations were contracted by electrical field stimulation consisting either of short trains of pulses or of long trains of pulses. Part of the stable response to short-train stimuli (28%) was resistant to atropine, was not potentiated by physostigmine and was blocked by tetrodotoxin. The stable responses to long-train stimuli were fully blocked by atropine. It is concluded that the detrusor of the bladder in man, in common with other mammalian species, contains a non

[Autonomic ophthalmologic aspects of occupational environmental health effects on seamen aboard transport and commercial vessels].

Complex investigations of the functional state of the visual analyser and vascular system has been carried out in 150 practically healthy members of a screw working in extreme conditions of a long-term voyage. It is shown that approximately one month after the beginning of the voyage the observed members of the screw developed signs of functional changes in visual acuity, light sensitivity, intraocular pressure level, hemodynamic indices. The main tendency is seen in some rise of the level of the studied indices within the first three-six months with their subsequent fall. Possible mechanisms

[False-positive test of autonomic neuropathy in HIV infection and AIDS? Case control study of heart rate variability in 62 HIV positive patients].

False positive tests for autonomic neuropathy in HIV-infection and AIDS? Case-control-study of heart rate variation in 62 cases Autonomic neuropathy is said to be common in AIDS. In a cross section study we tested heart rate variability, a simple and sensitive test, in relation to disease severity. The expiratory-inspiratory RR-interval-ratio (RRR) was normal in HIV-infected clinically healthy persons (mean 1.45; n = 10), but progressively lowered in LAS (1.24; n = 12), ARC (1.20; n = 7), AIDS (1.13; n = 20), AIDS with pneumocystis carinii pneumonia (1.07; n = 7) and patients who died within t

[Cold constitution: analysis of the questionnaire].

"Hie-sho" means having a cold constitution. The authors studied basic data for cold constitution with a questionnaire and with the Cornell Medical Index. The subjects were 318 females, and the average age was 26.4 +/- 6.1 (Mean +/- SD) and the range was 20 approximately 51 years of age. The results were as follows: (1) 38.7% of the subjects had been conscious of having a cold constitution, and it was recognized that the subjects with a cold constitution are found not only in the climacterium but also in the young. (2) The average age of onset of cold constitution was 19.3 +/- 5.1 years old. (3

Orthostatic hypotension-induced autonomic dysreflexia.

Three patients with chronic traumatic cervical myelopathy had severe orthostatic hyperhidrosis. Orthostatic challenge revealed that hypotension preceded hyperhidrosis, hypertension, and chills, all manifestations of autonomic dysreflexia. Treatment of orthostatic hypotension with fludrocortisone acetate relieved these symptoms. Therefore, orthostatic hypotension may trigger autonomic dysreflexia and the usual way of managing such patients, propping them upright, may be counterproductive.

Diabetic neuropathy. Human and experimental.

Human diabetic neuropathy consists of various clinical syndromes and it is unlikely that any single aetiological process is responsible for all of them. There is probably a complex interaction of different metabolic factors, microvascular disease, and perhaps even a genetic predisposition and other as yet unidentified environmental factors. Abnormalities of sorbitol and myo-inositol metabolism have been described in animal models and human studies of neuropathy and must be regarded as of major importance, especially since they can be profoundly influenced by blocking the enzyme aldose reductas

Dilute solutions of phenylephrine and pilocarpine in the diagnosis of disordered autonomic innervation of the iris. Observations in normal subjects, and in the syndromes of Horner and Holmes-Adie.

A standardized method of testing irideal sensitivity to 1% phenylephrine and 0.05% pilocarpine is described, and a quantitative basis for pupillary neurotransmitter "supersensitivity" established. In a normal 20-year-old subject the pupillary diameter increases in bright light by no more than 2.1 mm 1 h after ocular application of phenylephrine; this figure should be adjusted for age since "phenylephrine sensitivity" of the iris increases by 0.23 mm per decade. The pupillary diameter, when measured in darkness, has normally decreased by less than or equal to 1.4 mm within 30 min of administrat

Adrenergic hyperactivity and epidural block in severe tetanus. A case report.

In a 69-year-old man with severe tetanus, sympatho-adrenal overactivity was successfully treated with a lumbar epidural block. Cardiovascular disturbances were reduced and fluctuations in plasma catecholamines were decreased. The duration of other manifestations of tetanus was, however, not influenced by the epidural block in this case.

Cardiorespiratory arrest in a patient with advanced diabetic autonomic neuropathy.

A 53-year-old diabetic woman who had been diabetic for 14 years had recurrent episodes of cardiorespiratory arrest, which were easily resuscitated by a few chest massages. In 2 of 4 episodes a radial pulse was detected, so respiratory arrest was thought to be a primary event. Pentazocine was injected several hours prior to each of the 3 episodes. This was considered to be a precipitating factor. From the first episode of cardiorespiratory arrest, she received oxygen inhalation. When oxygen inhalation was withdrawn for 5-10 min, she became cyanotic. This was considered to be a sign of lack of h

Autonomic hyperreflexia: a mortal danger for spinal cord-damaged women in labor.

Reproductive care of women with spinal cord damage demands knowledge of such women's reproductive potential and the specific complications to which these women are prone during pregnancy and childbirth, especially autonomic hyperreflexia. Fertility in cord-damaged women of reproductive age is generally undiminished as are libido, ability to have intercourse, and ability to bear children. Frequent complications of cord-damaged pregnant women include urinary tract infection, anemia, pressure sores, sepsis, unattended birth, and autonomic hyperreflexia. Autonomic hyperreflexia or autonomic dysref

Subtle cardiac autonomic impairment in Chagas' disease detected by baroreflex sensitivity testing.

The sensitivity of baroreflex bradycardia and tachycardia was determined in fourteen patients with Chagas' disease who seemed to be normal with respect to cardiac autonomic control evaluated in terms of heart rate responses to the conventional atropinization and Valsalva maneuver tests. Eleven normal subjects were studied for comparison. Baroreflex sensitivity was determined by relating the beat-to-beat pulse intervals to systolic pressure values during transient phenylephrine- and amyl nitrite-induced changes in arterial pressure. Chagasic patients showed mean bradycardia sensitivity (10.1 +/

[Varieties of alcoholic abstinence].

On the basis of the evidence obtained from a clinical-psychopathological study of 154 patients with chronic alcoholism, four types of the abstinence syndrome were specified, including 1) ordinary nonpsychotic astheno-vegetative dysthymia; 2) an astheno-melancholic reaction as exogenically resolved "soil depression"; 3) a depressive-vegetative cyclothymic reaction or phase; 4) a schizothymovegetative reaction or exacerbation of an uneventful schizophrenic shift.

Abnormal breathholding in association with carotid body dysfunction mimicking sleep apnea.

Apneic episodes lasting as long as 126 s were observed in a previously well 22-yr-old woman and were felt to result from voluntary breathholding superimposed on a blunt hypoxic ventilatory drive secondary to carotid body dysfunction. Distinguishing features between this life-threatening voluntary apnea, sleep apnea, and nonconvulsive epilepsy are discussed as well as a theoretical review of breathholding per se.

[Interrelationship between the lateral hypothalamic self-stimulation reaction and the periodic motor activity of the stomach].

In 26 dogs, self-stimulation of the dorsal portion of the lateral hypothalamus at the level of anterior parts of ventromedial nuclei inhibits completely the stomach motor activity. Self-stimulation of the lateral portion of the middle hypothalamus at the level of posterior parts of ventro--medial nuclei does not affect at all the periodic contractions of the stomach. The intensity of self-stimulation depends on phase of the stomach periodic activity: during the contractions the number of lever-pressings is by 20% higher on the average than at resting.
