[Articular fractures of the base of the first metacarpal. Comparative study of direct osteosynthesis and closed pinning].

The authors retrospectively studied 35 articular fractures of the base of the first metacarpal, with an average follow-up of 3 years. Treatment consisted of closed reduction and pinning in 19 cases, open reduction and rigid fixation in 16 cases. Results were estimated in terms of pain, range of movement (flexion-adduction arc and abduction-extension arc), strength and X-rays. There was no significant difference between the two groups, for age, sex, occupation, side and postoperative care. Only one significant difference between both procedures was found: flexion-adduction was better after oste

Osteosynthesis of fractures of the base of the first metacarpal by an external fixator.

Among the various methods for treating fractures of the base of the first metacarpal, use of the external fixator appears to be of interest for it maintains a satisfactory reduction while respecting the arch of the first web interspace as well as allowing early mobilization of all joints not involved in the injury. The miniaturization of the material, today possible, facilitates its use. The experience with 20 cases gained at the CTO de la CRAM de Strasbourg is presented. The authors discuss the utilization of the method, the practical means of doing it and their results. Its limitations are i

Dual versus single implant fixation for geriatric distal femur fractures: protocol for a randomised, controlled pilot study at five US level 1 trauma centres.

INTRODUCTION: Geriatric distal femur fractures are associated with mortality rates exceeding 20%, comparable to hip fractures. Traditional single implant fixation often requires weight-bearing restrictions that delay recovery. This pilot study aims to assess the feasibility of conducting a multicentre randomised controlled trial comparing dual vs single implant fixation for geriatric distal femur fractures. METHODS AND ANALYSIS: This multicentre, prospective, randomised controlled pilot trial will enrol 80 participants aged 60 years or older with displaced distal femur fractures at five US le

The Community Orthopaedic Surgeon Taking Trauma Call: Radial Neck Fracture Pearls and Pitfalls.

Pediatric radial neck fractures will inevitably be encountered by the general orthopaedist, and they carry great potential for poor outcomes. Acceptable displacement varies by age, and unlike adults, most of these injuries can be managed by closed means. If they are taken to the operating room, the mainstays of treatment are percutaneous techniques. Increasing patient age, inadequate reduction, unnecessary open reductions, and prolonged immobilization are all associated with worse outcomes, most commonly manifesting as significant elbow stiffness. Knowledge of the age-specific acceptable displ

Long-term radiographic outcomes of a central hook-rod construct for osteotomy closure: minimum 5-year follow-up.

STUDY DESIGN: Retrospective study. OBJECTIVE: To evaluate long-term effectiveness of central hook-rod constructs for posterior spinal osteotomy closure. SUMMARY OF BACKGROUND DATA: During osteotomy site closure various techniques are used, including patient positioning, rod cantilevering, extending fixation points, and compressing through pedicle fixation points. All add premature stress on fixation points and may lead to loosening/eventual fixation failure. To avoid this, we often use a central compression hook-rod construct for osteotomy closure. METHODS: Fifty-six consecutive patients wi

The learning curve of proximal femoral nailing.

OBJECTIVE: The aim of this study was to evaluate operation time and surgical complication rate of proximal femoral nailing during resident training. METHODS: This retrospective evaluation included 659 patients (508 females, 151 males; mean age: 80.7 years), operated by 63 supervised residents between 1998 and 2010. All patients received the same implant (Targon® PF). Fracture classification, operation time and early surgical complications causing reoperation or hospital readmission (i.e. hematoma, infection, hip perforation, nonunion, implant malpositioning) were recorded and correlated with

[Trochanteric femoral fractures].

At the present time proximal femoral fractures account for 30% of all fractures referred to hospitals for treatment. Our population is ageing, the proportion of patients with post-menopausal or senile osteoporosis is increasing and therefore the number of proximal femoral fractures requiring urgent treatment is growing too. In the age category of 50 years and older, the incidence of these fractures has increased exponentially. Our department serves as a trauma centre for half of Prague and part of the Central Bohemia Region with a population of 1 150 000. Prague in particular has a high number

Locking intramedullary nails and locking plates in the treatment of two-part proximal humeral surgical neck fractures: a prospective randomized trial with a minimum of three years of follow-up.

BACKGROUND: locking intramedullary nails and locking plates specially designed for proximal humeral fractures are widely used. The purpose of our study was to compare the outcomes between these two types of implants in patients with a two-part surgical neck fracture. The advantages and shortcomings of each method were analyzed. METHODS: a prospective randomized study was performed. Fifty-one consecutive patients with a fresh two-part surgical neck fracture were randomized to be treated with a locking intramedullary nail (n = 25) or a locking plate (n = 26). Clinical and radiographic assessmen

[Volar fixed angle plate LCP 3.5 for dorsally distal radius fracture. About 24 cases].

PURPOSE OF THE STUDY: Fracture of the distal radius with posterior displacement is the most common wrist fracture. Pinning is generally proposed. The problem is that a secondary displacement may persist, even for the least complex fractures. With the advent of the fixed-angle plate with a self-locking screw, we considered using this method in combination with an anterior approach to the radius. We report our preliminary results. MATERIAL AND METHODS: From November 1 to March 31, 2003, among the 195 fractures of the distal radius with posterior displacement which we treated in our unit, 135 we

Multivectorial, external halo-assisted midface distraction in patients with severe hypoplasia.

Osteogenesis distraction is an increasingly used technique for the correction of facial deformities because it offers a faster and more controlled growth of the hypoplasic bone along with the elongation of the soft tissues. In this article, we describe the technique and our experience in midface distraction using an external multivectoral distraction device (BLUE Device; W. Lorenz, Jacksonville, FL) and custom-made midface osteotomies for the correction of severe hypoplasia.

The importance of the posteromedial fragment and its specific pinning in fractures of the distal radius.

In fractures of the lower end of the radius, the presence of a posteromedial fragment should be recognized if present. This fragment with two articular surfaces, if not reduced, will damage the radiocarpal and inferior radioulnar articulations. The diagnosis needs to be made on 3/4 X-ray views, as the fracture is often mistaken for a Colles' fracture. The author advises reduction and fixation by a method of double pinning. By comparing two sets of statistics, the overall results are shown to be improved without more complications.

Functional Outcome of Subtrochanteric Femoral Fractures Fixation by Proximal Femoral Locking Compression Plate.

Subtrochanteric femoral fractures are one of the common fractures encountered in today's Orthopaedic practice. This area consists of mostly cortical bone with high stress generation thus heal slowly which leads implant failure. The inherent instability of this fracture and forces of the muscles with comminuted medial calcar is giving the fracture a tendency to varus collapse. Intramedullary nails are not suitable for short proximal segment and wide medullary canal while extramedullary implants are associated with higher rate of implant failure. Recently proximal femoral locking compression pla

Collagen Structure, Synthesis, and Its Applications: A Systematic Review.

Resorbable collagen has been utilized to treat wounds, close graft, and tooth extraction sites, and enhance recovery. Collagen-based membranes are also used as barriers in periodontal and implant therapy to limit epithelial migration and allow cells with the regenerative capacity to fill the problem area. This systematic review was carried out to analyze the studies focusing on collagen structure, synthesis, and its applications. A detailed and extensive search was performed with the help of the keywords "collagen structure", "collagen synthesis" and "collagen applications". There was extensiv

Revision surgery occurs frequently after percutaneous fixation of stable femoral neck fractures in elderly patients.

BACKGROUND: Femoral neck fractures are a major public health problem. Multiple-screw fixation is the most commonly used surgical technique for the treatment of stable femoral neck fractures. QUESTIONS/PURPOSES: We determined (1) the proportion of hips that had conversion surgery to THA, and (2) the proportion of hips that underwent repeat fracture surgery after percutaneous screw fixation of stable (Garden Stages I and II) femoral neck fractures in patients older than 65 years and the causes of these reoperations. METHODS: We performed a retrospective study of all patients older than 65 year

Congenital Posteromedial Bowing of Tibia: Comparison of Early and Late Lengthening.

BACKGROUND: Congenital posteromedial bowing of tibia (CPMBT), a rare anomaly, is characterized by a decreasing deformity and a gradually increasing limb shortening. Lengthening in CPMBT has not been studied extensively. Our series compares the duration and complications of lengthening in younger versus older children to determine early lengthening safety and benefits. METHODS: We studied 28 tibial lengthenings performed by a single surgeon in 23 patients, divided into 2 equal groups of 14 segments: group A 5 years or younger (preschool) and group B above 5 years. All were lengthened with circ

Surgical treatment of tibial metastases: Retrospective, multicenter, observational study of 25 patients.

INTRODUCTION: Long bone metastasis to the tibia is somewhat rare and has only been studied in a few publications with a limited number of cases. This led us to carry out a large multicenter, observational, retrospective study to 1) evaluate the clinical and radiological outcomes of surgical treatment at this location and 2) highlight the specific risks associated with this condition. HYPOTHESIS: We hypothesized that the clinical outcomes and survivorship were comparable to those reported in the literature. MATERIAL AND METHODS: The case series included 15 men and 10 women with a mean age of

The changing pattern of pediatric both-bone forearm shaft fractures among 86,000 children from 1997 to 2009.

INTRODUCTION: Forearm fractures are common among children. Unlike most pediatric fractures, there is a risk of unsatisfactory results in forearm shaft fractures. The healing of a tubular bone is most unlikely in the diaphysis far away from the metaphyseal zones. The treatment of forearm shaft fractures is evolving. The purpose of the study was to analyze the pattern of forearm shaft fractures and their treatment in a population of children in recent years. MATERIALS AND METHODS: All the children (from 0 to 16 years) with a both-bone forearm shaft fracture (AO-segment 22-D) during the years 19
