Computational motility models of neurogastroenterology and neuromodulation.

The success of neuromodulation therapies, particularly in the brain, spinal cord, and peripheral nerves, has been greatly aided by computational, biophysical models. However, treating gastrointestinal disorders with electrical stimulation has been much less explored, partly because the mode of action of such treatments is unclear, and selection of stimulation parameters is often empirical. Progress in gut neuromodulation is limited by the comparative lack of biophysical models capable of simulating neuromodulation of gastrointestinal function. Here, we review the recently developed biophysical

Point-of-Care Sinus Ultrasound: Impact Within a Large Internal Medicine Clinic and Review of Technique.

Outpatient diagnosis of acute bacterial sinusitis, using only traditional physical examination and clinical criteria, results in the overuse of antibiotics in patients with upper respiratory complaints. Point-of-care maxillary sinus ultrasound is easy to learn and quick to perform in a primary care clinic. The technique can reduce antibiotic prescribing by reassuring both patients and providers of the absence of fluid in the sinus, the hallmark of maxillary sinusitis. A review of the literature, description of technique, and results of sinus ultrasound implementation in a large internal medici

Role of contact force-guided radiofrequency catheter ablation for treatment of atrial fibrillation: A systematic review and meta-analysis.

INTRODUCTION: CF-sensing catheter emerged as a novel ablation technology and was increasingly used in clinical practice. Nonetheless, available evidence of efficacy and safety comparison between CF-guided RF catheter ablation and non-CF-guided ablation for treatment of AF was still lacking. METHODS AND RESULTS: Twenty-two eligible studies were included after systematic review through the MEDLINE, Google Scholar, the Cochrane Library and PubMed databases. AF/atrial tachycardia-free survival was markedly improved in CF-guided catheter ablation compared with non-CF-guided ablation at a median 12

Endoscopic Management of Colonic Perforation due to Ingestion of a Wooden Toothpick.

BACKGROUND Toothpick ingestion is implicated in bowel injuries that may cause violent complications, mimicking diseases causing acute abdomen. CASE REPORT A 18-year-old man was admitted with a 3-day history of a swallowed wooden toothpick. The patient had tenderness in the left flank area. Computed tomography indicated toothpick impaction at the splenic flexura of the colon. It was successfully removed with colonoscopy. After the procedure, abdominal radiography showed free air as a sign of perforation. Along with conservative management, the patient was discharged without surgery. CONCLUSIONS

Can cyclist safety be improved with intelligent transport systems?

In recent years, Intelligent Transport Systems (ITS) have assisted in the decrease of road traffic fatalities, particularly amongst passenger car occupants. Vulnerable Road Users (VRUs) such as pedestrians, cyclists, moped riders and motorcyclists, however, have not been that much in focus when developing ITS. Therefore, there is a clear need for ITS which specifically address VRUs as an integrated element of the traffic system. This paper presents the results of a quantitative safety impact assessment of five systems that were estimated to have high potential to improve the safety of cyclists

[A rare case of an acute abdomen patient with gangrene of the colon as a complication of systemic lupus erythematosus].

The authors present a case report of a 39-year-old woman with acute abdomen - a comorbid patient with systemic lupus erythematosus, chronic renal insufficiency as a complication of lupus nephritis, included in a haemodialysis programme. The patient had also undergone transplantation of the left kidney in the past. She was initially admitted to the Department of Traumatology for a total endoprosthesis procedure due to bionecrosis of the head of the thigh bone. Postoperatively, the patients condition was complicated by gangrene of the colon confirmed by CT scan and during the operation. The pati

Neurologic manifestations of inflammatory bowel diseases.

Inflammatory bowel diseases (IBD) are chronic, relapsing and remitting inflammatory conditions affecting the digestive system, comprising two main distinctive diseases, ulcerative colitis (UC) and Crohn's disease (CD). Besides the classic gastrointestinal manifestations, a variable number of IBD patients present with extraintestinal manifestations, including central and peripheral nervous system involvement. Peripheral neuropathy is one of the most common complications. An inflammatory myopathy has also been found. Cranial neuropathies include the Melkersson-Rosenthal syndrome, optic neuritis,

Multimodal imaging in the diagnosis and evaluation of intestinal malrotations in adults: a case report.

Midgut malrotation is a congenital anomaly referring to either lack of or incomplete rotation of the fetal intestines around the axis of the superior mesenteric artery during fetal development. It is rare in adulthood and the true incidence is difficult to estimate because most patients are asymptomatic. The diagnosis is usually performed with several radiological and surgical methods. We report a case of a woman who presented with cramp-like abdominal pain localized to the right iliac fossa. The patient underwent abdominal ultrasound, radiological examination without and with contrast, and co

Artificial Neural Networks, and Evolutionary Algorithms as a systems biology approach to a data-base on fetal growth restriction.

One of the specific aims of systems biology is to model and discover properties of cells, tissues and organisms functioning. A systems biology approach was undertaken to investigate possibly the entire system of intra-uterine growth we had available, to assess the variables of interest, discriminate those which were effectively related with appropriate or restricted intrauterine growth, and achieve an understanding of the systems in these two conditions. The Artificial Adaptive Systems, which include Artificial Neural Networks and Evolutionary Algorithms lead us to the first analyses. These an

Metric characterization of the aortic arch of early mouse fetuses and of a fetus featuring a double lumen aortic arch malformation.

This study aimed at providing an objective metric characterization of the aortic arch of a mouse fetus featuring a double lumen aortic arch malformation. As a side effect it provides reference data defining the length and the diameters of the aortic arch segments of normally developed mouse fetuses at developmental stage 22 according to Theiler (TS22). We analyzed a total of 22 TS22 mouse fetuses of the Him:OF1 strain. We produced high-resolution three-dimensional (3D) computer models and measured the diameters and cross sectional areas of the aortic arch segments and of the ascending and desc

Visualization of spatiotemporal behavior of discrete maps via generation of recursive median elements.

Spatial interpolation is one of the demanding techniques in Geographic Information Science (GISci) to generate interpolated maps in a continuous manner by using two discrete spatial and/or temporal data sets. Noise-free data (thematic layers) depicting a specific theme at varied spatial or temporal resolutions consist of connected components either in aggregated or in disaggregated forms. This short paper provides a simple framework: 1) to categorize the connected components of layered sets of two different time instants through their spatial relationships and the Hausdorff distances between t

Neuroimaging in superficial siderosis: an in-depth look.

Despite extensive imaging, a source of bleeding is often not evident during the evaluation of patients with superficial siderosis (SS) of the central nervous system. An intraspinal fluid-filled collection of variable dimensions is frequently seen on spine MR imaging in patients with idiopathic SS. A similar finding has also been reported in patients with craniospinal hypotension. This review discusses the role of multitechnique imaging in the work-up of patients with SS and focuses on recent developments.

Designing an emergency medical information system for the early stages of disasters in developing countries: the human interface advantage, simplicity and efficiency.

The means of designing an effective user interface software package varies from one application to another. Almost the entire ICT infrastructure was damaged following the impact of the tsunami tidal wave. Under such circumstances, transporting critically ill patients is a must and becomes the first priority. Many considerations are needed when designing a specific user interface for emergency situations in developing countries. In this study we proposed how to design the user interface in order to support emergency medical care in the early stages of disasters. The user interface was classifie

Current state and future of joint replacements in the hip and knee.

Joint replacements of the hip and knee are among the most clinically successful operations. According to figures compiled by the American Academy of Orthopaedic Surgeons, the number of primary total hip replacements performed in the USA was 220,000 in 2003. This was 38% more than in 1996 and this number is expected to rise to 572,000 (plus another 97,000 revisions) by 2030. The number of primary total knee replacements performed in 2003 was approximately 418,000 and is expected to rise exponentially with the increasing numbers of baby boomers and the aging population. Current research focuses
