A family-focused approach is the most effective clinical method in the assessment and management of substance abuse in adolescents and their parents. The impact of alcoholism and substance abuse on parents and children, psychosocial risk factors, and indicators of alcoholism are important considerations when using this approach. Education regarding the family-focused approach to the treatment of substance abuse deserves a high priority in the training of pediatricians and other primary care physicians. As Doherty and Baird suggest," . . . the key to this training will not be . . . the developm

OBJECTIVES: The social context within which women engaged in sex work at a popular truck stop in South Africa are placed at risk of human immunodeficiency virus (HIV) infection and the factors that influence their ability to reduce their risk were assessed.
METHODS: Using qualitative and quantitative techniques, an elected sex worker from within the group collected all data.
RESULTS: Given the various pressing needs for basic survival, the risk of HIV infection is viewed as one more burden imposed on these women by their lack of social, legal, and economic power. Violence, or the threat ther

Co-trimoxazole is still widely used for indications where trimethoprim alone is equally effective. Microbiological and pharmacokinetic considerations reveal that trimethoprim alone provides adequate anti-microbial activity for treatment of conditions for which co-trimoxazole is often given. Synergy may be shown in vitro, but in clinical practice is an unusual occurrence. There is no evidence from clinical studies that the sulphonamide moiety fo co-trimoxazole prevents the development of resistance to trimethoprim. The adverse event profile of co-trimoxazole is a summation of that of sulphonami

Rapid socioeconomic development, urbanization and improved survival have given rise to a progressive increase in the occurrence of noncommunicable diseases in the Eastern Mediterranean Region. Cardiovascular diseases have emerged as a leading cause of morbidity and mortality in many countries. The prevalence of hypertension is already high in many countries of the Region and the number of hypertensives is likely to increase further in the coming years. Although the influence of geographical, ethnic and socioeconomic factors has not been studied adequately, it seems that the epidemiological and…

This study is an essential prerequisite to gain more insight into the complex relationship between public health and the costs of medical care. It offers a first tentative but comprehensive description of the total direct costs generated by all diseases in the Dutch population. We classified estimated direct costs of illness (39.8 thousand million guilders) by type of care, sex, age and 48 diagnostic categories for 1988. In order to estimate the costs of health care in the year 2030, we linked this information with demographic development. We were able to allocate 75% of all costs to diseases.

The paper examines various aspects of the growth monitoring (GM) component of the Integrated Child Development Services (ICDS) programme of India. Data of 3704 rural children, 0-6 years of age revealed that growth of almost half of the children had never been monitored, and monitored inadequately in another 25 per cent of cases. While very few mothers (1 per cent) could interpret growth charts (GC) correctly, 87 per cent of the ICDS functionaries could do this. There was a significant and positive relationship between maternal knowledge and functionary knowledge of the growth chart (GC), and c

PURPOSE, PATIENTS, AND METHODS: We used decision analysis to calculate the economic implications of routine testing for Neisseria gonorrhoeae infection of the cervix. This study compared performing endocervical cultures in all women seeking routine gynecologic care to a no-test strategy. Estimates derived from the medical literature were varied in a sensitivity analysis, and a Monte Carlo technique was used to incorporate the estimates of experts into the analysis.
RESULTS: We found that routinely testing for gonorrhea reduced overall medical costs when the prevalence of infection exceeded 1.

The highly differentiated thyroid cancers account for 0.70% of malignancies. Their management is primarily hormonal, in addition to the percutaneous irradiation and application of iodine isotope. This method is not aimed at the inhibition of the potential artificial hypothyreosis but mainly at the prevention of the tumour development or tumorous dissemination. The authors, in case of their 28 patients with the above diagnosis, tried to clarify if the TRH-tolerance TSH-level determination promotes the optimal patient management, the ideal hormone substitution lasting for several years. Parasitic arthropods are responsible for enormous economic losses to livestock producers throughout the world. These production losses may range from simple irritation caused by biting and non-biting flies to deaths and/or damage to carcass, fleece, or skin resulting from attack by myiasis flies. The estimated costs of these losses are colossal but even these usually include only direct losses and ignore those associated with pesticide application. In the USA alone (in 1976), these losses were conservatively estimated at more than 650 million US dollars. The long term use of chemical control m

With the development of nonsurgical methods of urinary diversion and the availability of new and more effective methods for treating cancer, an increasing number of patients will be candidates for palliative urinary diversion. Urinary diversion is clearly indicated to provide the time necessary to establish the correct diagnosis, to treat pain or sepsis, and to provide time for other treatments to control the underlying disease. When diversion is being considered solely for the purpose of treating uremia and the tumor has proved to be refractory to all known effective forms of treatment, the d

BACKGROUND: The societal costs of problematic class A drug use in England and Wales exceed £15B; drug-related crime accounts for almost 90% of costs. Diversion plus treatment and/or aftercare programmes may reduce drug-related crime and costs.
OBJECTIVES: To assess the effectiveness and cost-effectiveness of diversion and aftercare for class A drug-using offenders, compared with no diversion.
POPULATION: Adult class A drug-using offenders diverted to treatment or an aftercare programme for their drug use.
INTERVENTIONS: Programmes to identify and divert problematic drug users to treatment (

Tissue engineering is an emerging field within biomedicine, related to developing functional substitutes for damaged tissues or organs. Despite significant advancements, the development of effective engineering tissue constructs remains challenging, particularly when replicating elastic stretchability, which plays a critical role in many tissues. Therefore, the development of tough, elastomeric scaffolds that mimic the complex elasticity of native tissues, such as the myocardium, heart valves, and blood vessels, is of particular interest. This study aims to evaluate a flexible printable materi

The control of infectious diseases can be improved via carefully designed decontamination equipment and systems. Research interest in ozone (a powerful antimicrobial agent) has significantly increased over the past decade. The COVID-19 pandemic has also instigated the development of new ozone-based technologies for the decontamination of personal protective equipment, surfaces, materials, and indoor environments. As this interest continues to grow, it is necessary to consider key factors affecting the applicability of lab-based findings to large-scale systems utilizing ozone. In this review, w

Acquired immunodeficiency syndrome (AIDS) caused by the human immunodeficiency virus (HIV) continues to be a public health problem. In 2020, 680,000 people died from HIV-related causes, and 1.5 million people were infected. Antiretrovirals are a way to control HIV infection but not to cure AIDS. As such, effective treatment must be developed to control AIDS. Developing a drug is not an easy task, and there is an enormous amount of work and economic resources invested. For this reason, it is highly convenient to employ computer-aided drug design methods, which can help generate and identify nov

Obesity in childhood and adolescence continues to be a major health issue due to significant health implications and to the economic burden that arise from treating this disease and its complications. Current data show that childhood obesity is no longer just a concern for developed countries, but more significantly affecting developing countries. In adult population, cardiovascular disease is the main cause of mortality and morbidity among obese patients. It is therefore believed that risk factors found in adult patients could also be observed in obese youth. These risk factors will then pers

Unexpected toxicity in areas such as cardiotoxicity, hepatotoxicity, and neurotoxicity is a serious complication of clinical therapy and one of the key causes for failure of promising drug candidates in development. Animal studies have been widely used for toxicology research to provide preclinical security evaluation of various therapeutic agents under development. Species differences in drug penetration of the blood-brain barrier, drug metabolism, and related toxicity contribute to failure of drug trials from animal models to human. The existing system for drug discovery has relied on immort

The health of children improved dramatically worldwide during the 20th century, although with major contrasts between developed and developing countries, and urban and rural areas. The quantitative evidence on urban child health from a broad historical and comparative perspective is briefly reviewed here. Before the sanitary revolution, urban mortality tended to be higher than rural mortality. However, after World War I, improvements in water, sanitation, hygiene, nutrition and child care resulted in lower urban child mortality in Europe. Despite a similar mortality decline, urban mortality in

Rabies is a major public health problem in developing countries in Asia and Africa. Although a number of laboratory diagnoses can be used for rabies control, the WHO and OIE recommended gold standard for rabies diagnosis is the direct fluorescent antibody test (FAT). However, FAT is not widely used in developing countries because of deficient financial sources to procure fluorescent microscope. Recently the direct rapid immunohistochemical test (dRIT) has been developed and has a worldwide promising application, particularly in developing countries, since its result can be read by inexpensive
